OBJECTIVE: The objective of this retrospective study was to determine if groin radiation was superior to no therapy in patients with small vulvar cancer with not palpable or not suspicious inguinal lymph nodes (T1, N0-N1).

The subject of this report is a surgical method, CO2 laser, for the treatment of non-invasive vulvar diseases. The combination therapy consists of skinning resection of the vulva of the central lesions and skin destruction by laser evaporation of the peripheral lesions. A female with lichen sclerosus et atrophicus of the vulva, which had been treated by intralesional corticosteroid injection and vulvectomy showed again a recurrence of the disease. Squamous cell cancer of the vulva diagnosed and surgically treated during pregnancy is a rare event. Only a few cases have been described until the present. We report two additional women with Stage II and III disease. Twenty patients undergoing inguinofemoral lymphadenectomy for vulvar malignancy had the femoral vessels covered with freeze-dried human cadaver dura mater; 5 patients had a radical vulvectomy and 15 had a modified radical vulvectomy, and a total of 38 groins were explored. Extramammary Paget disease of the vulva is a rare condition that accounts for only 1-2% of vulvar malignancies and represents a frequent cause of misdiagnosis. It is most commonly seen in postmenopausal women. Clinically it is similar to Paget disease of the breast, appearing as red, well-demarcated eczematoid lesions, with slightly raised edges. A high degree of clinical suspicion is very important when evaluating these lesions in order to avoid misdiagnosis and delay of effective treatment. We present a case of vulvar Paget disease treated with vulvectomy.

A retrospective review of 37 cases of carcinoma of the vulva presenting between 1996 and 2000 has been carried out. Thirty-three cases were managed with curative intent and four cases with advanced loco-regional disease were managed with palliative intent. Forty-four cases of melanoma of the vulva are reviewed. A leveling system comparable to that of Clark's (in cutaneous melanoma of nongenital origin), is described. Survival appears to be directly related to depth of invasion, with no patient with a Level II lesion succumbing to disease. Minimal therapy recommended is radical vulvectomy with bilateral inguinal-femoral node dissection.

We report on a suboptimal surgical treated cancer of the vulva with the accidental diagnosis of a second cancer within the completion of the surgery three weeks later. The patient was 48 years old, presenting with a central, multifocal, exophytic tumor of the clitoris. The primary surgery was: vulvectomy with bilateral inguinal lymphadenectomy ("triple incision").
