Precision immunomodulation for pediatric hemophagocytic lymphohistiocytosis in intensive care.

Hemophagocytic lymphohistiocytosis (HLH) and related cytokine storm syndromes are life-threatening hyperinflammatory disorders that frequently intersect with pediatric critical care. Critically ill children may present with fever, shock, cytopenias, liver dysfunction, coagulopathy, hyperferritinemia, respiratory failure, neurologic deterioration, and multiple organ dysfunction, creating substantial overlap with severe sepsis. Traditional diagnostic criteria remain foundational but do not identify the dominant mechanism or optimal therapy for an individual child. In this review, precision immun

The role of manganese-based MRI contrast agents for cancer theranostics: Where do we stand in 2025?

Magnetic resonance imaging (MRI) guidance in the realm of anticancer therapy is crucial to visualize the spread of tumors in deep tissues, accumulate the therapeutics, and trigger them for precise therapy. Recent studies bridge this gap by integrating MRI contrast agents (CAs) with different therapeutic regimes for a better outcome. In this context, manganese-based materials hold great potential owing to their T1/T2 dual-modal MR-relaxation, less toxicity, and other therapeutic capabilities such as chemodynamic therapy, immunotherapy, etc., which have gained increasing interest among researche

Single arterial access ECMELLA: A new concept and step-by-step procedure.

Therapy for cardiogenic shock using temporary mechanical circulatory support has improved significantly in the last decades, providing patients with new technologies for both acute phase stabilization and bridging to long-term therapies. A combination of a venoarterial extracorporeal life support system and the Impella left ventricular assist device (known as the ECMELLA approach) represents an effective therapy for severe cardiogenic shock that achieves high-flow circulatory support with simultaneous left-ventricular unloading. We present the new ECMELLA 2.0 concept, whereby a single arterial

Switching patients at high risk of PML from natalizumab to another disease-modifying therapy.

There are several options for switching people with multiple sclerosis (MS) who are at high risk of developing progressive multifocal leukoencephalopathy (PML) from natalizumab to alemtuzumab. However, some of these have risks that need to be managed, for example, the risks of carrying over asymptomatic PML from natalizumab on to the new therapy, and the risk of rebound disease activity associated with a prolonged washout after starting natalizumab. We propose a pragmatic bridging strategy, using another disease-modifying therapy (DMT), to reduce the risk of switching from natalizumab to alemt

Meta-analysis Comparing the Efficacy of Dobutamine Versus Milrinone in Acute Decompensated Heart Failure and Cardiogenic Shock.

This study aims to evaluate the difference between dobutamine and milrinone in patients presenting with acute decompensated heart failure (AHF). Inotropes are indicated for treating AHF, especially in patients with concomitant hypoperfusion indicative of cardiogenic shock. However, previous studies have not identified the optimal inotrope. We sought to compare outcomes associated with milrinone versus dobutamine in patients with AHF. A systematic literature search was performed to identify relevant trials from inception to August 2021. Our primary outcome of interest was mortality. Analysis wa

Extracorporeal membrane oxygenation for idiopathic inflammatory myopathy-associated interstitial lung disease: a narrative review.

BACKGROUND AND OBJECTIVE: Idiopathic inflammatory myopathy-associated interstitial lung disease (IIM-ILD), particularly its rapidly progressive form (RP-ILD), carries a grave prognosis, with refractory respiratory failure being a leading cause of death. For patients with IIM-ILD who progress to refractory respiratory failure, extracorporeal membrane oxygenation (ECMO) can bridge patients to recovery or transplantation. This review synthesizes existing literature on the role of ECMO in managing IIM-ILD with severe respiratory failure. METHODS: PubMed was systematically searched from its incept

Insights from the Greek experience of the use of Blinatumomab in pediatric relapsed and refractory acute lymphoblastic leukemia patients.

Pediatric refractory or relapsed acute lymphoblastic leukemia (ALL) poses unique therapeutic challenges, with novel immunotherapy approaches offering potential cure opportunities. In this frame, the use of Blinatumomab may induce durable remissions, serving as a successful bridge to allogeneic hematopoietic stem cell transplantation (allo-HSCT). Herein, we retrospectively summarize the Greek experience on pediatric relapsed/refractory B-cell precursor ALL patients that were treated with Blinatumomab in a compassionate, off-label setting as an effort to achieve disease clearance and proceed to

Ozone therapy in femoral head necrosis: a perspective on biomarker-guided treatment rationalization and mechanistic exploration.

