Myosin inhibitor therapy in Fabry cardiomyopathy with dynamic left ventricular outflow obstruction.

A 56-year-old woman with presumed hypertrophic cardiomyopathy was diagnosed with Fabry disease (GLA: p.Glu358del) and a concomitant PLN variant. Imaging revealed advanced Fabry cardiomyopathy with dynamic LVOT obstruction. Despite optimized medical and enzyme replacement therapy, symptoms progressed (NYHA-class III) with a marked increase in LVOT gradient. Mavacamten was initiated and uptitrated under echocardiographic monitoring. At three months, LVOT gradient significantly decreased and functional status improved (NYHA-class I), while systolic function remained preserved. This case highlight

What every intensivist should know about dynamic left ventricular outflow tract obstruction.

Dynamic left ventricular outflow tract obstruction (LVOTO) is an underrecognized, load dependent cause of hemodynamic instability in critically ill patients. Although classically associated with hypertrophic cardiomyopathy, it frequently occurs in the ICU in settings such as septic shock, hypovolemia, postoperative states, and right ventricular failure. Standard shock therapies, including inotropes and afterload reduction, may paradoxically worsen obstruction and lead to escalating hemodynamic instability. Bedside echocardiography is central to diagnosis, typically revealing a small hyperdynam

Continuous Assessment of Left Ventricular Outflow Obstruction in Transcatheter Aortic Valve Implantation.

BACKGROUND: Left ventricular outflow tract (LVOT) obstruction is a rare but fatal complication after transcatheter aortic valve implantation (TAVI). Early detection and timely intervention are essential for favorable clinical outcomes. CASE SUMMARY: We report a case of LVOT obstruction diagnosed and dynamically monitored via a SavvyWire (Haemonetics). A 78-year-old woman underwent transfemoral TAVI for severe valve stenosis, and continuous hemodynamic monitoring by the SavvyWire identified a significant aorto-left ventricular pressure gradient with systemic hypotension immediately after valve

Left Ventricular Outflow Tract Pseudoaneurysm Leading to Dynamic Left Main Artery Obstruction.

Left ventricular outflow tract pseudoaneurysm is a rare complication from multiple etiologies. We present a unique case of a patient with a history of Neisseria gonorrhoeae infective endocarditis requiring bioprosthetic aortic valve replacement. Subsequently, this patient developed acute coronary syndrome with dynamic left main coronary artery compression due to a left ventricular outflow tract pseudoaneurysm.

Case report: obstruction in the right ventricular outflow tract due to hemolymphangioma.

Cardiac masses commonly involve primary tumors, metastatic diseases, and nonbacterial thrombotic and infective endocarditis. myxomas are the most common primary tumors, accounting for 75%. Hemolymphangiomas are a group of congenital vascular and lymphatic malformations that originate from the mesenchyme, with an incidence rate of 0.12%-0.28% per year. Hemolymphangiomas have been found in the rectum, small intestine, spleen, liver, chest wall, and mediastinum but have not yet been reported in the ventricular outflow tract in the heart. Herein, we report a case of a hemolymphangioma tumor in the

Flail subaortic membrane mimicking left ventricular outflow tract obstruction in hypertrophic cardiomyopathy.

A subaortic membrane is an uncommon cause for left ventricular outflow tract obstruction. Hypertrophic cardiomyopathy with dynamic left ventricular outflow tract obstruction would mask the presence of the subaortic membrane on transthoracic echocardiography and cause a false diagnosis. We report a patient with subaortic stenosis due to flail subaortic membrane misdiagnosed as obstructive hypertrophic cardiomyopathy on transthoracic echocardiography, identified on transesophageal echocardiography and cardiac catheterization.

Accessory mitral valve without left ventricular outflow tract obstruction in an adult.

Accessory mitral valve is a rare congenital abnormality and an unusual cause of subvalvular obstruction of the left ventricular outflow tract. Accessory mitral valves are usually detected in children due to symptomatic obstruction; isolated nonobstructive accessory mitral valve is rarely seen in adults. We describe the echocardiographic diagnosis of accessory mitral valve as an isolated congenital anomaly not associated with a substantial degree of obstruction of the left ventricular outflow tract in an asymptomatic adult patient. This case highlights the importance of transthoracic and transe

Repair of complex left ventricular outflow tract obstruction with a pulmonary autograft.

