Predictors of massive blood transfusion: a Delphi Study to examine the views of experts.

BACKGROUND: Trauma patients requiring massive blood transfusion (MBT) have high morbidity and mortality: early and aggressive use of blood products during immediate resuscitation may improve survival. There is currently a lack of evidence to guide initial identification of these patients which is especially important in areas where plasma may need to be thawed. In the absence of this evidence, this study aimed to robustly evaluate expert opinion by using a Delphi process to identify predictors of massive transfusion. This process can be used to ensure that decision rules include variables that…

The effect of red blood cell transfusion on intermittent hypoxemia in ELBW infants.

OBJECTIVE: To test the hypothesis that the effect of red blood cell (RBC) transfusion on intermittent hypoxemia (IH) in extremely low birth weight (ELBW) infants is dependent on postnatal age. STUDY DESIGN: Oxygen saturation of 130 ELBW infants, who required transfusion, was monitored continuously for the first 8 weeks of life. We compared the characteristics of IH (SpO2⩽80% for ⩾4 s and ⩽3 min), 24 h before and both 24 h and 24 to 48 h after each RBC transfusion at three distinct time periods: Epoch 1, 1 to 7 days; Epoch 2, 8 to 28 days; and Epoch 3, >28 days. RESULT: In Epoch 1, the freque

Lead content of blood transfusions for extremely low-birth-weight infants.

OBJECTIVE: Our objective was to evaluate the levels of lead in blood transfusions for extremely low-birth-weight (ELBW) infants and how they compare with lead levels suspected of causing neurotoxicity. STUDY DESIGN: This prospective cohort included infants with a birth weight ≤ 1000 g. The quantity of transfused lead was calculated based on transfused volume and packed red blood cell (PRBC) unit lead levels. The results were compared with the exposure reference. RESULTS: Thirty-seven infants (birth weight 736 ± 157 g, gestational age 25.5 ± 1.5 weeks) received 322 transfusions from 47 PRBC u…

Mercury content of blood transfusions for infants with extremely low birth weight.

OBJECTIVES: To assess whether blood transfusions for infants with birth weights of 1000 g or less are a source of mercury and whether any mercury delivered through the transfusion is above the currently set oral reference dose. PATIENTS AND METHODS: We studied an observational cohort. Inclusion criteria included birth weight 1000 g or less and receipt of 1 or more packed red blood cell transfusions. Packed red blood cell units were tested prospectively for mercury levels. The quantity of transfused mercury was calculated on the basis of transfused volume and packed red blood cell mercury leve…

Is the blood donated by habitual nut quid chewers suitable for use in transfusion?

BACKGROUND/PURPOSE: Betel quid (BQ) chewing is a popular oral masticatory activity, and there are approximately 600 million BQ chewers worldwide. Although chewing BQ has been linked to the patho-genesis of oral cancer, leukoplakia, and oral submucous fibrosis. The question whether the mixed constituents present in areca nut, which may exert cytotoxic effects on red blood cells (RBCs), has never been addressed. METHODS: Heparinized blood specimens were obtained with informed consent from healthy laboratory personnel. RBCs were separated with the standard procedure and adjusted to 10% hematocri

Perioperative problems and treatment of a teenager with a juvenile angiofibroma refusing blood transfusion.

OBJECTIVES: Juvenile angiofibroma is a benign, non-encapsulated neoplasm, consisting of vascular and connective tissue. Its main feature is … vascular network surrounding the sphenoid bone. METHODS: If the patient or his/her parents refuse blood transfusion and use of blood products, it is necessary to apply modifications in the routine perioperative treatment. In the case of the patient refusing blood and blood products transfusion because of their beliefs, it is possible to get consent to use differe …

Fetal transfusion of red blood cells for alloimmunization: validity of a published equation.

OBJECTIVE: To validate the equation published in 1990 by Leduc et al. for red blood cell fetal transfusion where fetoplacental blood volume (VO) = 100 ml/kg, then improve its precision. METHODS: We reviewed 101 fetal transfusions among 32 patients. We analyzed risk factors for an inaccurate estimation with uni- and multivariate analysis. We compared the obtained Leduc formula with three other published equations. RESULTS: Fetal weight and gestational age were risk factors for an inaccurate estimation of the final Hct. Before 32 weeks the estimation of VO was 120 ml/kg instead of 100 ml/kg. A

A case study of an older adult with severe anemia refusing blood transfusion.

PURPOSE: To discuss the diagnosis and treatment of severe anemia in an older adult who presents the challenge of declining blood transfusion in a real-world scenario where critical thinking, evidence-based care, and collaboration with other providers must come together to serve this patient's unique needs. DATA SOURCES: Extensive review of the scientific literature on anemia and the situation in which a patient refuses blood transfusion presented in a case study format. CONCLUSIONS: A thorough physical assessment, complete health history, and appropriate diagnostic workup should be used to d…

Commentary: is it safe to limit allogeneic red blood cell transfusions to neonates?

Two randomized clinical trials, conducted independently, have reported results of neonates transfused with red blood cells (RBCs) given per either liberal (relatively high pretransfusion blood hematocrit levels) or restrictive (relatively low pretransfusion blood hematocrit levels) transfusion programs. Both found fewer RBC transfusions given per restrictive programs and comparable outcomes for several clinical endpoints. However, the Iowa trial found significantly more problems with apnea, intraparenchymal brain hemorrhage and periventricular leukomalacia in infants transfused per the restric

Red blood cell transfusion in a patient with anti-AnWj: a case report.

Anti-AnWj (Anton) has been associated with clinically significant hemolytic transfusion reactions. More than 99 percent of studied populations have RBCs that express the antigen. Reported here is a patient with anti-AnWj who was transfused with antigen-positive RBCs without adverse reaction.

Reevaluation of recent criteria for blood exchange transfusion in term infants with hyperbilirubinemia.

For hyperbilirubinemic infants treated according to the higher criterion for a blood exchange transfusion (BET), auditory brainstem response (ABR) was performed to evaluate whether the treatment was adequate. Twenty hyperbilirubinemic infants were collected as the study group. They were divided into 2 groups; group A consisted of 17 infants receiving intensive phototherapy only, while group B consisted of 3 infants receiving BET plus phototherapy. Fourteen healthy neonates were collected as the control group. Language development was evaluated with "language/communication development milestone

The effect of blood transfusion protocol on retinopathy of prematurity: A prospective, randomized study.

OBJECTIVE: Controversy exists regarding the potential influence of anemia and blood transfusions on the rate of retinopathy of prematurity (ROP) in premature infants. A prospective, randomized, masked trial was performed to determine the influence of red blood cell transfusion protocol on ROP incidence and severity in a population of high-risk infants. METHODS: A total of 50 infants with birth weights <1251 g were divided randomly into two groups beginning on day of life 29. Group 1 (n = 24) received red cell transfusions during the 6-week study period, only if certain symptom-based guideline…

"No allogeneic blood transfusion" protocol for the surgical correction of craniosynostoses. I. Rationale.

Improved anesthesiological and surgical care has resulted in a progressively declining need for allogeneic blood transfusion. In infants with craniosynostosis, however, allogeneic blood transfusion is still performed as a routine procedure. In the present paper, the authors describe a protocol they have devised with the aim of limiting or even avoiding allogeneic blood transfusion even in very young patients, consequently avoiding the risks of infective or immunologic reactions associated with the procedure. The protocol is based on stimulation of the hematopoietic system with erythropoietin, …
