Variation in Health Care Resources in Rural and Urban Counties Associated with Differences in COVID-19 Mortality.

Few studies have examined whether the variation of health care infrastructure and workforce had potential impacts on COVID-19 mortality in rural counties. Linking national datasets, we stratified 1,548 rural and 1,148 urban U.S. counties using 2021 COVID-19 median mortality rates (19.5 vs. 14.0 per 10,000 population, respectively) as a cut-point for high and low mortality to compare sociodemographics, vaccination status, access to health centers and hospitals, and health care workforce supply between and within rural and urban counties. High-mortality (compared with low-mortality) rural counti

A Bridge to Comfort: Nurses Driving Early Palliative Consults in the Intensive Care Unit.

This project established a protocol that enabled all intensive care unit (ICU) patients to receive a palliative care (PC) screening. Those who met criteria received timely access to PC in the ICU setting. Delayed access to PC can result in untreated symptoms, lack of understanding of care preferences, unwanted invasive procedures, and underutilization of hospice care. A Plan-Do-Study-Act framework was utilized to establish this protocol. The multidisciplinary team sought to increase PC consults in the ICU by 30% using a nurse-driven standardized screening tool. The PC Referral Criteria establi

A Scoping Review of Certified Nurse-Midwife and Certified Midwife Care in the United States: Assessing Outcomes Across Six Patient Care Domains.

UNLABELLED: Policy Points Certified nurse-midwife (CNM)/certified midwife (CM) care is associated with outcomes that are comparable or improved compared to physician care across multiple domains of health care quality, especially safety and effectiveness. CNM/CM care is consistently associated with lower rates of intrapartum interventions and improved birth outcomes and patient satisfaction. Integration of CNM/CM care remains limited across many US health systems due to scope of practice restrictions and institutional policies. Growing, diversifying, and integrating CNM/CM care offers a critic

A novel use of a statewide telecolposcopy network for recruitment of participants in a Phase I clinical trial of a human papillomavirus therapeutic vaccine.

BACKGROUND: Historically, recruitment and retention of young women in intervention-based clinical trials have been challenging. In August 2012, enrollment for a clinical trial testing of an investigational human papillomavirus therapeutic vaccine called PepCan was opened at our institution. This study was an open-label, single-arm, single-institution, dose-escalation Phase I clinical trial. Women with recent Papanicolaou smear results showing high-grade squamous intraepithelial lesions or results that could not rule out high-grade squamous intraepithelial lesion were eligible to enroll. Normalizing daily awakening and breathing coordination at 15 heterogenous ICUs: a multicenter post-implementation survey.

BACKGROUND: Despite high post-implementation adherence, clinicians may have unresolved questions or concerns regarding use of a protocol to standardize routine daily coordination of the spontaneous awakening trial (SAT) and spontaneous breathing trial (SBT) on ventilated patients. Unresolved questions or concerns may unwittingly curtail practice normalization, impacting practice sustainment when implementation support is withdrawn. The objective of this study was to identify unresolved questions or concerns that may persist following successful implementation of a coordinated SAT/SBT (C-SAT/SB

High-Fidelity, Indigenously Prepared, Low-Cost Moulage as a Valid Simulation Tool to Improve Trauma Education.

Background Simulation-based trauma education facilitates repeated practice in a controlled and safer environment for the learner without any risk to the patient's well-being. Moulage contributes to the perception of reality during training using standardized patients. However, the high cost of commercial moulage items is often prohibitive for regular use. This study aimed to assess the effectiveness of indigenously prepared, low-cost moulage as a valid simulation tool to improve trauma education, explore possible replacements of commercial moulage products, and determine their merits and demer

Microscopic anatomy of the upper aerodigestive tract in harbor seals (Phoca vitulina): Functional adaptations to swallowing.

Abandoned harbor seal pups (Phoca vitulina) are frequently recovered by rehabilitation centers and often require intensive nursing, gavage feeding and swallowing rehabilitation prior to anticipated release. Seal upper aerodigestive tract (UAT) histology descriptions relevant to deglutition are limited, impacting advances in rehabilitation practice. Therefore, we examined the histological characteristics of the harbor seal UAT to understand species-specific functional anatomy and characterize adaptations. To this end, we conducted gross dissections, compiled measurements and reviewed histologic

"One must know things, but above all, one must know how to be": an existential-phenomenological study of professional identity in palliative care.

BACKGROUND: Palliative care-while overlapping with but distinct from end-of-life care-involves multidisciplinary teams providing medical, psychosocial, and existential support to patients and families facing serious illness. While prior research has examined professional identity in palliative care, the existential dimensions of identity formation-what it means to be a palliative care professional-remain largely underexplored. AIM: To explore the lived experiences of palliative care professionals, focussing on how their professional identity is constructed through the existential notion of "b
