Disseminated Fusarium infections in patients following bone marrow transplantation.

Intensive immunosuppressive therapy and broad spectrum antibiotics predispose cancer patients to opportunistic fungal infections. Fusarium has rarely been reported as a pathogen in immunocompromised patients, but is almost uniformly fatal. Only six cases of disseminated Fusarium infection have been described in patients following bone marrow transplantation (BMT). We report here two additional cases. Fusarium infection initially presented with pyomyositis in one patient and with embolic skin lesions in another following T cell-depleted BMT. Both patients died with active Fusarium infection des

[Combined GM-CSF and erythropoietin therapy in myelodysplastic syndrome].

A 60-year-old patient with a myelodysplastic syndrome (MDS) corresponding to refractory anemia with an increase in blast cells (RAEB) was treated with granulocyte-macrophage colony stimulating factor (GM-CSF) and erythropoietin (EPO) for severe symptomatic pancytopenia. During the GM-CSF treatment a distinct increase in granulocytes was observed, but the reticulocytes and thrombocytes decreased to the point where treatment had to be discontinued after eight days. After subsequent treatment with EPO the reticulocyte count rose from 0% to 2%. However, this rise alone was insufficient to decrease

Coombs-positive refractory acquired thrombotic thrombocytopenic purpura in a patient with chronic myelomonocytic leukemia successfully treated with rituximab.

Acquired thrombotic thrombocytopenic purpura (aTTP) is a rare autoimmune disorder characterized by auto-antibodies to Willebrand factor (vWF) cleaving enzyme (ADAMTS13), resulting in unusually large vWF multimers that lead to platelet aggregation, microthrombi formation and microangiopathic hemolytic anemia. Hemolysis in aTTP is mechanical; thus, direct antiglobulin test (Coombs test) is usually negative. Multiple autoimmune conditions and various auto-antibodies have been described in the context of chronic myelomonocytic leukemia (CMML). In this paper, we describe the first case of CMML with

[Factors influencing leukemic transformation in myelodysplastic syndrome].

Prognostic factors affecting the leukemic transformation were studied in 43 patients with myelodysplastic syndrome (MDS). Acute leukemia developed in 17 cases and it was nonlymphocytic leukemia in every case. No remission was achieved following antileukemic therapy and most of the cases proved to be true drug-resistant leukemia. Initial granulopenia, thrombopenia or anemia alone did not influence the occurrence of leukemic transformation but pancytopenia indicates bad prognosis. According to FAB classification especially refractory anemia with excess of blasts (RAEB) and RAEB in transformation

Surgical repair of paraesophageal hernia resolves unexplained iron deficiency anemia in the vast majority of patients: a propensity-matched multicenter study.

BACKGROUND: Paraesophageal hernias (PEH) frequently associate with iron deficiency anemia (IDA) through unclear mechanisms. We investigated whether surgical repair effectively treats unexplained IDA in patients with PEH. METHODS: This multicenter retrospective study (2010-2024) analyzed 1700 patients undergoing hiatal hernia repair across Asuta Medical Centers. After excluding patients with identifiable bleeding sources (Cameron lesions, erosive gastritis, ulcers), 522 patients remained: 136 with unexplained IDA and 386 controls. Propensity score matching (1:3) aligned 99 anemic patients with

Laparoscopic splenectomy in a Jehovah's Witness with profound anemia.

Open surgery in a severely anemic patient may be complicated by a substantial blood loss from a large incision and subsequent poor wound healing secondary to the anemia. We report our success in performing a splenectomy laparoscopically in a profoundly anemic patient. A 50-year-old white male Jehovah's Witness who was HIV positive was referred for splenectomy after he developed profound, worsening anemia secondary to hypersplenism that was refractory to medical management. His preoperative hemoglobin and hematocrit levels were 2.7 g/dl and 8.8%, respectively, but his religious beliefs preclude

[Myelodysplastic syndromes in the aged].

Seventy-five cases of myelodysplastic syndromes (MDS) in the aged (over 60 years) were analysed for hematological findings, immunological parameters and response to treatment in respect to prognostic significance. They were diagnosed according to the FAB classification, but patients with hypoplastic marrow were included if myelodysplasia was evident. Thirty-four percent of patients with primary acquired refractory anemia (PARA) or primary acquired sideroblastic anemia (PASA), and 36% of patients with RA with excess of blasts (RAEB) had hypoplastic bone marrow. The positive rates of antinuclear

Extended cytogenetic follow-up of patients with myelodysplastic syndrome (MDS).

The prognostic significance of clonal karyotype status in myelodysplastic syndrome (MDS) is assessed after an extended follow-up period of 5 years. There are three karyotype, single abnormalities or multiple abnormalities at the time of referral. However, there is no correlation between the size of the abnormal clone and prognosis. Karyotype status has independent prognostic significance in 'high risk' MDS so that patients with a refractory anaemia with excess of blasts (RAEB)/RAEB in transformation (RAEB-t) and a normal karyotype survive significantly longer than those with an abnormal karyot

[Prolonged erythrocytic T activation, depression of blood group A and increase of fetal hemoglobin in a myelodysplastic syndrome evolving into erythroleukemia].

