[Foramen retroarticulare atlantis and the vertebral angiogram (author's transl)].

The incidence of a foramen retroarticulare atlantis in a group of 380 patients was investigated and put in relation to the frequency of headaches, migraine and Meniere's syndrome. These complaints are found more frequently in patients with a retroarticular foramen. Angiograms of the vertebral artery show vascular constrictions in the region of the foramen retroarticulare in the majority of patients examined. In 5 patients, the clinical symptoms of a mechanical irritation with changes in blood flow through the vertebral artery in the region of a foramen retroarticulare atlantis have been descri

Novel Approach to Retro-Odontoid Pseudotumor Resection without Atlantoaxial Dislocation.

BACKGROUND Retro-odontoid pseudotumor is a fibrous or pannus mass behind the odontoid process, mainly caused by atlantoaxial instability, degeneration, or abnormal deposits behind the dens. Most scholars agree immobilization of the upper cervical spine can solve this problem; the mass can absorb spontaneously. However, retro-odontoid pseudotumor cases without atlantoaxial instability are limited, and it remains controversial whether the mass should be removed. CASE REPORT A 67-year-old man had intermittent neck pain for 10 years and numbness and weakness of the extremities for 1 month. Primary

A Surgical Case of Bow Hunter's Syndrome Diagnosed by Cervical Rotational MRA.

Bow hunter's syndrome is an ischemic manifestation of vertebrobasilar artery (VA) insufficiency due to stenosis or occlusion of the contralateral VA at the bony elements of the atlas and axis during neck rotation. In early reports, VA stenosis at the craniovertebral junction was the main cause, but later, symptoms due to VA occlusion at the middle and lower cervical vertebrae were also included in this pathology. Although the confirmed diagnosis is usually determined by dynamic digital subtraction angiography (DSA), we have experienced a method of minimally invasive MR angiogram (MRA) that pro

Classification of internal carotid artery aberrations based on CTA imaging: evaluating the risk of ICA injury in the transoral approach to upper cervical surgery.

OBJECTIVE: Cervical internal carotid artery (ICA) aberrations increase transoral atlantoaxial surgery risk. Existing classifications ignore the ICA's relationship with the atlanto-axial complex. This study aimed to define this relationship using CTA and assess associated injury risk. METHODS: The study included 846 patients underwent CTA from January 2019 to February 2024 in our medical center. These classifications were based on the position of aberrant ICAs relative to the odontoid processes and the lateral masses of the atlas. RESULTS: In a cohort of 846 patients with a total of 1,692 ICA

Inpatient outcomes of upper cervical fractures in the elderly: a retrospective analysis of 268 cases.

BACKGROUND: The incidence of upper cervical fractures in elderly individuals is increasing, necessitating enhanced treatment approaches. METHOD: A retrospective study of 268 elderly patients with upper cervical fractures was conducted to assess inpatient outcomes aged 75 and older with atlas and/or axis fractures. Patient risk was evaluated using the ASA score and Charlson comorbidity index (CCI). In-hospital mortality and functional outcomes were assessed, with fracture treatment strategies following AO principles. RESULTS: Patients with C1, C2 or combined fractures did not differ in age, C

Atlanto-axial rotatory subluxations in postmortem CT: radiologists be aware of a common pitfall.

The aim of the present study was to determine the frequency of atlanto-axial rotatory subluxations (AARS) in multi detector computed tomography (MDCT) performed on human corpses for forensic purposes and to investigate whether these are a physiological postmortem finding or indicate a trauma to the neck region. 80 forensic cases examined with MDCT from November 2003 to March 2007 were included in the study. The study was approved by the regional ethics committee. For each case volumes were rendered and investigated with reference to suspected AARS and any other anomalies of the head and neck r

Individualized 3D printing navigation template for pedicle screw fixation in upper cervical spine.

PURPOSE: Pedicle screw fixation in the upper cervical spine is a difficult and high-risk procedure. The screw is difficult to place rapidly and accurately, and can lead to serious injury of spinal cord or vertebral artery. The aim of this study was to design an individualized 3D printing navigation template for pedicle screw fixation in the upper cervical spine. METHODS: Using CT thin slices data, we employed computer software to design the navigation template for pedicle screw fixation in the upper cervical spine (atlas and axis). The upper cervical spine models and navigation templates were

SOX17: A Highly Sensitive and Specific Immunomarker for Ovarian and Endometrial Carcinomas.

PAX8 is the most commonly used immunomarker to link a carcinoma to the gynecologic tract; however, it lacks specificity. Through mining The Cancer Genome Atlas mRNA expression profile data, we identified SOX17 as a potential specific marker at the mRNA level for gynecologic tumors. To evaluate the utility of this marker in the identification of the gynecologic origin of a given carcinoma, we performed immunochemical staining in a large cohort of ovarian and endometrial cancer cases (n = 416), together with a large cohort of solid tumors from other organs (n = 1544) in tissue microarrays. Cervical to Petrous Carotid Artery High-Flow Bypass for Carotid Artery Pseudoaneurysm Through Zygomatic Approach: 2-Dimensional Operative Video.

