The conduction bundle at the atrioventricular junction. An anatomical study.

The variability in the topographical anatomy of the conduction bundle at the site of the atrioventricular junction has been studied in four normal human hearts. The junctional area has been removed en bloc and serially sectioned. The conduction bundle and adjacent structures such as the posterior limb of the trabecula septomarginalis and the membranous septum have been reconstructed based on calculations from the histological sections. The study reveals marked variability particularly in the extent of the posterior limb of the trabecula septomarginalis. In one instance, the muscle was almost t

Ablation therapy for patients with supraventricular tachycardia.

Over the past decade, a more complete understanding of the mechanisms of supraventricular tachycardia has made possible the development of surgical and catheter techniques designed to control these arrhythmias. Ablative techniques are generally used when the arrhythmia proves refractory to drug and/or antitachycardia pacing therapy or when lifelong drug therapy is believed to be less desirable than a potentially curative procedure. In this essay, we explore the known mechanisms for supraventricular arrhythmias as well as the rationale for the use of ablative techniques.

Maintained prolongation of AV conduction time by acetylcholine.

Although chronotropic responses to maintained cholinergic activation have been reported to fade during tonic vagal stimulation, discrepancy exists as to whether AV conduction behaves similarly. Since previous studies have examined only low grade AV block, we sought to more fully characterize the dromotropic responses to different degrees of maintained cholinergic activation. We used anesthetized mongrel dogs in which the nutrient artery supplying the AV node region was cannulated and perfused with either autologous blood or oxygenated Tyrode solution containing various concentrations of acetyl

Damage and healing in the conducting tissue of the heart (an experimental study in dogs infected with Trypanosoma cruzi).

The consequences of acute inflammatory damage and chronic repair in the conducting system of the canine heart were observed after experimental Trypanosoma cruzi infection of young dogs. Acute infection lasted for 18-25 days and caused hyaline and lytic necrosis of the atrio-ventricular conducting fibres, the sinus node and the neurons in the cardiac para-sympathetic ganglia. Dogs that survived acute disease were examined after periods from 8 months to 3 years. Three main types of healing were found in the conducting system: fibrosis (diffuse and/or focal), sclero-atrophy and fatty infiltration

[New diagnostic method in arrhythmias and heart conduction disorders].

The authors developed a method for recording spike potentials of the heart conduction system from the body surface on a curve of differentially intensified spike potential (DISP). They examined 187 patients and 60 healthy persons. The spike potentials of the sinoatrial node, atria and the bundle of His recorded on the DISP curves coincide exactly in time with spike potentials on intracardiac ECG recorded by microcatheterization of the heart through the subclavian vein, which makes it possible to distinguish on the DISP curve reference points for determining conductivity on definite areas of th…

Elimination of the delta wave in the Wolff-Parkinson-White syndrome. A misleading indicator of therapeutic effectiveness.

A 66-year-old white woman with a greater than 20-year history of electrocardiographic evidence of the Wolff-Parkinson-White syndrome, including documented recurrent supraventricular tachycardias, was studied. Despite the disappearance of the delta wave after initiation of therapy with digoxin and quinidine sulfate, the patient continued to have frequent episodes of supraventricular tachycardia. At a time when the serum levels of digoxin and quinidine were in the therapeutic range, extensive electrophysiologic studies were performed. Supraventricular tachycardia at a rate of 160 beats per minut

Electrophysiologic delineation of the specialized atrioventricular conduction system in two patients with corrected transposition of the great arteries in situs inversus (I,D,D).

Electrophysiologic delineation of the atrioventricular conduction system at surgery is described in two patients with corrected transpostion of the great arteries in situs inversus. Intra-atrial electrograms were recorded in one patient from sites immediately adjacent to the coronary sinus located in the left-sided right atrium. The intraventricular portion of the atrioventricular conduction system was identified in both patients along the posterior and inferior margin of the ventricular septal defect, in contrast to the superior and anterior location found in corrected transposition of the gr

Bifascicular block complicating blunt cardiac injury. A case report and review of the literature.

A thirty-five-year-old horse trainer presented to the emergency room of the authors' hospital with minimal nonpenetrating chest injury after having been kicked by a horse. No rib or sternum fractures were demonstrated. The admission ECG demonstrated a right bundle branch block and a left anterior hemiblock that were previously absent. The authors are aware of only two similar reports, but analogous conduction disturbances might have been classified as intraventricular conduction defects. The rarity of these defects may be explained by the anatomic pathways of the bundle of His and its bifurcat…

[Idiopathic sustained ventricular tachycardia in the young adult with right bundle-branch and left axis deviation].

