An anonymous web-based survey of alpaca owners was used to learn more about the clinical presentation, diagnosis, and treatment of coccidioi … tly affected. Alpacas appear to be highly susceptible to severe illness as a result of infection by Coccidioides spp., frequently resulting in death. More research is needed to better understand the epidemiology, clinical signs, and treatment protocols for coccidioidomycosis in alpacas.

The mycoses selected for presentation in this section are relatively common diseases of companion animals or livestock in certain areas of the world. Malasseziosis is arguably the most frequent mycosis of dogs (as otitis externa and dermatitis) throughout the world, although its diagnosis is often overlooked. Protothecosis is also geographically widespread, particularly in cattle where severe mastitis is a result of adventitious infection from the environment. In contrast, coccidioidomycosis and pythiosis are geographically limited in their occurrence (coccidioidomycosis by geographic region a

Counterimmunoelectrophoresis, was performed for serologic testing for coccidioidomycosis, employing coccidioidin and spherulin as antigens. Results obtained by counterimmunoelectrophoresis were compared with results obtained by complement-fixation and radial immunodiffusion. Of the 37 sera positive by complement-fixation, 28 were positive by counter-immunoelectrophoresis utilizing coccidioidin, whereas only 11 were positive by counterimmunoelectrophoresis utilizing spherulin (although an additional 14 were positive with counterimmunoelectrophoresis employing spherulin when the quantity of sphe

An agar dilution method was employed to test the susceptibility of 12 strains of Coccidioides immitis to polymyxin B (PB). After 3 days of incubation, eight strains were markedly inhibited by 5.0 mug of PB per ml and four strains did not grow. PB at 10 mug/ml inhibited the growth of all strains through 20 days of incubation. To determine whether PB has anticoccidioidal activity in vivo, mice in groups of 30 were infected intraperitoneally with a mean lethal dose (LD(50)) or 20 LD(50) of arthrospores of C. immitis ATCC 28868 (Silveira). Treatment by intraperitoneal injection of PB (2.5 mg/kg) w…

INTRODUCTION: Coccidioidomycosis infections can pose severe clinical complications with high morbidity and mortality. There exist numerous treatment options with emerging evidence accumulating for each.
AREAS COVERED: This review summarizes evidence regarding current and novel agents for the treatment of coccidioidomycosis. Key topics include a review of therapeutic agents, current guidelines, and evidence regarding selection of the most optimal treatment regimen.
EXPERT OPINION: There exist multiple antifungal regimens that have been demonstrated to be efficacious in treating systemic cocci

Coccidioidomycosis is a fungal infection that is often asymptomatic in immunocompetent individuals. When symptoms do occur, they often resemble a mild, flu-like illness. However, this disease can become clinically severe and disseminate, especially in immunocompromised patients or other high-risk groups. Failure to consider coccidioidomycosis in the differential diagnosis may lead to missed or delayed diagnosis, resulting in postponement of appropriate treatment.

A case of pulmonary coccidioidomycosis from the rural zone of Bertolinia, PI, is reported. The patient, a farm worker, attributed his illness to the dust inhaled while digging a water well during the dry season of the year, some weeks before the onset of the clinical manifestations. The main symptoms of the disease were severe chest pain and moderate fever. The diagnosis was made histopathologically: tissue phase fungal organisms--immature spherules and spherules with endospores--were observed in histological sections of a lung fragment obtained by open chest biopsy. This is the twelfth autoch

Disseminated peritoneal coccidioidomycosis in the setting of early pregnancy after fertility treatment is rare and can present as a diagnost … e abdominal wall and normal-appearing uterus and adnexa. The pathology results indicated peritoneal Coccidioides immitis infection. Hormonal changes associated with fertility treatment and immune tolerance in pregnancy may increase the risk for disseminated peritoneal coccidioidomycosis. A high index of suspicion and a multidisciplinary team are important for …

Coccidioidomycosis causes substantial morbidity and mortality in endemic areas, and dissemination is frequent in patients with impaired cellular immunity such as AIDS. Immune reconstitution inflammatory syndrome (IRIS) is paradoxical clinical worsening after initiation of antiretroviral therapy (ART) in a patient with HIV and a simultaneous opportunistic infection (OI). Immune reconstitution inflammatory syndrome has been well described for a host of mycobacterial, viral, and fungal OIs and malignancies such as Kaposi sarcoma. To date, only 3 cases of IRIS due to coccidioidomycosis have been r

Coccidioidal infection is a well-recognised cause of pulmonary disease in certain parts of the south-western USA, Central and South America; however, it is rarely encountered elsewhere in the world. We describe the case of a previously healthy man presenting to a Dublin hospital with fever, dry cough and chest pain, following a visit to the western USA. Despite treatment with broad-spectrum antimicrobials, the patient developed progressive bilateral pulmonary infiltrates and a large pleural effusion. After extensive investigations including CT, bronchoscopy and pleural fluid analysis, a diagno
