Choroidal malign melanoma showing rapid progression in a boy 17 years old is presented. The patient was under therapy with methoxy psoralen + Ultraviolet A (PUVA)* for psoriasis vulgaris. He had undergone complete ophthalmologic examinations with three-month intervals from the beginning of this therapy. The tumor was detected in the seventh month, rapidly enlarging within the next twenty days, and eventually leading to enucleation. PUVA is known to be a risk factor for cutaneous malign melanoma. A 68-year-old man had visual loss secondary to isolated choroidal nonperfusion as a clinical manifestation of giant cell arteritis. Ophthalmoscopy disclosed scattered yellow-white lesions at the level of the retinal pigment epithelium in the posterior pole of the right eye. Eight choroidal melanomata and 3 suspected naevi were observed in 11 diabetics. One xenon-arc treated tumor was followed up for 7 years; two tumors had Strontium90 radioactive plaque brachytherapy and their follow up was 3 years and 6 years.

A 54-year-old Chinese woman with miliary choroidal tuberculosis was followed for more than three years. She had had tuberculous meningitis for about one month before an ophthalmologic examination for blurred vision OU. Twenty-four patients (31 eyes) with choroidal folds unassociated with orbital tumors were evaluated with standardized echography. Hypermetropia was the most commonly associated finding (eight eyes); in ten eyes, no consistent abnormal findings could be established. Among the less common causes were swelling of the optic nerve proper or the perineural sheaths and thickened extraocular muscles. Wegener's granulomatosis and lymphomatosis are systemic necrotizing vasculitides of unknown cause. We report similar involvement of the choriocapillaris in these two diseases. A presumed choriocapillaritis caused ischemia and infarction of the choriocapillaris-retinal pigment epithelium-outer neurosensory retina complex. Systemic corticosteroid and immunosuppressive agents were successful in controlling systemic disease and preserving useful vision. At the time of this study, one patient had not had a recurrence for 12 years, and the other patient had been asymptomatic for seven years.

Two cases of spontaneous pseudophakic ciliochoroidal detachment, associated with extracapsular cataract extraction, posterior chamber intraocular lens implantation with intact posterior capsules, and small cyclodialysis clefts are documented. In both patients, the choroidal detachments occurred in a delayed fashion following uncomplicated cataract surgery and both presented with characteristic ocular hypotony. Although one patient required surgical management, both patients regained excellent vision after closure of the clefts and resolution of the ciliochoroidal detachments.

Malignant melanoma is the most frequently encountered primary intraocular neoplasm. Within the eye this tumour arises in the uveal tract where the choroid is affected more often than the iris or ciliary body. The tumour occurs typically in middle life and the aetiology is not known. Uveal tissue is heavily pigmented in all races but ocular melanoma is notably rare in negroes. There is clinical and pathological evidence that malignant change may take place in a choroidal naevus, but melanoma has arisen in eyes documented to have no pre-existing pigmented lesion.

Approximately 50% of patients undergoing enucleation for choroidal melanoma will eventually die of metastases. The majority of tumor related deaths will occur within 5 years of enucleation, but late metastases may occur 20-30 years after enucleation. We report the first case of diffuse choroidal atrophy associated with Klinefelter syndrome. Retinal findings included midperipheral bone corpuscular pigmentation, large areas suggestive of choroidal atrophy, and unusual golden crystalline structures that apparently were mainly in the neurosensory retina. A sensorineural hearing deficit was also noted. Biochemical studies, including amino acid blood levels, were within normal limits. This case is of interest because of the rarity of association between Klinefelter syndrome and retinal or, in fact, any ocular abnormalities.
