Cost of Veteran Coronary Artery Bypass Grafting: Veterans Health Administration Versus Community Care.

INTRODUCTION: The community care program within the Veterans Health Administration (VHA) funds veteran care in the community. Cost of this program has increased with limited comparisons between VHA and community care hospitals. We seek to compare differences in cost of coronary artery bypass grafting (CABG) between VHA and community care hospitals. METHODS: We perform a retrospective cohort review of veterans undergoing CABG between October 1, 2018, and September 30, 2020, within VHA and community care hospitals within Veteran Integrated Service Network 19. Cost of index hospitalization was m…

Effects of TBI history and sleep on subjective cognition in Veterans: A VA Million Veteran Program study.

Traumatic brain injury (TBI) is a major public health issue often linked to cognitive dysfunction, with poor sleep further compounding these difficulties. While poor sleep has been shown to exacerbate post-TBI cognitive deficits, its role among Veterans remains understudied. This study examined the association between TBI history and poor sleep, as well as the interaction between TBI and sleep on subjective cognitive symptoms in a sample of 6959 Iraq/Afghanistan-era Veterans (77 % male) enrolled in the VA Million Veteran Program (MVP). Veterans completed the TBI Screening and Evaluation Progra…

Shallow Subsidies for Veterans Facing Housing Barriers in the VA Supportive Services for Veteran Families Program.

The Department of Veterans Affairs provides a shallow subsidy (i.e., subsidizing 50% of an individual's rent for two years) to Veterans experiencing housing instability. We sought to describe the characteristics of Veterans who received these subsidies. Methods. We conducted a retrospective cohort study of Veterans between 10/2019-9/2021. We identified Veteran-level characteristics associated with receiving a shallow subsidy using a multivariable two-part regression model. We also conducted qualitative interviews to identify how shallow subsidies are allocated. Results Black race, higher incom…

Association between suicide attempts and homelessness in a population-based sample of US veterans and non-veterans.

BACKGROUND: Suicide and homelessness share many of the same risk factors, but there is little understanding of how they are related to each other. METHODS: Data on 36 155 US adults (3101 veterans and 33 024 non-veterans) in the National Epidemiological Survey of Alcohol and Related Conditions-III were analysed to examine the association between lifetime homelessness and suicide, net of other factors, in a nationally representative US sample. RESULTS: US veterans with homeless histories were 7.8 times more likely to have attempted suicide than veterans with no homeless histories (24.5% vs 2.8…

Suicide risk factors among veterans: risk management in the changing culture of the Department of Veterans Affairs.

Suicide risk management in the Department of Veterans Affairs (VA) health care system is particularly challenging because of both patient characteristics and aspects of the delivery system. The prototypical suicide-prone person is an older white male with alcoholism, depression, physical problems, and poor psychosocial support. This describes a large portion of the veteran patient population. Suicide risk factors that are common in VA patients include male gender, older age, diminished social environment support (exemplified by homelessness and unmarried status), availability and knowledge of …

The prevalence of physical and sexual abuse in women veterans seeking care at a Veterans Affairs Medical Center.

Physical and sexual abuse are increasingly recognized as common harmful experiences for women. We surveyed 828 women veterans seeking care at the Baltimore Veterans Affairs Medical Center to determine the prevalence of physical and sexual abuse experiences, both during and outside of military service. Data were collected through an anonymous, mailed questionnaire, with a response rate of 52%. Sixty-eight percent of respondents reported at least one form of abuse, and 27% reported all three. Sexual abuse was most common (55%), followed by physical abuse (48%), and rape (41%). Enlisted women, wo…

Exploring veteran and non-veteran nursing student satisfaction with life utilizing marginality and financial stress.

BACKGROUND: The projected nursing shortage in the United States is 500,000 by 2030, and approximately 40,000 veterans who are attending nursing school in the United States can contribute to reducing this shortage. Nursing students experience numerous internal and external stressors during school which can contribute to high levels or anxiety, delayed graduation, or even dropout. This problem can ultimately have a negative effect on the number of nurses entering the workforce, worsening the shortage. PURPOSE: This study explored the concepts of financial stress, marginality, and satisfaction w…

Capitalizing on strengths and minimizing weaknesses of veterans in civilian employment interviews: Perceptions of interviewers and veteran interviewees.

Like all job applicants, veterans have to face the ubiquitous employment interview and pass this potential hurdle to civilian sector employment. So, because of the uniqueness of transitioning from the military to civilian employment, the present paper sought to identify perceived interviewing strengths and weaknesses of veteran interviewees from (a) the perspective of civilian sector human resource professionals (i.e. hiring personnel) with experience interviewing veterans (Study 1, five focus groups, N = 14), and (b) veterans (Study 2, N = 93). Qualitative analysis of the focus group transcri…

Impact of an unannounced standardized veteran program on access to community-based services for veterans experiencing homelessness.

BACKGROUND: The United States Department of Veterans Affairs established a program in which actors incognito portray veterans experiencing homelessness with pre-determined needs to identify barriers to access and services at community-based organizations. METHODS: From 2017 to 2019, actors who varied in gender, skin color and age portrayed one of three scripts at all VA Community-Based Resource and Referral Centers (CRRCs) serving veterans experiencing homelessness in 30 cities and completed an evaluative survey. They carried authentic VA identification and were registered in a VA patient dat…

BACKGROUND: United States veterans face an even greater risk of homelessness and associated medical conditions, mental health conditions, and fatal and nonfatal overdose as compared with nonveterans. Beginning 2009, the Department of Veterans Affairs developed a strategy and allocated considerable resources to address veteran homelessness and the medical conditions commonly associated with this condition. OBJECTIVE: This study aimed to examine the Veterans Health Administration National Center for Patient Safety database for patient safety events in the homeless veteran population to mitigate…

Antihypertensive Deprescribing in Older Adult Veterans at End of Life Admitted to Veteran Affairs Nursing Homes.

OBJECTIVES: Geriatric palliative care approaches support deprescribing of antihypertensives in older nursing home (NH) residents with limited life expectancy and/or advanced dementia (LLE/AD) who are intensely treated for hypertension (HTN), but information on real-world deprescribing patterns in this population is limited. We examined the incidence and factors associated with antihypertensive deprescribing. DESIGN: National, retrospective cohort study. Safer Housing for Homeless Women Veterans.

Homeless women veterans face unique vulnerability and significant mental health needs; it is important for their housing to include gender-specific safety measures. Providers of supportive housing for veterans can take important steps to accommodate the women they serve, including providing separate housing facilities or areas for women. More than half of all homeless women veterans were sexually assaulted during their military service, and many exhibit mental health disabilities as a result, which provides a strong legal basis for requiring gender-based accommodations. While significant progr…
