OBJECTIVE: Respiratory regulation is critical for patients with respiratory dysfunction. Clinically used ventilators can lead to long-term dependence and injury. Extracorporeal assistance approaches such as iron-lung devices provide a noninvasive alternative, however, artificial actuator counterparts have not achieved marvelous biomimetic ventilation as human respiratory muscles. Here, we propose a bionic soft exoskeleton robot that can achieve extracorporeal closed-loop respiratory regulation by emulating natural human breath.
METHODS: For inspiration, a soft vacuum chamber is actuated to pr

Relative regional ventilation and perfusion were measured with xenon-133 in 16 seated patients with pure or predominant mitral stenosis (group 1) and in 12 patients with chronic pulmonary congestion due to left-sided heart disease without mitral stenosis (group 2). The apex-base perfusion gradient was abnormally reduced and often reversed in group 1 patients. There was a significant negative correlation between perfusion gradient and mean pulmonary capillary pressure in both groups, but for a given elevation of pulmonary capillary pressure the perfusion gradient tended to be greater (i.e., les

Central apneas during sleep are frequently of longer duration than predicted and persist despite high levels of chemical stimuli. We provide evidence that suggests that this apnea prolongation represents a central inertia in the control system mediated by reciprocal inhibition of inspiration.

The intrinsic tongue muscle, genioglossus, and soft palatal muscles, tensor palatini, levator palatini and palatoglossus, are known to exhibit phasic respiratory activity and to respond reflexly to negative pressure in man, which may be important in the maintenance of upper airway patency. We hypothesized that the palatopharyngeus muscle should also have respiratory activity and increased activity in response to negative upper airway pressure. We have, therefore, examined the palatopharyngeus and the antagonist muscle, levator palatini, in eight awake sleep apnoea patients, using bipolar elect

The article by Markel in the new Historical Perspectives of Pediatrics section of the ARCHIVES brings back a flood of memories, as I belong to the generation of pediatricians who actually used tank respirators in treating patients with poliomyelitis. In 1949, the city of Baltimore, Md, closed its contagious disease hospital (Sydenham), which served all of Maryland, and transferred its function to the Baltimore City Hospitals. The reasoning, in addition to a very low occupancy rate at Sydenham, was that the only contagious disease then likely ever to occur in epidemic form was poliomyelitis.

The authors measured antidiuretic hormone (ADH) excretion and renal and cardiovascular function in eight patients with flail chest during four ventilatory conditions: continuous positive-pressure ventilation (CPPV), intermittent positive pressure ventilation (IPPV), spontaneous breathing with continuous positive airway pressure (CPAP) and spontaneous breathing (SB). Each condition was maintained for six to eight days. ADH excretion was significantly greater during CPPV (181 +/- 14 ng/24 h, mean +/- SE) than during IPPV (86 +/- 10), CPAP (181 +/- 14 ng/24 h, mean +/- 6). Free-water clearance wa

The afferent pathways and upper airway receptor locations involved in negative upper airway pressure (NUAP) augmentation of soft palate muscle activity have not been defined. We studied the electromyographic (EMG) response to NUAP for the palatinus, tensor veli palatini, and levator veli palatini muscles in 11 adult, supine, tracheostomized, anesthetized dogs. NUAP was applied to the nasal or laryngeal end of the isolated upper airway in six dogs and to four to six serial upper airway sites from the nasal cavity to the subglottis in five dogs. When NUAP was applied at the larynx, peak inspirat

Flail chest after blunt trauma usually requires good pain control and positive pressure support. Continuous negative extrathoracic pressure (CNEP) causes a splinting effect around the anterior chest wall and upper abdomen which increases functional residual capacity and improves lung mechanics. We report an 82-year-old woman with flail chest after cardiopulmonary resuscitation. She underwent mechanical ventilation, with two failed attempts at extubation. She was subsequently ventilated non-invasively using CNEP, allowing early successful extubation. This case illustrates the use of CNEP for we

In spontaneous breathing intrathoracic pressure alternates between positive and negative in a biphasic sequential pattern. By contrast, during mechanical ventilation (IPPV, CPPV) the intrathoracic pressure remains above atmospheric all the time. Due to these unphysiological conditions there are extensive causal and side effects on the lung parenchyma and other organs. Errors in the artificial ventilation technique can magnify these effects. In order to minimize these deleterious effects of positive pressure ventilation it is essential to keep the procedure as short and little invasive as possi

1. The hypermetabolism frequently observed at rest in patients with chronic obstructive pulmonary disease has been attributed to a high cost of breathing. However, measurement of the cost of breathing by the usual hyperventilation procedure is fraught with methodological problems. The purpose of this study was to measure more directly the cost of breathing in a group of ambulatory patients with stable chronic obstructive pulmonary disease. 2. The cost of breathing was calculated as the difference in oxygen consumption measured by indirect calorimetry between spontaneous breathing and noninvasi

