Efficacy of a specific model for cognitive-behavioral therapy among panic disorder patients with agoraphobia: a randomized clinical trial.

CONTEXT AND OBJECTIVE: Cognitive-behavioral therapy is frequently indicated for panic disorder. The aim here was to evaluate the efficacy of a model for cognitive-behavioral therapy for treating panic disorder with agoraphobia. DESIGN AND SETTING: Randomized clinical trial at Instituto de Psiquiatria da Universidade Federal do Rio de Janeiro. METHODS: A group of 50 patients with a diagnosis of panic disorder with agoraphobia was randomized into two groups to receive: a) cognitive-behavioral therapy with medication; or b) medication (tricyclic antidepressants or selective serotonin reuptake i

Impact of alexithymia on treatment outcome: a naturalistic study of short-term cognitive-behavioral group therapy for panic disorder.

BACKGROUND: It is often suggested in the literature that alexithymic patients are less responsive to psychotherapy than nonalexithymic patients. However, few empirical studies have examined this issue. Furthermore, it is unclear whether or not alexithymia itself may improve during psychotherapy. METHODS: Fifty-five consecutive outpatients with panic disorder received short-term cognitive-behavioral group therapy (CBGT) and were followed up 6 months later. Nineteen patients (35%) were on concomitant antidepressant medication. Alexithymia was measured using the 20-item Toronto Alexithymia Scale

Motivational interviewing versus cognitive behavioral group therapy in the treatment of problem and pathological gambling: a randomized controlled trial.

Pathological gambling is a widespread problem with major implications for society and the individual. There are effective treatments, but little is known about the relative effectiveness of different treatments. The aim of this study was to test the effectiveness of motivational interviewing, cognitive behavioral group therapy, and a no-treatment control (wait-list) in the treatment of pathological gambling. This was done in a randomized controlled trial at an outpatient dependency clinic at Karolinska Institute (Stockholm, Sweden). A total of 150 primarily self-recruited patients with current

Cognitive behavioral therapy for insomnia improves sleep and decreases pain in older adults with co-morbid insomnia and osteoarthritis.

STUDY OBJECTIVES: Osteoarthritis pain affects more than half of all older adults, many of whom experience co-morbid sleep disturbance. Pain initiates and exacerbates sleep disturbance, whereas disturbed sleep maintains and exacerbates pain, which implies that improving the sleep of patients with osteoarthritis may also reduce their pain. We examined this possibility in a secondary analysis of a previously published randomized controlled trial of cognitive behavioral therapy for insomnia (CBT-I) in patients with osteoarthritis and co-morbid insomnia. METHODS: Twenty-three patients (mean age 69…

Promoting walking as an adjunct intervention to group cognitive behavioral therapy for anxiety disorders--a pilot group randomized trial.

A group randomized trial of adding a home-based walking program to a standard group cognitive behavioral therapy (GCBT+EX) was compared with groups receiving GCBT and educational sessions (GCBT+ED). The study was implemented in an outpatient clinic providing GCBT for clients diagnosed with panic disorder, generalized anxiety disorder or social phobia. Pre- and post-treatment measures included the self-report depression, anxiety, and stress scale (DASS-21) and measures of physical activity. From January 2004 to May 2005, six groups were allocated to GCBT+EX (n=38) and five to GCBT+ED (n=36). An…

A cognitive behavioral therapy for alcohol-dependent domestic violence offenders: an integrated substance abuse-domestic violence treatment approach (SADV).

This pilot study evaluated the efficacy of a twelve-session cognitive behavioral group therapy for alcohol-dependent males with co-occurring interpersonal violence (IPV). Participants were 85 alcohol-dependent males who were arrested for domestic violence within the past year. Seventy-eight male adults were randomized to either a cognitive behavioral Substance Abuse Domestic Violence (SADV) group (N = 40) or a Twelve-Step Facilitation (TSF) Group (N = 38). There was no significant difference between SADV versus TSF in the number of sessions attended. Regarding substance use, the group assigned

Subjective distress, self-harm, and suicidal ideation or behavior throughout trauma-focused cognitive-behavioral therapy in transitional age youth.