Pseudoaneurysms of the cervical internal carotid artery may generate grave risk from catastrophic rupture, thromboembolic stroke, or mass effect. They have many causes, including malignancy, infection, and iatrogenic and most commonly blunt or penetrating trauma.1 These aneurysms require treatment to eliminate their risk. Treatment options include trapping, with or without revascularization, or endovascular stenting. Trapping without revascularization requires evaluation of the cerebral collateral under a physiological challenge, which is usually done with a balloon occlusion test, which is no

The post-occipital spinal venous sinus of the Nile crocodile Crocodylus niloticus: its anatomy and use for blood sample collection and intravenous infusions.

The post-occipital sinus of the spinal vein is often used for the collection of blood samples from crocodilians. Although this sampling method has been reported for several crocodilian species, the technique and associated anatomy has not been described in detail in any crocodilian, including the Nile crocodile (Crocodylus niloticus). The anatomy of the cranial neck region was investigated macroscopically, microscopically, radiographically and by means of computed tomography. Latex was injected into the spinal vein and spinal venous sinus of crocodiles to visualise the regional vasculature. Th

Intra-patient semi-automated segmentation of the cervix-uterus in CT-images for adaptive radiotherapy of cervical cancer.

For online adaptive radiotherapy of cervical cancer, fast and accurate image segmentation is required to facilitate daily treatment adaptation. Our aim was twofold: (1) to test and compare three intra-patient automated segmentation methods for the cervix-uterus structure in CT-images and (2) to improve the segmentation accuracy by including prior knowledge on the daily bladder volume or on the daily coordinates of implanted fiducial markers. The tested methods were: shape deformation (SD) and atlas-based segmentation (ABAS) using two non-rigid registration methods: demons and a hierarchical al

Widespread choroid plexus contamination in sampling and profiling of brain tissue.

The choroid plexus, a tissue responsible for producing cerebrospinal fluid, is found predominantly in the lateral and fourth ventricles of the brain. This highly vascularized and ciliated tissue is made up of specialized epithelial cells and capillary networks surrounded by connective tissue. Given the complex structure of the choroid plexus, this can potentially result in contamination during routine tissue dissection. Bulk and single-cell RNA sequencing studies, as well as genome-wide in situ hybridization experiments (Allen Brain Atlas), have identified several canonical markers of choroid

Motion-preserving reduction and fixation of C1 Jefferson fracture using a C1 lateral mass screw construct.

The treatment of C1 Jefferson fractures is controversial. Non-surgical treatment with halo fixation always bears the risk of insufficient healing with further instability and increasing neck pain. However, a C1-2 fusion can markedly decrease the rotatory motion of the neck. The aim of this report is to describe a new treatment for C1 Jefferson fractures. We used open reduction and C1 fixation using a bilateral C1 lateral mass screw construct. The screws were connected with a rod and nuts to reduce lateral spread of the lateral masses. This method is an alternative surgical option for C1 Jeffer

Jefferson fractures of the immature spine. Report of 3 cases.

Jefferson fractures of the immature spine have received little attention in the study of pediatric spinal trauma. Fractures through synchondroses are a possibility in the immature spine, in addition to fractures through osseous portions of the vertebral ring, and they create opportunities for misinterpretation of diagnostic imaging. The authors describe 3 examples of Jefferson fractures in young children. All 3 cases featured fractures through an anterior synchondrosis in association with persistence of the posterior synchondrosis or a fracture of the posterior arch. The possibility of a Jeffe

Classification and clinical anatomy of the first spinal nerve: surgical implications.

OBJECT: Data regarding the first cervical nerve are scanty and conflicting, however this nerve may need to be identified for neurosurgical procedures such as rhizotomy for torticollis and suboccipital pain syndromes. The authors performed the present study to elucidate further the detailed anatomy of the first cervical nerve and review its clinical relevance. METHODS: Forty adult cadavers (80 sides) were used in this study. Dissection was performed at the craniocervical junction with special attention to the formation and presence of the components of the C-1 spinal nerve. Additionally, conne

The combined transmastoid retro- and infralabyrinthine transjugular transcondylar transtubercular high cervical approach for resection of glomus jugulare tumors.

Complex tumors OF the glomus jugulare present a surgical challenge because of their difficult location, extreme vascularity, and involvement with multiple cranial nerves. Modern microneurosurgical and cranial base techniques have enabled safe total removal of these complicated tumors. We describe a one-stage transjugular posterior infratemporal fossa approach for radical resection of glomus jugulare tumors located around the jugular foramen, the lower clivus, and the high cervical region from an anterolateral direction. This approach is a combination of transmastoid, suprajugular, transjugular

Posterior arch bifocal fracture of the atlas vertebra: a variant of Jefferson fracture.

Fracture of the atlas vertebra is rare in children. We report two paediatric cases of bifocal pedicular fracture of the posterior arch of C1. Evaluation was performed by nonenhanced computed tomography scan, which successively confirmed both diagnosis and healing. In both cases, nonoperative management was successful.

Traumatic basal subarachnoid hemorrhages: autopsy material analysis.

Fourteen cases of traumatic basal subarachnoid hemorrhages, examined in the years 1980-1988, were presented. Bleeding was connected with the fracture of the transverse process of the atlas in eleven cases and with atlanto-occipital dislocation in three. The source of hemorrhage--vertebral artery rupture--was determined only four times. In nine cases (63%) atlases showed various developmental malformations (posterior ponticle and foramen arcuale, epitransverse process, incomplete ossification of the transverse process). The results were related to the literature of the subject. Special attentio
