Ankle sprains commonly injure the superior fascicle of the anterior talofibular ligament (ATFL), triggering a biomechanical "domino effect" that leads to intra-articular and extra-articular changes. These secondary alterations may present with or without classic instability symptoms. Due to the limited sensitivity of standard imaging, thorough clinical assessment is essential. If conservative treatment fails, surgical repair-particularly arthroscopic all-inside repair of the ATFL-is recommended.

Ultrasound-guided interventions for the ankle and foot offer precise treatment for musculoskeletal conditions. This article reviews injections of the talocrural joint, first metatarsophalangeal joint, Achilles tendon (via stripping and retrocalcaneal bursa injections), plantar aponeurosis, and tibialis posterior tendon, detailing probe selection, patient positioning, and procedural techniques. Precise needle guidance is emphasized to prevent complications like nerve injury and tendon rupture. Text is supplemented with anatomical notes.

Ankle fractures in geriatric patients are common and challenging due to comorbidities and the risk of functional decline. Management must ensure fracture stability while promoting early mobilization to prevent complications and maintain independence. Open reduction and internal fixation remains the standard for unstable fractures; however, alternative techniques such as fibular intramedullary nail or tibiotalocalcaneal nail are suitable for fragile patients, as they reduce wound complications and permit early weight-bearing. Conservative treatment is appropriate for stable fractures or patient…

BACKGROUND: Equinus contractures can commonly be due to contractures of gastrocnemius muscle or combined contractures of the gastrocnemius-soleus Achilles tendon complex. The decision to release part or all of the gastrocnemius-soleus Achilles tendon complex is often assessed intraoperatively while the patient is under anesthesia. It remains unknown whether the administration of general anesthesia affects the measurement of passive ankle dorsiflexion.
METHODS: The unaffected, nonoperative limb on 46 foot and ankle patients underwent a Silfverskiold test measuring passive ankle dorsiflexion pr…

Fracture-related infection (FRI) poses significant challenges in the management of foot and ankle fractures, potentially leading to functional impairment and heightened health care expenses. This review addresses the diagnosis, risk factors, and management strategies for FRI. Surgical management involves multidisciplinary collaboration, emphasizing debridement, antimicrobial therapy, and implant removal when indicated. Specific considerations for limited soft tissue coverage, tenuous blood supply, and fracture type in the foot and the ankle underscore the importance of tailored approaches to o…

Neuropathic destabilization of the hindfoot and/or ankle is a significant complication for the patient with diabetes/neuropathy. The loss of ligamentous integrity and bony destruction results in a limb that is not stable for weight bearing. This loss of independence adds significant health risks to the patient. Management of this disease process is both time consuming and technically demanding for both the practitioner and the patient. Attention to detail and aggressive decision making is often necessary to salvage the limb. The goal for treatment is to produce a stable, weight-bearing limb th…

High-resolution ultrasound (US) can be used to assess soft tissue abnormalities in the foot and ankle. Compared to MRI, it has lower cost, is widely available, allows portability and dynamic assessment. US is an excellent method to evaluate foot and ankle tendon injuries, ligament tears, plantar fascia, peripheral nerves, and the different causes of metatarsalgia.

Seronegative spondyloarthritis (SpA) is an umbrella term that includes ankylosing spondylitis (AS), psoriatic arthritis, reactive arthritis, and arthritis related to inflammatory bowel disease. Apart from AS, these other conditions predominantly affect the appendicular skeleton. Both the foot and ankle are frequently involved peripheral joints. According to the latest Assessment of Spondyloarthritis International Society criteria, imaging is a key way to diagnose peripheral seronegative SpA. Common imaging features are enthesitis, synovitis, tenosynovitis, erosive and bone-proliferative change…

Peripheral neuropathies of the foot and ankle can be challenging to diagnose clinically due to concomitant traumatic and nontraumatic or degenerative orthopedic conditions. Although clinical history, physical examination, and electrodiagnostic testing comprised of nerve conduction velocities and electromyography are used primarily for the identification and classification of peripheral nerve disorders, MR neurography (MRN) can be used to visualize the peripheral nerves as well as the skeletal muscles of the foot and ankle for primary neurogenic pathology and skeletal muscle denervation effect.…

Angioleiomyoma is a benign tumor, which arises from the smooth muscle. It comprises approximately 4.4% of all benign soft tissues' neoplasms and they are commonly located at the lower extremities. They are most frequently found in middle-aged women. Angioleiomyoma is usually presented as a painful solitary lesion in the subcutaneous tissue. Due to the lack of evidence in the literature, the aim of this current concepts review was to provide foot and ankle surgeons the most updated and useful information for diagnosis and management of foot or ankle's angioleiomyoma. The possible diagnosis of a…

BACKGROUND: Literature has shown implicit bias in the treatment between non-operative and surgical treatment in patients with certain types of ankle fractures, which comprise 7.6% of all adult fractures. An understanding of any bias across all ankle fracture management may prove to be critical for the understanding of potential correlations between treatment methods and outcomes of patients with ankle fractures. Therefore, this study aimed to determine whether there is a sex-based bias in the operative and non-operative treatment of all ankle fractures.
METHODS: A retrospective study of 1175 …

This article reviews situations in which surgical treatment is required in the context of different types of pathology in the foot and ankle of children, focusing on the role of imaging in surgical planning and postsurgical assessment. The types of pathology analyzed from this perspective are congenital pathology, neuromuscular disorders, osteochondral lesions, fractures and infection, ligament injuries, and tumors. We address the most common pitfalls of postsurgical imaging of the ankle and foot in children. With some exceptions, postsurgical follow-up focuses on clinical assessment, with imag…

The supramalleolar osteotomy (SMO) is a joint-preserving surgical procedure that allows realignment of the ankle joint in severe deformity secondary to arthritis. This osteotomy realigns the mechanical axis to provide better weight distribution through the ankle joint. With an aligned mechanical axis, the overloaded asymmetric ankle joint will shift toward the preserved joint area in a valgus or varus ankle joint. The SMO also can be used via a staged approach to correct severe deformity in an end-stage arthritic ankle before total ankle arthroplasty to optimize the implant's longevity and imp…

In-office needle arthroscopy (IONA) has been a readily available tool dating back to the 1990s, primarily for diagnostic purposes. This technique was not fully accepted and implemented because of significant limitations with the image quality and lack of instrumentation available to simultaneously treat the identified pathologies. However, recent advancements in IONA technology have made it possible to perform arthroscopic procedures under local anesthesia in the office setting, which once required a full operating suite. IONA has revolutionized how we treat foot and ankle pathologies in our p…

Winter sports injuries of the foot and ankle are a relevant entity due to the high enthusiasm for alpine skiing and snowboarding. The condition of the footwear and rigidity of the binding have a significant influence on the pathobiomechanics of fracture development and have been shown to influence the frequency of knee and ankle injuries. Foot injuries are rare due to the sport-specific footwear with good padding and frequent hard shell casings.

Ankle and foot injuries are very common injuries in the general population, and more so in athletes. MR imaging is the optimal modality to evaluate for ligamentous injuries of the ankle and associated conditions after ankle sprain. In this article, the authors discuss the epidemiology, biomechanics, normal anatomy, and pathology of the ankle as well as injuries of the hindfoot and midfoot that are often associated with ankle injuries.