T cryptantigen can be exposed on the red cell membrane as a result of removal of terminal glycosides, either by bacterial enzymes or by incomplete synthesis of the cell membrane due to somatic mutation, usually caused by a neoplasm. T-activated erythrocytes have been observed in different pathologies, but they have not been seen associated with other abnormalities of red blood cell proteins described in myelodysplastic syndromes or acute leukaemias. A patient with initial diagnosis of refractory anaemia that evolved into erythroleukaemia showed prolonged T-activation, a depressed A blood-group

Interferon gamma induces differentiation of cells from patients with myelodysplastic syndromes in vitro and in vivo.

A recombinant human interferon-gamma (IFN-gamma) was added to the clonal and suspension cultures of bone marrow (BM) cells from three patients with refractory anemia with excess of blasts (RAEB) before and after IFN-gamma therapy. IFN-gamma inhibited the hematopoietic progenitor cell growth from normal BM in one patient. In the remaining two patients with RAEB, IFN-gamma at lower concentrations (10-10(2) U/ml) consistently enhanced the growth of hematopoietic progenitors. The addition of IFN-gamma to suspension cultures promoted cellular differentiation in all three cases, as evidenced by pers

Serum beta 2-microglobulin in patients with adult T-cell leukemia.

Thirty-seven patients with adult T-cell leukemia had serum beta 2-microglobulin (beta 2M) estimated at presentation. A significant association was found between beta 2M and the subtypes of the disease: beta 2M in the acute type is significantly higher than in the chronic and smoldering types (p less than 0.01), and beta 2M in the lymphoma type is apparently higher than in the chronic and smoldering types, although the difference between the lymphoma and chronic types was not statistically significant. An apparent reduction in beta 2M levels after chemotherapy was observed in 7 patients who rea

Monocytic involvement by monosomy 7 preceded acute myelomonocytic leukemia in a patient with myelodysplastic syndrome.

Novel techniques were used to detect which cell lineages were affected by monosomy 7 in a patient who had myelodysplastic syndrome and later developed acute leukemia. The patient had had paroxysmal nocturnal hemoglobinuria for 20 years before developing refractory anemia with excess of blasts. Cytogenetic analysis at the myelodysplastic stage disclosed monosomy 7 in bone marrow mitoses. Restriction fragment length polymorphism analysis of fractionated white blood cells with the chromosome 7-specific probes MetH and MetD revealed that blood monocytes and most bone marrow erythroblasts but not b

Cytogenetic study of circulating blasts in leukemias.

A special cultivation technique of separated blasts of peripheral blood in suspension culture has been used for cytogenetic diagnosis of patients suffering from acute nonlymphocytic leukemia, chronic myeloid leukemia, and refractory anemia with excess of blasts. Twenty-three patients were examined; in ten cases isolation of blasts was performed on Ficoll-Verografin and in the remaining 13 patients further separation of T-lymphocyte precursors by means of sheep erythrocytes was performed. Remarkably, a 100% success rate was attained in all cultivations. The optimum harvesting time was 72-96 hou

Unusual Infections following Haploidentical Hematopoietic Stem cell Transplant in a Case with severe Aplastic Anaemia'.

We report a case of very severe aplastic anemia who developed from an uncommon bacterial, recalcitrant parasitic infection and viral reactivation after haploidentical hematopoietic stem cell transplant. The risk factors for multiple infections in this setting included immunocompromised state due to pretransplant prolonged immunosuppression, viral reactivation and graft-versus-host disease (GVHD) requiring multiple immunosuppressive agents. The index case was successfully treated for pulmonary nocardiosis, refractory giardiasis, cytomegalovirus, and adenovirus reactivation highlighting signific

Gilteritinib combined with venetoclax and azacitidine for relapsed acute myeloid leukemia cocurrent with pure red cell aplasia after allogeneic hematopoietic stem cell transplantation: a case report.

Pure red cell aplasia (PRCA) is a rare bone marrow (BM) disorder characterized by ineffective erythropoiesis, reduced reticulocyte count, normocytic anemia, and the absence of erythroid precursors. Here, we present a rare instance of PRCA occurring after ABO-matched allo-HSCT in a refractory/relapsed acute myeloid leukemia (R/R AML) patient. In this case, the patient received a combination treatment of Gilteritinib, Venetoclax, and Azacitidine. Remarkably, this treatment not only reduced myeloblasts but also facilitated the restoration of erythroid hematopoiesis.

Bacteraemia caused by Leptotrichia trevisanii in a neutropenic patient.

We describe an episode of Leptotrichia trevisanii bacteraemia in a neutropenic hemato-oncology patient receiving chemotherapy for Refractory Anemia with Excess Blasts-2 (RAEB-2). Although Leptotrichia spp. colonize the oral cavity and genitourinary tract, serious episodes of bacteraemia might occur in immunocompromised patients, particularly in those with severe neutropenia. Therefore, microbiologists should consider the possibility of Leptotrichia spp. septicemia in patients with blood cultures positive for gram negative bacilli, when routine microbiology tests fail to reveal a correct identi
