We describe the first recorded case of segmental mediolytic arteritis (SMA) in Japan. A 71-year-old Japanese woman developed sudden abdominal pain and went into shock during hospitalization for treatment of valvular heart disease. Laparotomy revealed a ruptured pseudoaneurysm of the left gastric artery. Histological examination of the resected artery revealed multiple extensive areas of the dilated wall where the media had partially or totally disappeared with or without the intima. These features were typical of those described previously for SMA (1,2). Three-dimensional analysis of the arter…

Cerebral infarction due to fungal arteritis is an uncommon complication of neurosurgical operations and adjuvant immunosuppressive therapy, including long-term steroids. If unrecognized, the neurological deterioration which ensues may be mistreated by increasing the dose of steroids. A case of a 38-year-old Caucasian male who had no obvious immune deficiency or fungal infection prior to a craniotomy for cerebral tumour is described in whom perioperative aspergillus infection resulted in cerebral arteritis and extensive cerebral infarction with a fatal outcome. Long-term steroid therapy used in…

We report a case of rheumatic polymyalgia associated with temporal arteritis, in which clinical-biological symptoms and evolution with steroid or immunosuppressive therapy were analyzed. We confirm the lack of effectiveness of the treatment with AINES, the low response to steroids and the high response to immunosuppressive therapy.

Over a 16-year period, 205 patients with hypertension were shown to have a renovascular aetiology. Of these, 125 (61 per cent) had Takayasu's arteritis, 58 (28.3 per cent) had fibromuscular dysplasia, 16 (7.8 per cent) had atherosclerosis, five (2.4 per cent) had polyarteritis nodosa and one (0.5 per cent) had renal artery aneurysm. Among patients with Takayasu's arteritis, males were affected as commonly as females. The mean age of these patients at the time of detection was 26.8 +/- 8.6 years (range 5-52 years). Type I arteritis was seen in nine (7.2 per cent), Type II in 40 (32 per cent) an…

In giant cell arteritis, a common vasculitic disease of elderly patients, the biopsy of the temporal artery has been considered an important part of the diagnostic procedure. The necessity to perform this biopsy has recently been questioned. It has also been proposed to substantially reduce the high doses of steroids usually given.

Prompted by one of our own case studies, which we report here, we reviewed the literature for coincidence of rheumatoid arteritis (RA) with polymyalgia rheumatica (PMR) and temporal arteritis (TA), respectively. The indicative feature of this uncommon condition was found in 13 cases, whereas in 70 other cases reported, the diagnosis of combined RA and PMR/TA was probable. Diagnostic criteria for determining combined RA and PMR/TA will be discussed, as well as the clinical important feature of senile RA.

A 61-year-old woman, suffering from classic seropositive rheumatoid arthritis with rheumatoid nodule histologically documented, developed temporal arteritis. HLA-DR typing revealed the presence of DR3 and DR4 antigens. The findings from previous studies support the association of HLA-DR antigens, giant cell arteritis-polymyalgia rheumatica and rheumatoid arthritis, and suggest the participation of a common immunogenetic mechanism in their pathogenesis.

We present the first report of PMR/GCA occurring in husband and wife in Great Britain. The aetiology of the condition remains unclear but recent reports suggesting case clustering and 3 other reports of spouses developing PMR/GCA support the hypothesis that an infective agent may be implicated.

A patient with biopsy proven temporal arteritis/polymyalgia rheumatica and erosive rheumatoid arthritis is presented. Only 15 such patients have previously been documented in the literature. The coexistence has been thought to be extremely infrequent, but could merely by chance appear in far more patients than previously reported.

In order to asses the importance of cardiac damage in Takayasu's arteritis, 125 cases were studied and followed for 5.8 +/- 5.5 years. The arterial lesion involved the aorta and the principal abdominal branches in 10.4% of cases. Isolated lesions of the supraaortic vessels were present in 25.6% of cases. The rest of cases had obstructions in both arterial territories (64%). Cardiac damage was present in 82.4% of cases with the following manifestations: precordial murmurs (65%), cardiac enlargement (70%), heart failure (28%), angor pectoris (13.6%), abnormal electrocardiogram (60%): left ventri…

A 37-year-old woman with primary Sjögren syndrome developed mixed cryoglobulinemia and systemic vasculitis. Subarachnoid hemorrhage occurred as a result of necrotizing anterior spinal arteritis. Although rarely seen in mixed cryoglobulinemia, central nervous system complications have recently been documented in Sjögren syndrome. The patient's serum contained antibodies to the Ro(SSA) cytoplasmic antigen, and these antibodies were concentrated in the cryoglobulin fraction. Anti-Ro(SSA) antibodies are associated with the occurrence of vasculitis in patients with Sjögren syndrome, which suggests …