The authors report 4 cases of sustained ventricular tachycardia with right bundle branch block and left axis deviation morphology. These ventricular tachycardias which are usually sensitive to Verapamil are often mistaken for supraventricular tachycardia. However, they are a specific clinical, electrocardiographic and electrophysiological entity. The origin of the tachycardia is probably in the Purkinje fibres of the left posterior hemibranch of the His bundle. The mechanism is controversial: much evidence points to reentrant phenomenon, but, in the present state of our knowledge, triggered ac

Abolition and modification of reentry within the His-Purkinje system by procainamide in man.

The effects of intravenous procainamide infusion of 10--14 mg/kg body weight (i.e., 750 mg) of procainamide (PA) on reentry within the His-Purkinje system (HPS) were studied in 13 patients using His bundle electrograms and ventricular extrastimulus method. PA abolished reentry in eight patients (group 1) and decreased the width of reentry zone in the remaining five (group 2). At comparable S1S2 intervals, the S2H2 intervals after PA were longer than control in all patients. In group 1 patients, after PA, reentry did not occur even at S2H2 intervals that were significantly longer than control c

Pitfalls in diagnosis of atypical atrioventricular nodal reentrant tachycardia by a conventional electrophysiological study.

BACKGROUND: During atypical atrioventricular nodal reentrant tachycardia (AVNRT), the earliest atrial activation site following retrograde slow pathway (SP) conduction is at the atrial exit of the left inferior extension (LIE) in the coronary sinus (CS) or the right inferior extension (RIE) on the tricuspid annulus (TA). OBJECTIVE: The purpose of this study was to assess the validity of a conventional 2-catheter (His bundle and CS) mapping method for localizing the atrial ends of these extensions. METHODS: We retrospectively evaluated the efficiency of electrode placement-based mapping of th

Transcatheter ablative techniques for treatment of the permanent form of junctional reciprocating tachycardia in young patients.

Five patients with medically refractory incessant supraventricular tachycardia due to a posterior septal, slowly conducting accessory connection underwent transcatheter closed chest ablative treatment. The tachycardia characteristics were consistent with the permanent form of junctional reciprocating tachycardia. In each patient the ablative attempts resulted in independent interruption of either the anterograde limb (atrioventricular node-His bundle conduction) or the retrograde limb (accessory connection) of the tachycardia circuit. Permanent retrograde pathway ablation was achieved in only

Radiofrequency ablation of a slow pathway in AV-nodal tachycardia.

A 35-year-old woman with frequent attacks of paroxysmal supraventricular tachycardia and psychiatric background was submitted to ablation of AV-nodal tachycardia. Before ablation, tachycardia with a cycle length of 455 ms was easily induced with regular atrial stimulation or with one extrastimulus. Radiofrequency current (35 Watts) was applied on a site, localized between the His bundle and the ostium of the coronary sinus. The local electrogram showed an atrial activation 20 ms before the atrial activation at the His bundle site during tachycardia. Application terminated AV-nodal tachycardia

[Left ventricle in Noonan's syndrome. Electro-vecto-echo and angiocardiographic aspects].

The electrocardiographic features of Noonan's syndrome have been known for several years, but the discordance between these electrical findings and the underlying haemodynamic disorders remains unexplained. In an attempt to elucidate the genesis of electrical abnormalities, we present here a retrospective study of 14 children with Noonan's disease, aged from a few days to 16 years and evaluated by electrocardiography, vectography, one- or two-dimensional echocardiography, angiography and His bundle electrophysiology. The electrocardiographic abnormalities observed concerned ventricular depolar

Synchronous ventricular pacing with direct capture of the atrioventricular conduction system: Functional anatomy, terminology, and challenges.

Right ventricular apical pacing is associated with an increased incidence of heart failure, atrial fibrillation, and overall mortality. As a result, pacing the ventricles in a manner that closely mimics normal AV conduction with an intact His-Purkinje system has been explored. Recently, the sustainable benefits of selective His-bundle stimulation have been demonstrated and proposed as the preferred method of ventricular stimulation for appropriate patients. Ideally, conduction system pacing should be selective without myocardial capture, overcome distal bundle branch block when present, and no

Supraventricular tachycardia originating from the posterior atrioventricular node in the univentricular heart with single atrium.

Supraventricular tachycardia (SVT) was observed in a 13-year-old male patient with complex clinical features that included univentricular heart with single atrium, pulmonary atresia, and polysplenia syndrome. During electrophysiologic study, atrial burst stimuli reproducibly induced and terminated the SVT, while the occurrence of ventriculoatrial block did not interrupt the SVT. His bundle electrograms (HBEs) were recognized both in the anterior and posterior regions on the common atrioventricular (AV) valve annulus. The posterior His bundle activation was progressively delayed along with the