Acute ventilatory insufficiency is characterized by hypercapnia, respiratory acidosis and secondary hypoxemia. The primary target of mechanical ventilation is improvement of alveolar ventilation, that means compensation of the ventilatory insufficiency. Noninvasive ventilation started as ventilatory support during the big polio epidemic, at that time in form of negative pressure ventilation. In the last two decades NIV is in form of positive pressure ventilation important for long-term ventilation at home, but there is also growing importance of NIV in the treatment of acute respiratory insuff

Middle ear ventilation was investigated by repeated tympanometry in different groups of patients with impaired air current in the upper airways. Among patients with total blocking of the nose by bilateral anterior packing, 46% had a negative pressure of -100 mm H2O or less, which quickly returned to normal after removal of the packing. Unilateral anterior packing produced no significant changes in middle ear ventilation. Unilateral posterior packing produced changes in 50% of ears, tonsillectomy in 60% of ears, amounting to a negative pressure of -100 mm H2O or less. Patients with infectious m

BACKGROUND AND OBJECTIVES: Negative pressure pulmonary edema occurs by increased intrathoracic negative pressure following inspiration against obstructed upper airway. The pressure generated is transmitted to the pulmonary capillaries and exceeds the pressure of hydrostatic equilibrium, causing fluid extravasation into the pulmonary parenchyma and alveoli. In anesthesiology, common situations such as laryngospasm and upper airway obstruction can trigger this complication, which presents considerable morbidity and requires immediate diagnosis and propaedeutics. Upper airway patency, noninvasive

Recently, interest in the use of mechanical ventilation outside the hospital setting has been increasing. Patients with various types of chronic respiratory failure may benefit from this approach. Evaluation for long-term mechanical ventilation necessitates assessment of the underlying disease process, the goals of the medical team, and the needs of the patient and family. Externally applied negative-pressure devices can provide adequate ventilation for many patients, particularly those with neuromuscular diseases. Positive-pressure ventilation by means of a tracheostomy provides greater contr

BACKGROUND: The evolution of compliance and driving pressure in ARDS and the effects of time spent on noninvasive respiratory support prior to intubation have not been well studied. We conducted this study to assess the effect of the duration of noninvasive respiratory support prior to intubation (ie, noninvasive ventilation [NIV], high-flow nasal cannula [HFNC], or a combination of NIV and HFNC) on static compliance and driving pressure and retrospectively describe its trajectory over time for COVID-19 and non-COVID-19 ARDS while on mechanical ventilation.
METHODS: This is a retrospective an

A 52-year-old male with a ruptured right hydatid cyst underwent thoracotomy and cyst excision. After repair of diffuse parenchymal air leaks, application of negative pleural suction caused sudden ventilatory collapse with refractory hypotension, unresponsive to tube exchange or vasopressors. Lung-protective ventilation failed until clamping the drain immediately restored tidal volumes, confirming suction-mediated negative-pressure failure. Ultrasound showed good re-expansion without pneumothorax; subsequent pink frothy secretions suggested re-expansion/negative-pressure pulmonary oedema. Switc

Five patients with chronic respiratory failure from neuromuscular disease and symptomatic worsening nocturnal hypoventilation were treated with nocturnal ventilation. Home ventilation at night was provided by a volume-cycled positive pressure ventilator attached to a nasal mask originally designed to administer nasal continuous positive airway pressure (CPAP) for obstructive sleep apnea. The device was well tolerated. Symptoms of headache, insomnia/somnolence, and impaired intellectual capacity rapidly disappeared with nocturnal ventilatory support. Daytime arterial PO2 and PCO2 improved after

Mechanical ventilation with high peak inspiratory pressure and large tidal volume (VT) produces permeability pulmonary edema. Whether it is mean or peak inspiratory pressure (i.e., mean or end-inspiratory volume) that is the major determinant of ventilation-induced lung injury is unsettled. Rats were ventilated with increasing tidal volumes starting from different degrees of FRC that were set by increasing end-expiratory pressure during positive-pressure ventilation. Pulmonary edema was assessed by the measurement of extravascular lung water content. The importance of permeability alterations

PURPOSE: Patients with obstructive sleep apnea syndrome (OSAS) have difficulties in compliance with continuous positive airway pressure (CPAP) and the treatment outcome is heterogeneous. We proposed a proof-of-concept study of a novel intermittent negative air pressure (iNAP®) device for physicians to apply on patients who have failed or refused to use CPAP.
METHODS: The iNAP® device retains the tongue and the soft palate in a forward position to decrease airway obstruction. A full nightly usage with the device was evaluated with polysomnography. Subgrouping by baseline apnea-hypopnea index (
