Minimally invasive techniques to decrease the impact of colorectal surgery for neoplasia.

Minimally invasive colorectal surgery has evolved over the past few decades. Techniques currently in practice to decrease the impact of colorectal surgery for neoplasia include laparoscopy and its derivations, robotic surgery, transanal endoscopic surgery (TES), transanal total mesorectal excision (taTME), natural orifice specimen extraction (NOSE), needlescopic surgery, and advanced endoscopy. Many of the new techniques are variations of standard laparoscopy that aim to decrease the size of the incisions (needlescopic surgery), decrease the number of incisions (single-incision laparoscopic su

Cancer genetics clinics and the surgeon: a valuable role for family history screening.

INTRODUCTION: This study examines how a colorectal surgeon can use a regional cancer genetics service to deal safely and efficiently, with community referrals for colorectal cancer screening on the basis of family history. PATIENTS AND METHODS: A retrospective review of consecutive asymptomatic people with a strong family of colorectal cancer referred by the surgeon to the genetics service over a 30-month period. RESULTS: A total of 45 people were referred by the surgeon to the cancer genetics service. Following official verification of family histories, 15 were thought to be in a low-risk c

Analysis of diverting ileostomy for colorectal cancer surgery: stenosis and T4 invasion are risk factors of reoperation due to anastomotic leakage, even when ileostomy is performed.

PURPOSE: In colorectal cancer (CRC), diverting ileostomy prevents the occurrence and severity of anastomotic leakage (AL) during surgery. However, an ileostomy cannot prevent reoperation due to severe AL in some cases, and an approach other than ileostomy may be required. This study identified the risk factors of AL and reoperation due to AL in patients with diverting ileostomies. METHODS: Patients diagnosed with CRC who underwent resection surgery accompanied by diverting ileostomy between January 2015 and December 2023 were included. We analyzed the risk factors for AL and reoperation due t

Prognostic Factors for Cancer-Specific Survival after Recurrence in Chemo-Naïve Stage I-III Colorectal Cancer.

BACKGROUND: Survival after recurrence in colorectal cancer (CRC) is typically evaluated in adjuvant-treated Stage II-III patients, where therapy may select for resistant subclones and biologically modify recurrent tumors. Conversely, the natural history of recurrence in Stage I or chemo-naïve disease remains poorly characterized. We investigated prognostic determinants of cancer-specific survival after recurrence (CS-SAR) in a strictly chemo-naïve cohort. METHODS: Among 721 patients with pathological Stage I-III CRC who underwent curative resection without perioperative chemotherapy, we ident

Complete pathological response of colorectal peritoneal metastases in Lynch syndrome after immunotherapy case report: is a paradigm shift in cytoreductive surgery needed?

BACKGROUND: We report the first case of a patient affected by peritoneal metastases from colon cancer, arising in the context of Lynch syndrome with pathological complete response. The patient was treated with immunotherapy and cytoreductive surgery. This paper discusses the implications of these novel therapies for the management of PM. CASE PRESENTATION: A 50-year-old man affected by Lynch syndrome was referred to our institution for metachronous peritoneal recurrence of ascending colon adenocarcinoma. As a second-line treatment, he received Nivolumab therapy with stable disease. Patient un

Muir-Torre phenotype has a frequency of DNA mismatch-repair-gene mutations similar to that in hereditary nonpolyposis colorectal cancer families defined by the Amsterdam criteria.

Muir-Torre syndrome (MTS) is an autosomal dominant disease defined by the coincidence of at least one sebaceous skin tumor and one internal malignancy. About half of MTS patients are affected by colorectal cancer. In a subgroup of MTS patients the disease has an underlying DNA mismatch-repair (MMR) defect and thus is allelic to hereditary nonpolyposis colorectal cancer (HNPCC). The purpose of this study was to examine to what extent germ-line mutations in DNA MMR genes are the underlying cause of the MTS phenotype. We ascertained 16 MTS patients with sebaceous skin tumors and colorectal cancer

Clinical profile, treatment patterns, and early outcomes of colorectal cancer at Kassala Police Hospital, Eastern Sudan: a 10-year retrospective cohort study.

