BACKGROUND: This retrospective study aims to predict the risk of 3-month nonunion in patients with unstable distal clavicle fractures (UDCFs) treated with open reduction and internal fixation (ORIF) using machine learning (ML) methods. ML was chosen over traditional statistical approaches because of its superior ability to capture complex nonlinear interactions and to handle imbalanced datasets.
METHODS: We collected UDCFs patients at Nanjing Luhe People's Hospital (China) between January 2015 and May 2023. The unfavorable outcome was defined as 3-month nonunion, as represented by disappeared…

BACKGROUND: Unstable distal third clavicle fractures involving coracoclavicular (CC) ligament disruption are prone to nonunion and often require surgical fixation. While hook plates (HPs) and superolateral locking plate (SLP) with CC suture augmentation are both commonly used, biomechanical comparisons are limited.
METHODS: Eighteen fresh-frozen adult cadaveric shoulders (9 contralateral matched pairs) were randomized to receive either HPs or SLP with CC suture fixation constructs after simulated Neer type IIb distal clavicle fractures. Constructs were cyclically loaded at 70 N for 500 cycles…

INTRODUCTION: Clavicle fractures are common among young people, generally as a consequence of car accidents, bike falls, and contact sports injuries. 15-20% of all clavicle fractures involve the lateral end of the bone. Thus, the distal clavicle fractures, in particular, have a high non-union rate ranging from 21 to 33% when treated non-operatively, underscoring the usual advice for operative treatment. While significant research has been conducted on clavicle fractures and their treatment options, no definitive guidelines or optimal approaches have been established. Hence, the aim of this stu…

BACKGROUND: Acromioclavicular joint arthritis is a common, painful, and often missed diagnosis, and it often accompanies other shoulder conditions such as rotator cuff disease. Whether distal clavicle resection is important to perform in patients undergoing surgery for rotator cuff tears and concomitant acromioclavicular joint arthritis is controversial.
QUESTIONS/PURPOSES: The purpose of this study was to perform a systematic review and meta-analysis of randomized controlled trials (RCTs) to evaluate the effect of distal clavicle resection on (1) outcome scores; (2) shoulder ROM, joint pain …

OBJECTIVE: Conservative treatment using a backpack bandage (RSV) for clavicle fractures in children and adolescents serves to restore anatomy and function. The technique used in adult patients with elastic stable intramedullary nailing (ESIN) from the medial end of the clavicle involves the risk of growth disturbance of the growth plate, which has been open for a very long time; in addition, a cosmetically disturbing scar usually forms there. Treatment with an intrafocal intramedullary nailless osteosynthesis allows length and axis to be restored within the age-specific correction limits using…

PURPOSE: The most common complication following treatment of a clavicle fracture is nonunion. Most nonunions are symptomatic and treatment is mostly operative. The aim of this study was to describe risk profiles of patients developing nonunion and what outcome is observed following operative treatment of clavicle nonunions.
METHODS: In a consecutive series of 729 clavicle fractures we retrospectively identified 55 that developed nonunion (49 diaphysial and six lateral). All were treated operatively by reconstruction with (n = 28) or without (n = 27) autologous bone graft. Assessment of functi…

BACKGROUND: This retrospective study investigates the complications, particularly subacromial osteolysis (SAO), associated with hook plate (HP) fixation, in the treatment of unstable distal clavicle fractures characterized by complete coracoclavicular (CC) ligament rupture. The decision-making process for employing HP in fractures of this nature, such as Neer types IIB and V and Cho classification IIC, involves considerations of distal fragment size and displacement. While HP offers advantages in clinical practice, it is not without complications, with SAO being a notable concern. Factors such…

A number of meta-analyses have compared clinical outcomes following plate vs. intramedullary fixation for midshaft clavicle fractures (MSCF), but with conflicting results. There is a requirement for updated level-1 evidence to guide clinicians managing MSCF. The aim of the present systematic review and meta-analysis was to compare clinical outcomes following plate vs. intramedullary fixation of MSCF. The PubMed, Scopus, BioMed Central, Cochrane Central Register of Controlled Trials and Google Scholar databases were searched for records added until 1st July 2019. A total of 10 randomised contro…

Background Clavicle fractures are common injuries in the adult population. The commonest site of fracture in the clavicle is the mid-shaft followed by the lateral end fracture. The anatomy and biomechanics of the lateral end clavicle make it prone to be unstable. Conservative management usually fails due to the deforming forces. Aim Our study evaluates pain relief, functional outcome, and the union rate in unstable lateral end clavicle fracture fixed by two different modalities of operative management, namely clavicular hook plate fixation and distal radius volar plate fixation. Materials and …

RATIONALE: Excessive distal clavicle resection can result in severe bone loss and disruption of the coracoclavicular (CC) ligament attachment, leading to significant vertical instability of the acromioclavicular joint. The optimal surgical approach in such cases remains controversial due to the technical challenges associated with achieving anatomical fixation.
PATIENT CONCERNS: A 40-year-old male construction worker presented with persistent shoulder pain, limited range of motion, and visible clavicular prominence 1 year and 7months after undergoing distal clavicle resection at an outside ho…
