BACKGROUND: Most patients who develop metastatic carcinoma to the choroid are managed by local radiation or chemotherapy. Since transpupillary thermotherapy (TTT) is currently gaining attention as an optional treatment for choroidal melanomas and hemangiomas, we sought to determine whether TTT is suitable for treatment of solitary choroidal metastasis at the posterior pole.

We designed a suction attachment for the radiosurgical treatment of intraocular malignancies with the Leksell gamma unit (Gamma Knife). Our device consists of a circular suction chamber and an adjustable unit to be fixed to the Leksell stereotactic head frame. All components are made of plastic materials in order to avoid artifacts in CT or MRT imaging. A permanent suction of 600-800 mbar is provided by a standard vacuum pump, powered by a portable battery. Suction times up to 40 min were well tolerated in all cases. Investigations concerned a group of 15 patients with an intraocular tumor diagnosed by clinical examination, ultrasonography and computer tomography. The record of the rheographical curve of the healthy and affected eye was performed. In all the cases the eye was enucleated and histopathological examination done. In the evaluation of results attention was called to a much higher altitude of the curve in the eye with the tumor which could evidence an increased blood flow in the eye ball.