The surgical relief of complex multilevel left ventricular outflow tract (LVOT) obstruction remains a challenging surgical problem. We present a new operation which combines the concepts of aortoventriculoplasty, extended aortic root replacement and the use of a pulmonary autograft. Sixteen patients underwent this operation: 11 patients after previous attempts to relieve diffuse subvalvular stenosis and five patients who presented excessive gradients over an outgrown aortic valve prosthesis. All patients, except one survived the operation. One patient developed massive right ventricular infarc

Intravascular stents for management of pulmonary artery and right ventricular outflow obstruction.

This study was performed to determine the efficacy of balloon-expandable stents in the treatment of branch pulmonary artery-stenoses and conduit stenosis in children. A total of eight stainless steel stents were implanted in seven patients. Three patients had tetralogy of Fallot with pulmonary artery stenosis following total correction, one patient had conduit stenosis following correction of transposition of the great arteries, one patient had intra-cardiac conduit stenosis after septation for single left ventricle, and two patients had pulmonary artery stenosis after Fontan operation. Six st

Diagnosis, transport, and outcome in fetuses with left ventricular outflow tract obstruction.

Between March 1986 and April 1990, 22 consecutive fetuses (at gestational ages of 21 to 38 weeks) with a suspected diagnosis of critical (ductus-dependent) left ventricular outflow tract obstruction on fetal echocardiogram were referred to our center for delivery and surgical treatment. Diagnoses were hypoplastic left heart syndrome (n = 16), valvular aortic stenosis (n = 2), common atrioventricular canal with subaortic stenosis (n = 3), and single ventricle with subaortic stenosis (n = 1). Postnatal echocardiography revealed that fetal echocardiography was correct in predicting left ventricul

Extended aortic root replacement for treatment of left ventricular outflow tract obstruction.

Recurrent tunnel stenosis of the left ventricular outflow tract following operation for subaortic stenosis and hypoplastic aortic annulus remain a challenge for pediatric cardiac surgeons. We have recently applied a new technique of extended aortic root replacement using an aortic allograft to treat three patients who had previously been operated upon for subaortic stenosis and three who had aortic stenosis with a hypoplastic aortic annulus. This new procedure combines the concept of aortoventriculoplasty with allograft aortic root replacement and coronary artery reimplantation. The valved aor

Diagnosis of right ventricular outflow obstruction in infants by cross sectional echocardiography.

Cross sectional echocardiographic studies were assessed prospectively in 58 infants in whom right ventricular outflow obstruction was subsequently shown angiographically. A subcostal cut was used to display simultaneously the short axis of the aortic root and the long axis of the right ventricular outflow tract. This facilitated the differentiation of the common right ventricular outflow tract obstructive lesions. Tetralogy of Fallot was diagnosed correctly in 22 of 26 infants; pulmonary atresia with intact septum in all of 14 neonates; isolated severe pulmonary valve stenosis in all of nine i…

Angiographic evidence of absent ductus arteriosus in severe right ventricular outflow obstruction.

The angiocardiograms of 5 newborn infants with autopsy and/or surgically-proven congenital absence of the ductus arteriosus (ADA) and right ventricular outflow obstruction (Group A), and of 14 neonates with pulmonary atresia complex and patent ductus arteriosus (Group B) were reviewed. Aortic size was similar in both groups; however, the diameters of the right and left pulmonary arteries were much smaller in Group A than in Group B (right pulmonary artery: 2.6 vs 4.5 mm, P less than 0.005; left pulmonary artery: 2.5 vs 4.3 mm, P less than 0.005). Extensive bronchial collaterals were observed i…

Unexpected Severe Cyanosis Following Ventricular Septal Defect Closure and Repair of Right Ventricular Outflow Tract Obstruction.

Ventricular septal defect (VSD) is a common congenital heart defect that often requires surgical closure. We present the case of a seven-month-old infant with a large perimembranous VSD, moderate pulmonary valve stenosis, and hypertrophied right ventricular muscle bundles who failed to wean from cardiopulmonary bypass due to severe cyanosis after VSD closure and muscle bundle resection. An intraoperative transesophageal echocardiogram was performed with an agitated saline test, and a large atrial-level right-to-left shunt was seen. Return to cardiopulmonary bypass revealed an unroofed coronary
