The effect of the endolymphatic subarachnoid shunt operation on vestibular function.

Preoperative vestibular function was studied in 56 patients undergoing the endolymphatic subarachnoid shunt procedure for Ménière's disease … g relief of vertigo. The results of surgery could be predicted more accurately using the results of vestibular function tests than audiometric data. It appears that for best results the endolymphatic subarachnoid shunt procedure should be performed early in the course of Ménière's disease when hearing is fluctuating and vestibular function is normal.

MRI of the vestibular nerve after selective vestibular neurectomy.

CONCLUSION: In patients with Ménière's disease and persisting vertigo attacks after vestibular neurectomy (VNx) MRI of the vestibulocochlear nerve can identify residual vestibular nerve fibres that could be responsible for the vertigo attacks. OBJECTIVE: To test if MRI of the vestibulocochlear nerve can corroborate the presence of residual vestibular nerve fibres in patients with persisting vertigo attacks and residual vestibular function after VNx. MATERIALS AND METHODS: Vestibulocochlear nerve bundles of seven post-VNx unilateral Ménière's patients were imaged using 1.5 Tesla MRI with stea

Evaluation of vestibular function by dynamic posturography and other equilibrium examinations.

We evaluated the significance of dynamic posturography (EquiTest) in vestibular function assessment by comparing it with our routine equilibrium examinations comprised of nystagmographic data and other postural tests. Fifty-two patients with unilateral peripheral vestibular disorders were examined. Differences in test results between pendular rotation test and dynamic posturography were found in 23 out of 52 (44%) patients with unilateral peripheral vestibular disorders. In 5 patients, no abnormality of vestibular function could be detected by means of a three-test battery (spontaneous and pos

A distributed computer system for hierarchical control of a clinical vestibular laboratory.

A vestibular testing laboratory with five test stations was implemented as a clinical research and testing facility, automating and adapting tests for patients with inner-ear balance disorders. These were designed to quantitate postural control, and eye movement response to rotational, caloric, and visual (optokinetic) stimuli. Microprocessors were used as satellite processors in a star network under hierarchical control of a host minicomputer. All disk storage was controlled by the host, with data transfer from satellites via parallel interfaces. Network software was designed and implemented

Nicotine Effects on Vestibular Function in Pilot Selection.

INTRODUCTION: An optimal spatial orientation is essential for pilots. Therefore, pilot applicants in the Royal Danish Air Force undergo various ear-nose-throat evaluations. It is recommended to avoid alcohol for a minimum of 24 h before evaluation. No recommendation has been made regarding nicotine and, with the increasing availability of nicotine through pouches and other products, it is therefore relevant to examine its impact on vestibular function, particularly on nystagmus, which plays a key role in clinical vestibular assessment. METHODS: We conducted a structured literature search usin

Vestibular Loss and Cerebellar Ataxia: A Practical Approach.

Cerebellar ataxia has been the remit of neurology but increased awareness of genetic disorders affecting both cerebellar and vestibular function has led to a rise in their reported prevalence. It is not uncommon for such patients to be seen in otolaryngology clinics. This review explores the underlying etiologies of patients presenting with vestibular loss accompanied by signs of cerebellar ataxia and provides a practical approach to diagnosis. We provide a comprehensive overview of common conditions that can manifest with both vestibular dysfunction and cerebellar ataxia, acutely or chronical

Clinical implications of head-shaking nystagmus in central and peripheral vestibular disorders: is perverted head-shaking nystagmus specific for central vestibular pathology?

BACKGROUND AND PURPOSE: The patterns of head-shaking nystagmus (HSN) aid in differentiation between central and peripheral vestibular disorders, and perverted HSN (pHSN) has been considered a central sign. The aim was to determine the characteristics of HSN in a large number of patients with either peripheral or central vestibular disorders in a dizziness clinic of a university hospital. METHODS: The medical records of 7544 dizzy patients were reviewed during a year and 822 patients with a clinical diagnosis of vestibular disorders were recruited. The findings of spontaneous nystagmus (SN) an

Laboratory testing of the vestibular system.

PURPOSE OF REVIEW: Recent reports on vestibular testing, relevant to clinical diagnosis, are reviewed.Besides the case history and bedside examination, objective measurement of the vestibuloocular reflex in all of its facets remains the cornerstone in the diagnostic process. RECENT FINDINGS: In recent years, this has been enhanced considerably by reliable unilateral tests for the otolith organs, most notably by vestibular-evoked myogenic potential recording and estimation of subjective visual vertical. In addition, progress has been made in the investigation of multisensory interaction, invol
