Osteopetrosis. Current clinical considerations.

Osteopetrosis is an inherited skeletal condition characterized by increased bone radiodensity. There are three clinical groups: infantile-malignant autosomal recessive, fatal within the first few years of life (in the absence of effective therapy); intermediate autosomal recessive, appears during the first decade of life but does not follow a malignant course; and autosomal dominant, with full-life expectancy but many orthopaedic problems. The infantile variant shows a myelophthisic anemia, granulocytopenia, and thrombocytopenia, and patients eventually die from infection or bleeding or both.

Bone disease in patients on maintenance haemodialysis using softened or de-ionized water.

A clinical, biochemical and radiological study was carried out in 28 patients on maintenance haemodialysis in order to assess the prevalence of bone disease, particularly with regard to osteomalacia and/or osteopenia. The patients were selected from dialysis units using softened or deionized water, and the aluminium levels measured in the different water supplies. The results showed that symptomatic osteomalacia/osteopenia occurs more frequently in the units using softened water, which has a higher aluminium content, than in the de-ionized water unit. The patients dialysed on softened water al

Sphenoid Dysplasia in Neurofibromatosis: Patterns of Presentation and Outcomes of Treatment.

BACKGROUND: Sphenoid wing dysplasia in patients with neurofibromatosis type 1 may result in challenging and significant changes, including ultimately vision loss. The authors describe the radiographic patterns of sphenoid dysmorphology with time and age, and the impact of surgical intervention on preservation of vision. METHODS: A retrospective study was performed at a single pediatric hospital, identifying subjects with neurofibromatosis type 1. Records were reviewed in their entirety for each subject, with attention to ophthalmologic evaluation. Radiographic images were digitally analyzed a

Evaluation of Trochlear Dysplasia Severity Using Trochlear Angle: A Retrospective Study Based on Computed Tomography (CT) Scans.

BACKGROUND Trochlear dysplasia is an important pathological factor in patellofemoral instability. Quantitative evaluation of the severity of trochlear dysplasia is rare and remains unclear. MATERIAL AND METHODS Computed tomography (CT) data on 136 knees (95 patients) with trochlear dysplasia in the case group and an age- and sex-matched cohort of 120 knees (70 patients) in the control group were collected for this retrospective study. All participants had undergone CT scans in the supine position. The trochlear angles (TA) formed by the posterior condylar line and the trochlear line were calcu

Guided growth for tibia vara (Blount's disease).

Blount's disease is commonly attributed to an intrinsic, idiopathic defect in the posteromedial proximal tibial physis resulting in progressive bowing of the leg, intoeing, and lateral knee thrust. Treatment has historically included bracing, physeal stapling, or corrective osteotomy, and was determined primarily by age at presentation. As we feel the pathology is not necessarily age dependent, we have elected to use the technique of guided growth using a lateral tension band plate to correct limb alignment as a first-line treatment in all patients presenting to our clinic as long as they had

A mild case of frontonasal dysplasia: the rhinologic perspective.

Frontonasal dysplasia (FND) is a congenital malformation characterized by hypertelorism, broad nasion with a midline cleft in the bony dorsum, midline defect of the frontal bone, absence of the nasal tip, and deformities in the nasal alar region. The clinician should be aware of the mild forms of FND. We presented absence of crista galli in a mild case of FND. Computed tomography scanning should assess the facial bones, nose, and paranasal structures. If a surgical correction is planned, this complete work-up prevents unexpected complications and complements the evaluation of paranasal deformi

Facial skeletal changes following hypertelorbitism correction.

This is a retrospective study of skeletal changes in 19 patients with corrected hypertelorism. A favorable outcome, defined as relapse less than 5 mm, occurred in patients with an average interorbital distance of 32 mm, whereas patients with an average interorbital distance of 40 mm tended to relapse over 5 mm. Neither age, interorbital configuration, nor diagnosis affected the stability of orbital translocation. At last evaluation (mean 6.7 years postoperatively), the mean interorbital distance was 22.4 mm in the favorable outcome group and 28.3 mm in the unfavorable category. This study sugg

[The metabolic basis of generalized skeletal dysplasia (author's transl)].

Numerous control systems are involved in the growth and differentiation of chondro-osseous tissue. Theoretically, the failure of each single step will result in a peculiar skeletal dysplasia. The resulting disorders are caused by errors in the metabolism of collagen, minerals, complex carbohydrates and the protein of the ground substance. Skeletal dysplasias with a known or probable metabolic base include osteogenesis imperfecta, osteoectasia with macrocranium, Menkes syndrome, the mucopolysaccharidoses and hypophosphatasia.

GGCX-related congenital combined vitamin K-dependent clotting factors deficiency-1: Description of a fetus with chondrodysplasia punctata.

Congenital combined vitamin K-dependent clotting factors deficiency (VKCFD) is a rare autosomal recessive disease resulting in hemorrhagic symptoms usually associated with developmental disorders and bone abnormalities. Pathogenic variants in two genes encoding enzymes of the vitamin K cycle, GGCX and VKORC1, can lead to this disorder. We present the case of a male fetus with a brachytelephalangic chondrodysplasia punctata (CDP), absence of nasal bone, growth restriction, and bilateral ventriculomegaly at 18 weeks of gestation. Pathological examination showed a Binder phenotype, hypoplastic di

A new chondrodystrophic mutant in mice. Electron microscopy of normal and abnormal chondrogenesis.

