Activated mast cells in periprosthetic joint infection-associated tissue.

BACKGROUND: Periprosthetic joint infection (PJI) is a devastating complication of total joint arthroplasty surgery. Increased densities of activated mast cells have been predicted to be present in PJI compared to non-infectious arthroplasty failure based on analysis of transcriptomic data, but their presence in PJI-associated periprosthetic tissues has not been visually confirmed. OBJECTIVE: This preliminary study investigated the presence and activation status of mast cells in periprosthetic tissues associated with PJI. METHODS: Periprosthetic tissues from five PJI cases and three arthropla

Bacterial Burden, Not Local Immune Response, Differs Between Acute and Chronic Peri-Prosthetic Joint Infections.

Background: Conducting gram stains in peri-prosthetic joint infections (PJI) is known to have poor sensitivity. However, the aims of this study were to use gram stain results of acute and chronic PJI to determine differences with respect to bacterial burden and levels of local innate immunologic response. Patients and Methods: Patients with acute and chronic PJI from January 1, 2016 and December 31, 2020 were identified by use of Current Procedural Terminology codes. Manual review of medical records for infecting organisms and gram stain results for stained bacteria and for local tissue inflam

Pediatric musculoskeletal infections.

PURPOSE OF REVIEW: The aim of this article is to review the recent guidelines and literature regarding the diagnosis and the treatment of common pediatric musculoskeletal infections: septic arthritis, osteomyelitis, pyomyositis, and Lyme disease. RECENT FINDINGS: In the last decade, a better understanding of the causative organisms of common bacterial infections, including Kingella , leads to prompt targeted antimicrobial coverage in all musculoskeletal infections. Prompt diagnosis and treatment continues to be the mainstay in the treatment of children with osteoarticular infections. Efforts

Bacteriophage therapy for periprosthetic joint infections: Current limitations and research needed to advance this therapeutic.

Bacteriophage therapy is a promising treatment for periprosthetic joint infections (PJIs), particularly given these agents have innate abilities to degrade the biofilm matrix and lyse bacteria within. However, many aspects of this therapy are poorly understood causing treatments to lack uniform effectiveness and reproducibility, which is in part a consequence of several inherent limitations to using bacteriophages to treat PJI. Herein, these limitations are discussed as are additional translational research that needs to be conducted to advance this therapeutic. These include determining if ba

Field Control of Avian Reoviruses in Commercial Broiler Production.

Prevention of tenosynovitis/viral arthritis caused by variant avian reoviruses within commercial broiler production has become increasingly more challenging because of the lack of protection afforded by the current commercially available vaccines. Avian reoviruses isolated from clinical cases of tenosynovitis/viral arthritis in recent years are antigenically distinct from nearly all of the commercially licensed modified live and inactivated biologics available in the United States. The emergence of new variants is likely shaped by a lack of homologous protection coupled with selection pressure

Comparison of Arthroscopy versus Open Arthrotomy for Treatment of Septic Arthritis of the Native Knee: Analysis of 90-Day Postoperative Complications.

Septic arthritis is an orthopaedic emergency, often necessitating surgical debridement, and the knee joint is most frequently affected. Arthroscopic and open irrigation and debridement (I&D) are the two commonest surgical treatments for septic arthritis of the native knee. Several studies have compared outcomes of open and arthroscopic management without coming to a clear conclusion which yields superior outcomes. The purpose of this study was to compare the results of these two surgical techniques to treat septic arthritis of the native knee using a large nationwide database. Patients who und

Role of Staphylococcus aureus Formate Metabolism during Prosthetic Joint Infection.

Biofilms are bacterial communities characterized by antibiotic tolerance. Staphylococcus aureus is a leading cause of biofilm infections on medical devices, including prosthetic joints, which represent a significant health care burden. The major leukocyte infiltrate associated with S. aureus prosthetic joint infection (PJI) is granulocytic myeloid-derived suppressor cells (G-MDSCs), which produce IL-10 to promote biofilm persistence by inhibiting monocyte and macrophage proinflammatory activity. To determine how S. aureus biofilm responds to G-MDSCs and macrophages, biofilms were cocultured wi

Significance of nutritional status in the development of periprosthetic infections : A retrospective analysis of 194 patients.

BACKGROUND: Malnutrition caused by protein and vitamin deficiencies is a significant negative prognostic factor in surgical wound healing disorders and infections. Particularly in elective surgery, preoperative compensation of deficiencies is advisable to avoid negative postoperative consequences. This study examined the nutritional and protein balance of patients with periprosthetic hip and knee joint infections. MATERIAL AND METHODS: Patients with periprosthetic hip or knee joint infections constituted the study group (SG). Control group I (CG I) included patients with primary implants and

Suppurative Arthritis Induced by Scedosporium apiospermum and Mycobacterium fortuitum: a Case Report.

BACKGROUND: Suppurative arthritis induced by Scedosporium apiospermum (S. apiospermum) or Mycobacterium fortuitum (MF) was rare, and even more so when caused by a mixed infection of the two. In this paper, we report the rare case of suppurative arthritis induced by S. apiospermum and MF. METHODS: A 46-year-old patient whose left knee was accidentally injured by a rotary tiller. His left knee joint was clearly swollen although debridement and suturing had been performed twice at a local hospital. Bacterial culture result was MF and S. apiospermum after admission. Definitive therapy (debridemen

Retrospective evaluation of 103 cases of septic arthritis in dogs.

OBJECTIVE: This study examines inciting causes, diagnosis, treatment and risk factors for the recurrence and outcome of septic arthritis (SA) in dogs. STUDY DESIGN: Medical records spanning 17 years from five referral hospitals were surveyed for presumptive and confirmed cases of SA. RESULTS: SA was identified in 103 cases. Spontaneous septic SA was the most common inciting cause. The most commonly affected joints were the stifle (40%) and elbow (24%). Pre-existing osteoarthritis (OA) was present in 63% of septic joints and was associated with recurrence (p = 0.03). Treatment with antibiotic

One-stage Treatment of Prosthetic Joint Infection.

Management of prosthetic joint infection in hip and knee arthroplasty patients is traditionally undertaken with a two-stage treatment protocol. However, this strategy carries high morbidity and cost, yet a substantial portion of patients sustain reinfection. One-stage treatment protocols have been popularized in Europe and other parts of the world but remain infrequently performed in the United States, despite equivocal efficacy with two-stage treatment based on currently available data. Herein, we describe a current one-stage treatment protocol used as part of a multicenter randomized clinica

[Prosthetic infections and the increasing importance of psychological comorbidities : An epidemiological analysis for Germany from 2009 through 2019].

BACKGROUND: Periprosthetic joint infection (PJI) is a devastating complication in orthopedics and trauma surgery. A potential increase in PJI diagnoses, especially in combination with psychological comorbidities may become a significant challenge for healthcare stakeholders. So far, the prevalence for Germany is unknown, which makes it difficult to estimate future treatment demand or to predict developments that can be influenced by adapting prevention and treatment measures. OBJECTIVES: The present study provides a detailed overview of the epidemiology of periprosthetic joint infections and
