The umbilicus was preserved in 32 gastroschisis repairs. A modified technique was performed in the last eight infants, which provided an improved cosmetic appearance. The procedure is suitable for both primary and staged repairs.

BACKGROUND--Familial cases of gastroschisis are exceptional. CASE REPORT NO. 1--A boy was admitted at birth suffering from gastroschisis. Surgical repair was done but the patient died on day 24 with septicemia. CASE REPORT NO 2--Fifteen years later, gastroschisis was discovered by ultrasonography in a male fetus (GA = 26 weeks). Successful surgical repair was done just after the birth at a normal term. Mothers of both cases were sisters. CONCLUSIONS--The rare familial cases of gastroschisis have been seen in sibships. This is the first familial case seen in first cousins.

We present a patient with pyoderma gangrenosum, a rare complication of ulcerative colitis. The patient's disease was limited to the distal colon, was clinically mild and responded quickly to treatment, and yet it was associated with pyoderma gangrenosum and arthritis, complications generally associated with more severe and extensive ulcerative colitis.

The rapidly growing possibilities of prenatal diagnosis and intrauterine manipulation of the malformed fetus have created the need for animal experimental models as simple and easy to handle as possible. We report on our experience with the induction of body wall defects in the chick embryo. Laparoschisis was produced by evisceration of the bowel through the umbilical stalk and its exposition to the allantoic fluid at the 14th day of incubation. Exomphalos, Cantrell's Pentalogy and Spina Bifida were induced by sterile aspiration of 5 ml of egg albumen at the 24th hour of incubation. All embryo

Psoas abscess classically was described as secondary to tubercular spondylitis but now more frequently is a complication of an intraabdominal process such as Crohn's disease. Less well recognized is that the fever, flexion contracture of the hip, and weight loss characterizing psoas abscess may be the first indications of Crohn's disease; in fact, gastrointestinal symptoms may be completely absent. Psoas abscess was the first sign of Crohn's disease in 11 of 46 reported patients. We present three additional patients; two were asymptomatic before psoas abscess formation. Only seven of 26 patien

A case of gastroschisis and double small bowel atresia is reported. Primary surgical treatment consisted in jejunostomy located at 28 cm from the ligament of Treits, reinstatement of excluded bowel loops encased in a thick matrix of fibrinous material, and abdominal wall closure. Seven weeks later, the lower part of small bowel was anastomosed to the medium part. Because of uneven bowel caliber, an ileostomy was associated to parenteral nutrition using permanent instillation of secretions recovered from ileostomy was associated to parenteral nutrition. At 4 1/2 month, small bowel continuity wa

A case of necrotizing fasciitis of the abdominal wall, following an uneventful diagnostic laparoscopy in an elderly, diabetic patient is reported. The patient recovered after extensive surgical debridement and antibiotic therapy. The possibility of a lethal infection in patients with impaired immune defense mechanisms is discussed.

Gastroschisis complicated by intestinal atresia is a complex problem. Six cases are presented. A review of the literature and of our own cases shows a high mortality rate. Success or failure is related more to the pathology present than to any specific method of operative management. Resection and primary anastomosis is the favored method of treatment, as the intestine heals well in spite of its appearance. Primary closure of the abdominal wall musculature and skin is done whenever possible. A gastrostomy is used uniformly. Intravenous hyperalimentation is critical to survival of these babies
