Fractures of the trapezium occur infrequently, accounting for only 3.5 % of all carpal fractures. Trapezium fractures with concomitant Bennett's fracture-dislocations are exceedingly rare without a consensus on treatment recommendations. Here, we present a case of a simultaneous Bennett's fracture with a trapezium fracture in a pediatric patient. Our patient was treated with open reduction and internal fixation. Clinically, the patient was pain free with range of motion comparable to his uninjured side and without joint instability. A review of the literature reveals only twelve other cases re

We describe a unique C1-C2 lateral dislocation complicating a displaced type II odontoid fracture. We report a 63-year-old female pedestrian involved in a motor vehicle accident who required posterior open reduction and segmental C1-C2 instrumentation and fusion. Radiological examination of the cervical spine demonstrated a lateral dislocation of the atlantoaxial joint with a displaced type II fracture of the odontoid, fracture of the right lateral mass of C1 and left superior articular facet of C2. Neurological examination revealed the patient to be myelopathic and closed halo traction failed

Fourteen consecutive patients with acute displaced scaphoid waist fractures were treated with open reduction and internal fixation. The operative technique consisted of anatomic reduction of the displaced scaphoid waist fracture, correction of carpal instability, radial bone grafting for comminution, and internal fixation with K-wires or Herbert screw. The patients were evaluated an average of 26 months (range, 4-48 months) after surgery. Thirteen of the 14 (93%) fractures united. The average time to union was 11.5 weeks (range, 8-20 weeks). Fracture union was confirmed with trispiral tomograp

UNLABELLED: Ten patients with type III intercondylar eminence fractures with 6 years of follow-up underwent second-look arthroscopy after new injuries or complaints. Initial treatment was closed reduction (group I), arthroscopic reduction (group II), and arthroscopic reduction with internal fixation (group III). Lysholm ratings and one leg hop tests were normal in groups II and III and < 90% in group I. Tegner levels were 8, 9, and 10 in groups I, II, and III, respectively. International Knee Documentation Committee ratings were grades C, B, and A for groups I, II, and III, respectively. Arthr

This study investigated the use of endonasal infiltrative anesthesia for the management of pain associated with nasal bone fracture reduction. Fifty-two patients with nasal bone fractures were distributed in 2 groups. In the first group, topical endonasal anesthesia and external transcutaneous infiltrative anesthesia were employed. In the second group, endonasal infiltrative anesthesia was also added. Visual analog scale pain scores related to the different steps of the procedure were registered. The addition of endonasal infiltrative anesthesia was associated with a significant decrease (p <

The gold standard for fractures of the acetabulum is to perform an open reduction and internal fixation in order to achieve anatomical reduction. In a well-defined subset of patients, percutaneous techniques may be employed but achieving reduction by closed means can be challenging especially for fractures with large degrees of displacement. Such patient may include elderly patients who may not have the physiologic reserve to withstand open approaches. In our paper, we present a new option using laparoscopic assisted reduction of the acetabular fracture and percutaneous fixation. The young obe

There is still debate about the proper treatment of completely displaced metaphyseal fractures of the distal radius in children. Although historically these fractures were treated by closed reduction and casting, some authors have reported that up to 25% of these injuries re-displaced early in the cast despite good initial reduction. Since 2000 we have used closed reduction and percutaneous stabilisation with a short intramedullary wire, using a percutaneous technique, in children with completely displaced metaphyseal fractures of the distal radius. We retrospectively analysed the records of 1

In a series of 146 displaced ankle fractures, the effects of age, sex, side of injury, mechanism of injury, severity as determined by the Lauge-Hansen classification, type of injury (open or closed), open or closed treatment, and internal fixation of one or both malleoli were analyzed using subjective, objective, and radiographic parameters. Statistically significant prognostic features were identified and a prognostic scale was developed using multiple linear-regression analysis. The significant parameters were age, adequacy of the post-reduction positions of the medial and lateral malleoli,

Treatment of displaced radial neck fractures is challenging and controversial, as the risk of unsatisfactory outcome increases after operative as well non-operative treatment. Between 2004 and 2012, we treated 14 children with type IV fracture of radial neck with mean angulation of 72.8°, using the modified Metaizeau technique. The average follow-up was 39 months. Heterotrophic ossification and transient posterior interosseous nerve palsy were the only complication seen in two patients who had to undergo open reduction. We feel that the inability to achieve closed reduction can be attributed t

From 1972 to 1983 105 patients were admitted with a dislocated supracondylar fracture of the humerus. The average follow-up was five years. In general the functional results of treatment were good. The anatomical results were less satisfactory, and most times concerned varus deformities of the elbow, which did not influence the elbow function. Lesions of arteries and nerves only occurred in seriously dislocated fractures. The isolated nerve injuries were reversible. Two patients developed a Volkmann's ischemic contracture; their case history is discussed. In 24 cases a second reduction was per