Femoral head necrosis (FHN) is a progressive and disabling disorder caused by compromised blood supply to the femoral head. Current treatments primarily aim to slow progression. However, they often fail to reverse early-stage necrosis or reliably prevent disease advancement. Importantly, most approaches lack precise molecular targets for early intervention. Ozone therapy has emerged as a potential option because of its pleiotropic biological effects, such as modulating oxidative stress, improving microcirculation, and exerting anti-inflammatory activity. However, its mechanisms remain poorly u

Isolated Extreme Thrombocytosis as the Presenting Feature of Chronic Myeloid Leukemia-An Unusual Phenotype Treated With Plateletpheresis.

Chronic myeloid leukemia (CML) can rarely present with extreme thrombocytosis as the initial manifestation, a phenotype that can mimic essential thrombocythemia, leading to delayed diagnosis. We describe the case of a 54-year-old woman who was evaluated by outpatient hematology for isolated thrombocytosis with normal iron studies and leukocyte counts. Molecular testing confirmed BCR::ABL1 fusion, consistent with chronic phase CML. She was started on hydroxyurea, but her platelet count continued to rise, requiring hospitalization for close monitoring, where she was initiated on dasatinib, a tyr

Beyond Watermelon Stomach: Risk Factors and Treatment of Recurrent Bleeding in Gastric Antral Vascular Ectasia (GAVE).

Gastric Antral Vascular Ectasia (GAVE) is a rare but clinically significant cause of chronic gastrointestinal bleeding and transfusion-dependent anemia, particularly in older women. Patients with comorbidities, such as cirrhosis, chronic kidney disease, or autoimmune disorders, are at risk of developing GAVE. Pharmacological therapies demonstrate limited efficacy in the management of GAVE. They are primarily utilized as a temporary measure, serving as a bridge to definitive therapy. Additionally, these agents may be considered for patients with contraindications to endoscopic or surgical inter

Post-Transplant Lymphoproliferative Disorder Presenting as Biventricular Failure After Heart Transplantation.

BACKGROUND: The diagnosis of post-transplant lymphoproliferative disorder (PTLD) is challenging because of its diverse clinical presentations. CASE SUMMARY: A 65-year-old man, previously stable 61 months after heart transplantation for dilated cardiomyopathy, presented with biventricular failure and worsening dyspnea. Given the acute decline in graft function, acute rejection was suspected, and rejection therapy was initiated. However, the patient showed no clinical improvement, and he required venoarterial extracorporeal membrane oxygenation support as a bridge to redo heart transplantation.

Management of Melasma: Laser and Other Therapies-Review Study.

Melasma is a commonly occurring pigmented skin condition that can significantly affect one's appearance, described as symmetric hyperpigmentation that presents as irregular brown to gray-brown macules on various facial areas, such as the cheeks, forehead, nasal bridge, and upper lip, along with the mandible and upper arms. Due to its complex pathogenesis and recurrent nature, melasma management is challenging and the outcomes following treatment are not always deemed satisfactory. Solely treating hyperpigmentation may prove ineffective unless paired with regenerative techniques and photoprotec

What the pediatric nurse needs to know about the Impella cardiac assist device.

Background: Cardiogenic shock in children still carries a high mortality risk despite advances in medical therapy. The use of temporary mechanical circulatory supports is an accepted strategy to bridge patients with acute heart failure to recovery, decision, transplantation or destination therapy. These devices are however limited in children and extracorporeal membrane oxygenation (ECMO) remains the most commonly used device. Veno-arterial ECMO may provide adequate oxygen delivery, but it does not significantly unload the left ventricle, and this may prevent recovery. To improve the likelihoo

Top five considerations for improving outcomes in black patients with heart failure: A guide for primary care clinicians.

Black patients develop heart failure at younger ages and have worse outcomes such as higher mortality rates compared to other racial and ethnic groups in the United States. Despite significant recent improvements in heart failure medical therapy, these worse outcomes have persisted. Multiple reasons have been provided to explain the situation, including but not limited to higher baseline cluster of cardiovascular risk factors amongst Black patients, inadequate use of heart failure guideline directed medical therapy and delayed referral for advanced heart failure therapies and interventions.
