BACKGROUND AND PURPOSE: In this study we analyzed the value of ultrasound examination for diagnosis of vertebral artery dissection.
METHODS: The vertebrobasilar arterial system was assessed in 14 patients using transcranial and extracranial pulsed-wave Doppler and duplex sonography.
RESULTS: The dissections were verified by angiography (in 1 patient), magnetic resonance imaging (in 5), or both (in 8). The dissected segments were atlantoaxial (V-3) in 6, V-3 and intertransverse (V-2) in 3, V-3 and intracranial (V-4) in 3, and V-2 in 2 patients. Extracranial and transcranial Doppler examinatio…

A 34 year old woman developed cerebral infarction, following a tonic-clonic seizure. A vertebral artery dissection was demonstrated and may have been caused by arterial trauma during the seizure. This cause of morbidity following convulsive seizures may have been overlooked in the past and needs to be recognised in view of the potential benefits of anticoagulation.

We describe three cases of extracranial vertebral artery dissection that are unusual in both their modes of presentation and their associations with other pathologic conditions. The first patient had Marfan's syndrome and migraine; his dissection was asymptomatic and was diagnosed by chance at the time of repeat angiography following a previous internal carotid artery dissection. The second patient had systemic lupus erythematosus and presented with a subarachnoid hemorrhage attributed to an intracranial vertebral artery dissection by the demonstration of an extracranial dissection. The third …

Spontaneous (nontraumatic) dissection of the carotid and vertebral arteries, once thought to be rare, is being recognized and reported more frequently. We report one case of spontaneous dissection of the vertebral artery initially diagnosed by magnetic resonance imaging and subsequently confirmed by angiography.

BACKGROUND: The reported actual risk of rupture for vertebral artery dissection (VAD) in patients presenting with headache is very low, ranging from 0.4% to 1.0%. The authors report a case in which the dissection site dilated rapidly within several hours after the dissection occurred resulting in subarachnoid hemorrhage (SAH).
OBSERVATIONS: A 49-year-old healthy man who had participated in a marathon noticed a headache while running. Magnetic resonance imaging (MRI) performed 2 days later revealed no findings suspicious for right VAD, but a string sign was observed in the left side, suggestin…

This paper presents the case report of an 11-year-old boy with an acute dissection with thrombosis of the left vertebral artery and thrombos … treatment and subsequent oral warfarin therapy, resulted in a lasting improvement of the symptoms. Vertebral artery dissection after minor trauma is rare in children. While acute basilar artery occlusion as a complication is even more infrequent, it is potentially fatal, which means that prompt diagnosis and treatment are imperative. The lack of class I recommendation guide …

Vertebral artery dissection is one of the common causes of childhood stroke. It can be either spontaneous or traumatic. It commonly occurs after a strenuous exercise. Clinical examination will not provide any evidence of the condition as the condition may be asymptomatic or present with only mild symptoms. The gold standard for detection is angiography. The treatment involves antithrombotic medications. Here we present the case of a 16-year-old female child who presented with sudden onset of paralysis secondary to vertebrobasilar ischemic stroke due to arterial dissection.

BACKGROUND: Vertebral artery dissection (VAD) is a potentially catastrophic injury that may occur during sports participation. A comprehensive review is needed to collate documented cases to improve understanding and inform future preventative approaches.
OBJECTIVE: This review aimed to understand the extent of VAD in sport and characterise trends suggestive of mechanisms of injury.
METHODS: Electronic databases were searched using terms related to VAD and sport. Records were included if they described one or more cases of VAD attributed to sport.
RESULTS: A total of 79 records described 12…

Vertebral artery dissection is of special clinical importance because of its often-delayed presentation and the risk of potentially permanent neurological deficit, or even death, as a result of injury. Once a rarely discovered injury, the better availability and use of computed tomography and magnetic resonance imaging have contributed to an increased incidence. Early diagnosis and treatment can almost eliminate the threat of acute cerebral vascular injury and save lives. In this report, we review a case of delayed traumatic vertebral artery dissection and discuss the key clinical findings and…

Vertebral artery dissection following a posterior cervical foraminotomy with rhizolysis of the subaxial spine has not been described before. A 46-year-old lady underwent the procedure for a left C6 radiculopathy with a focal disc herniation with no intraoperative complications. Seven hours post-operatively, she developed a right homonymous hemianopia, thalamic dysphasia, gait and memory impairment. Imaging demonstrated an occlusion to the left vertebral artery from C7/T1 to C4 with a dissection flap noted at the inferior margin. This was further complicated with thrombosis of the dissected art…

Rarely, not stroke but peripheral weakness can result from cervical artery dissection. In these cases, a mural hematoma compressing the ipsilateral C5 and/or C6 root can be demonstrated.

Bow hunter's syndrome is characterized by transient vertebrobasilar insufficiency that is elicited by neck rotation. This syndrome is has various causes, such as osteophytes, tumors, fibrous bands, infection, and trauma. We report a unique case of bow hunter's syndrome. The patient visited our hospital because of left nuchal pain. A magnetic resonance imaging scan revealed left vertebral artery (VA) dissection, which was the cause of his nuchal pain. He began to feel faintness upon turning his neck to the left after left VA dissection. Digital subtraction angiography revealed that the right VA

Vertebral artery dissection (VAD) is rare in children but is increasingly recognized as a cause of stroke in the pediatric population. Traditionally, VAD was thought to be attributable to either trauma or spontaneous dissections. Recently, several underlying causes, such as bony cervical abnormalities, connective tissue diseases, and infection, have been determined to account for spontaneous VAD or those cases associated with only minor trauma. Two pediatric cases of VAD are presented, both caused by bony cervical abnormalities and each treated with different surgical procedures for symptom re…

UNLABELLED: Craniovertebral anomalies are rare causes of vertebral artery dissection. Therefore, appropriate evaluation is necessary in vertebrobasilar strokes, specially in the young. This is particularly true for patients with vertebral stroke without risk factors.
RESULTS: Here we report a 24-year-old male presenting with posterior circulation infarcts. The patient had no obvious vascular risk factors. However, digital subtraction angiography showed chronic dissection of both vertebral arteries with partial recanalization. Further evaluation with computed tomogram of neck revealed atlantoa…

We report a case of spontaneous vertebral artery dissection (VAD) in a patient who developed extensive subcutaneous emphysema following the removal of a chest tube after a cardiac transplant. The pathophysiology and management of this uncommon complication are reviewed. Although vertebral and carotid artery dissections are unusual events occurring in 2.5 to 3 per 100,000 people, they are increasingly acknowledged to be important causes of stroke in the young and middle-aged adult population accounting for up to 25% of such cases. VADs are associated with a variety of minor traumatic mechanisms…

Dissection of the extracranial carotid and vertebral arteries is increasingly recognized as a cause of transient ischemic attacks and stroke. The annual incidence of spontaneous carotid artery dissection is 2.5 to 3 per 100,000, while the annual incidence of spontaneous vertebral artery dissection is 1 to 1.5 per 100,000. Traumatic dissection occurs in approximately 1% of all patients with blunt injury mechanisms, and is frequently initially unrecognized. Overall, dissections are estimated to account for only 2% of all ischemic strokes, but they are an important factor in the young, and accoun…