The occurrence of a new mutation affecting cartilage and bone in mice is reported. The gene is lethal, shows autosomal recessive inheritance, and has high penetrance. It is not allelic to shorthead and probably not to phocomelia or achondroplasia. It results in a foreshortened face, cleft palate, defective trachea, and shortened long bones with flared metaphyses. Chondrocytes of epiphyseal cartilage from the mutant are not aligned in columns, and there is a decrease in the usual staining of the cartilage matrix. Electron microscope observations show large, wide collagen fibrils with "native" b

Dural Sinus Malformation Involving a Persistent Transtentorial Sinus: A Rare Variant.

This case report details a patient born with dural sinus malformation of the transtentorial sinus complicated by thrombosis, which caused compression of the fourth ventricle, venous infarction of the adjacent cerebellum, and disseminated intravascular coagulation. The enlarged, malformed pouch involved a transtentorial venous sinus, a sinus located in the tentorium cerebelli along the inner surface of the occipital bone that usually regresses before birth. The diagnosis was missed because of the rare presentation of a rare disease. In this case review, a multidisciplinary approach resulted in

Treatment of craniofacial fibrous dysplasia: a case report.

The concept of "fibro-osseous lesions" of bone has evolved over the last several years and now includes two mayor entities: the fibrous dysplasia (FD) and the cement-ossifying fibroma (COF). Fibrous dysplasia is considered to be a developmental, tumor-like (hamartomatous), fibro-osseous disease of unknown etiology. There is a maxillary predominance when craniofacial FD occurs in the jaws and the maxillary sinus is commonly involved. Differentiation of OF from FD is important because of differences in treatment and behaviour. This article report a case of 60-year-old female with a history of pa

Fibro-osseous Lesions of the Jaw: A Report of Two Cases.

Fibro-osseous lesions of the maxillofacial bones comprise a diverse group of pathologic conditions that include developmental lesions, reactive or dysplastic diseases, and neoplasms. The concept of fibro-osseous lesions has evolved over the last several decades and now includes two major entities: fibrous dysplasia and ossifying fibroma. The less common lesions include florid osseous dysplasia, periapical dysplasia, focal sclerosing osteomyelitis, proliferative periostitis of Garre, and osteitis deformans. We report two cases, the first, a craniofacial polyostotic fibrous dysplasia, and the se

Dental defects of congenital syphilis.

Diagnosis of the congenital form of syphilis is an important part of the palaeopathology of this disease. In theory, there are clear clinical signs to be found in the long bones and teeth, but it has rarely been possible to recognise the latter with a confidence in archaeological material, partly because the original descriptions of the dental deformities are sometimes contradictory and partly because it is nowadays difficult to find reference specimens in museums. This article describes two such specimens which have recently been rediscovered, and discusses the form of the dental defects whic

A novel Fanconi anaemia subtype associated with a dominant-negative mutation in RAD51.

Fanconi anaemia (FA) is a hereditary disease featuring hypersensitivity to DNA cross-linker-induced chromosomal instability in association with developmental abnormalities, bone marrow failure and a strong predisposition to cancer. A total of 17 FA disease genes have been reported, all of which act in a recessive mode of inheritance. Here we report on a de novo g.41022153G>A; p.Ala293Thr (NM_002875) missense mutation in one allele of the homologous recombination DNA repair gene RAD51 in an FA-like patient. This heterozygous mutation causes a novel FA subtype, 'FA-R', which appears to be the fi

The paradox of FGFR3 signaling in skeletal dysplasia: why chondrocytes growth arrest while other cells over proliferate.

Somatic mutations in receptor tyrosine kinase FGFR3 cause excessive cell proliferation, leading to cancer or skin overgrowth. Remarkably, the same mutations inhibit chondrocyte proliferation and differentiation in developing bones, resulting in skeletal dysplasias, such as hypochondroplasia, achondroplasia, SADDAN and thanatophoric dysplasia. A similar phenotype is observed in Noonan syndrome, Leopard syndrome, hereditary gingival fibromatosis, neurofibromatosis type 1, Costello syndrome, Legius syndrome and cardiofaciocutaneous syndrome. Collectively termed RASopathies, the latter syndromes a

P450 oxidoreductase deficiency - a new form of congenital adrenal hyperplasia.

Patients with adrenal insufficiency, genital anomalies and bony malformations resembling the Antley- Bixler syndrome (a craniosynostosis syndrome), are likely to have P450 oxidoreductase (POR) deficiency. Since our first report in 2004, about 26 recessive POR mutations have been identified in 50 patients. POR is the obligate electron donor to all microsomal (type II) P450 enzymes, including the steroidogenic enzymes CYP17A1, CYP21A2 and CYP19A1. POR deficiency may cause disordered sexual development manifested as genital undervirilization in 46,XY newborns as well as overvirilization in those

[Constitutional familial leucocytopenia with partial Pelger anomaly and delayed development of bone (author's transl)].

The clinical and hematological findings in 4 boys of a family with a chronic constitutional leukocytopenia are described. Possible X-chromosomal sex-linked genetic transmission, the benign clinical course as well as the association with clinical and cytological hints of the involvement of the lymphatic cellular immunological system suggest a disease of its own. It was classified as a mild form of myelolymphatic insufficiency similar to reticular dysgenesis. The significance of Pelger-Huët anomalies of neutrophils demonstrated in the blood of the mother and of Pelger-like forms in the blood of