OBJECTIVE: We describe here 10 cases of posterior shoulder dislocation associated with fracture of the humeral anatomic neck.
METHODS: Patients were treated according to our uniform treatment guidelines, in which only the dislocated humeral head (closed, if possible) was reduced, without any concomitant repositioning or internal fixation for fractures.
RESULTS: Reduction resulted in an acceptable repositioning of the fractured fragments in all but one case. Anatomic neck fractures were impacted by applying longitudinal pressure for stabilization. Although early physiotherapy was initiated, r

BACKGROUND: Supracondylar fractures of the humerus are the most common fracture pattern of the elbow in childhood. They require close follow-up subsequent to successful treatment, because of having a great risk of many complications. Many treatment methods have been described for the treatment of displaced supracondylar fractures of the humerus, however it has not been reached a consensus for the choice of the treatment. The purpose of this study is to investigate the late results of three different treatment methods; closed reduction+cast immobilization, skeletal traction+cast immobilization

The goal of reduction in displaced ankle fractures is the restoration of normal function in the involved extremity. This can be accomplished only by restoring the normal anatomy of the involved joints. Closed reduction is not always successful or indicated if vascular compromise, interspersed soft tissue, or osseous fragments are present. Open reduction with internal fixation allows for direct visualization of the involved parts and an accurate and rigid form of fixation. A case report of a displaced ankle fracture treated with open reduction and internal A-O fixation is presented.

BACKGROUND: Closed reduction of upper limb fractures and/or dislocations are common in the emergency department (ED), which involves physically re-aligning the injured part prior to immobilisation. As this is painful, numerous techniques are available to provide regional anaesthesia to ensure patient tolerance. A Bier block (BB) is typically performed as part of routine care. An alternative technique gaining interest in the ED is ultrasound-guided supraclavicular block (UGSCB), which involves injecting local anaesthetic around the brachial plexus at the supraclavicular fossa under ultrasound g

Maisonneuve fracture (MF) is defined as an ankle fracture-dislocation associated always with a fracture of the proximal quarter of the fibula and rupture of the anterior and interosseous tibiofibular ligaments. Other injuries are variable. Recent CT studies have demonstrated that MF is a far more complex injury than initially supposed. Therefore it is necessary to change substantially the current concepts related to this issue. MF is combined in about 80% of cases with a fracture of the posterior malleolus and also with malposition of the distal fibula in the fibular notch. An exact assessment

INTRODUCTION: Proximal humeral fracture-dislocations (PHF-D) are complex injuries, often requiring urgent intervention. However, management protocols remain unclear when anatomical reduction of the glenohumeral joint is achieved, but significant displacement of the greater tuberosity persists. The lack of consensus on whether to reclassify such injuries after reduction creates uncertainty in rehabilitation strategies.
PRESENTATION OF CASE: We present the case of a 53-year-old female who suffered an anterior shoulder dislocation with a displaced greater tuberosity fracture after falling due to

INTRODUCTION: The Essex-Lopresti injury, as defined in the literature, is characterized by a distinct triad: Radial head (RH) fracture, tearing of the interosseous membrane and disruption of the distal radioulnar joint (DRUJ). We present an atypical case of DRUJ disruption resulting from a displaced capitellum fracture rather than an RH fracture.
CASE REPORT: A 35-year-old female sustained a distal humerus capitellum fracture with associated DRUJ instability following a high-energy mechanical fall. Surgical management involved open reduction and internal fixation (ORIF) of the capitellum frac

Although complications following distal radial fractures are common, nonunion is rare. We report four cases of radial nonunion following distal radial fractures that occurred in elderly women after low-energy falls. The distinctive feature of these injuries was the presence of an associated distal ulnar shaft fracture. All patients had initial closed reduction and cast immobilization, which was unsuccessful. Reconstruction was complicated by deformity, disuse osteoporosis, radiocarpal stiffness, and distal radioulnar joint disruption. Wrist arthrodesis improved function and decreased pain and

Failure of manipulative reduction of acute anterior dislocation of the shoulder is extremely rare. A 55-year-old man dislocated his right shoulder when he fell heavily. Initial radiographs and computed tomographs demonstrated an anterior dislocation with fracture of the glenoid rim. Several attempts at closed reduction were unsuccessful. At the time of open reduction, the cause of failure was found to be interposition of a fragment of the anterior inferior glenoid rim in the joint. To prevent redislocation, the fragment was held in place by two Herbert mini bone screws after anatomic reduction

The infraorbital nerve (ION) is often involved in trauma to the zygomatic complex (ZC), resulting in sensory disturbance of the area innervated by it. The purpose of the study was to compare the incidence of persistent sensory disturbance after recovery from isolated simple fractures of the ZC, with four treatment methods: 1) closed reduction via subcutaneous approach without fixation; 2) open reduction via subcutaneous approach without fixation; 3) open reduction via oroantral approach and support of the complex by an intraantral Foley catheter; and 4) open reduction via Gillies approach with