OBJECTIVE: Elevations in distress, self-harm, and suicidal ideation or behavior are of significant concern in clinical practice. We examined these in a pilot trial of Trauma Focused-Cognitive Behavioral Therapy (TF-CBT) for transitional age youth (aged 15-25 years) with histories of interpersonal trauma and symptoms of posttraumatic stress disorder. METHOD: Participants were 20 young people (13 females, M = 19.5 years) from a pilot study of TF-CBT. Frequencies of elevated distress, self-harm, and suicidal ideation or behavior were measured throughout treatment sessions and across the treatmen…

The startle reflex in alcohol-dependent patients: changes after cognitive-behavioral therapy and predictive validity for drinking behavior. A pilot study.

BACKGROUND: Previous studies demonstrated an attenuation of the affect-modulated startle reflex when alcohol-dependent patients were viewing alcohol-associated pictures. This indicates an appetitive valence of these stimuli. We used the affect-modulated startle reflex to assess the effects of behavioral treatment on the emotional processing of alcohol-associated stimuli. Further, we examined whether the affect-modulated startle reflex is a predictor of treatment success. METHODS: Forty-three alcohol-dependent patients (21 females, mean age 45.67 years, SD 9.45) were recruited consecutively fr

A mixed-method process evaluation of guided online cognitive behavioral therapy for insomnia in patients with borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is often associated with insomnia, which may contribute to a vicious cycle of reciprocal exacerbation of sleep disturbance and BPD symptoms. Online cognitive behavioral therapy for insomnia (iCBT-I) has shown promise for treating insomnia in BPD patients. This study evaluated the key determinants, underlying processes, and change mechanisms of successful implementation of guided iCBT-I in BPD patients. METHODS: In the context of our clinical trial, we conducted this mixed-method process evaluation in 30 patients with BPD and comorbid insomnia …

Effectiveness of attexis, a digital intervention based on cognitive behavioral therapy for adults with ADHD: a randomized controlled trial.

Access to evidence-based psychosocial interventions for adults with attention-deficit/hyperactivity disorder (ADHD) remains limited, despite strong patient demand for nonpharmacological options such as cognitive behavioral therapy (CBT). Digital interventions may offer a scalable, low-threshold solution to meet this need and complement existing care. This pragmatic randomized controlled trial evaluated the effectiveness of attexis, a fully self-guided digital intervention based on CBT and mindfulness principles, as an adjunct to treatment as usual (TAU). A total of 337 adults with confirmed AD…

Helpful and hindering factors in group-based cognitive-behavioral therapy for adolescents with obsessive-compulsive disorder: a qualitative study.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a debilitating condition in which an individual experiences intense obsessions and extensive compulsions. The disorder is often associated with feelings of shame. Cognitive-behavioral therapy (CBT) including exposure and response prevention is the recommended first-line treatment for youth with OCD. A group-based CBT (GCBT) format offers potential benefits such as peer support and shared experiences. However, optimizing treatment outcomes requires the identification of key factors that clinicians must consider when offering GCBT. The aim of th

Does attachment change serve as a theory-specific mediator of depression reduction in interpersonal psychotherapy versus cognitive behavioral therapy?

OBJECTIVE: Although interpersonal psychotherapy (IPT) and cognitive behavioral therapy (CBT) for depression are comparably efficacious, less is known about the respective patient-level processes that help transmit their beneficial effect. Given its explicit and primary focus on attachment theory and relationship processes, it is plausible that IPT helps ameliorate depression specifically through decreasing patients' attachment insecurity. Testing this hypothesis, we examined whether IPT relative to CBT promoted greater increases in secure attachment during therapy, which in turn associated wit…

Efficacy of repetitive transcranial magnetic stimulation combined with cognitive behavioral therapy for depression: a systematic review and meta-analysis.

BACKGROUND: Repetitive transcranial magnetic stimulation (rTMS) and cognitive behavioral therapy (CBT) are established treatments for depression. However, the incremental benefit of combining these approaches remains uncertain, partly because prior reviews have pooled studies with clinically heterogeneous comparator conditions. This study systematically reviewed the efficacy of rTMS combined with CBT for depression and conducted a meta-analysis restricted to trials using rTMS-based control conditions. METHODS: We systematically searched Web of Science, PubMed, Embase, EBSCOhost, China Nationa…