In low-resource settings with limited access to multimodal care, colorectal cancer is characterized by delayed diagnosis and a high incidence of emergency presentation. Robust contemporary longitudinal data from Eastern Sudan are scarce. This is a retrospective cohort study recruiting all patients who underwent colonic or rectal resection for colorectal cancer between June 2013 and June 2023. Extracted data included demographic, clinical, operative, oncologic, and outcome variables. Fisher’s exact tests, χ² tests, t-tests and ANOVA were used for bivariate analysis. A Concise Review Evaluating the Safety and Effectiveness of Emergency General Surgery in Cancer Patients.

INTRODUCTION: Emergency general surgery (EGS) is frequently required in patients with cancer and is associated with high mortality. However, data describing outcomes in this population remains limited. This concise review aims to synthesize the available evidence on short-term mortality and postoperative complications following EGS in adult cancer patients. METHODS: This concise review searched five major databases, including ProQuest, Cochrane, Google Scholar, PubMed, and Embase, to identify studies published from January 2000 through December 2025 evaluating outcomes of EGS in adult cancer

Canonical and uncanonical pathogenic germline variants in colorectal cancer patients by next-generation sequencing in a European referral center.

BACKGROUND: Despite increasing use of next-generation sequencing (NGS), data concerning the gain in germline pathogenic variants (PVs) remain scanty, especially with respect to uncanonical ones. We aimed to verify the impact of different cancer predisposition genes (CPGs) on colorectal cancer (CRC) in patients referred for genetic evaluation. MATERIALS AND METHODS: We enrolled for NGS, by Illumina TruSight Cancer panel comprising 94 CPGs, 190 consecutive subjects referred for microsatellite instability (MSI) CRC, polyposis, and/or family history. RESULTS: Overall, 51 (26.8%) subjects carried

Clinical and Pathological Characterization of Lynch-Like Syndrome.

BACKGROUND & AIMS: Lynch syndrome is characterized by DNA mismatch repair (MMR) deficiency. Some patients with suspected Lynch syndrome have DNA MMR deficiencies but no detectable mutations in genes that encode MMR proteins-this is called Lynch-like syndrome (LLS). There is no consensus on management of patients with LLS. We collected data from a large series of patients with LLS to identify clinical and pathology features. METHODS: We collected data from a nationwide-registry of patients with colorectal cancer (CRC) in Spain. We identified patients whose colorectal tumors had loss of MSH2, M

Metachronous colorectal cancer following segmental or extended colectomy in Lynch syndrome: a systematic review and meta-analysis.

Around 5% of colorectal cancers are due to mutations within DNA mismatch repair genes, resulting in Lynch syndrome (LS). These mutations have a high penetrance with early onset of colorectal cancer at a mean age of 45 years. The mainstay of surgical management is either a segmental or extensive colectomy. Currently there is no unified agreement as to which management strategy is superior due to limited conclusive empirical evidence available. A systematic review and meta- analysis to evaluate the risk of metachronous colorectal cancer (MCC) and mortality in LS following segmental and extensive

Genetic predisposition to colorectal cancer: Implications for treatment and prevention.

Colorectal cancer (CRC) is the third most common cancer diagnosed in men and women and approximately 5% of cases are associated with identifiable germline mutations associated with hereditary cancer syndromes. Lifetime risks for CRC can approach 50%-80% for mutation carriers in the absence of endoscopic and/or surgical intervention, and early identification of at-risk individuals can guide clinical interventions for cancer prevention and treatment. Personal and family history and molecular phenotype of CRC tumors are used in determining which patients should be referred for clinical genetic ev

Is the phenotype mixed or mistaken? Hereditary nonpolyposis colorectal cancer and hyperplastic polyposis syndrome.

PURPOSE: Hereditary nonpolyposis colorectal cancer is a hereditary syndrome defined by personal and family history of colorectal and other cancers. Some patients with this condition have multiple serrated polyps, which are the hallmark of hyperplastic polyposis syndrome, a rare colorectal cancer syndrome characterized by multiple hyperplastic/serrated polyps and an increased risk of colorectal cancer. We hypothesized that this may represent a unique group of patients, and this study investigates a possible association between the two syndromes. METHODS: A hereditary colorectal cancer registry

Positive family history promotes participation in colorectal cancer screening.

BACKGROUND: Participation rates in colorectal cancer (CRC) screening are rather low. We evaluated the interest of first degree relatives (FDR) of CRC patients to participate in a colonoscopy screening and compared the findings to controls with a negative family history. METHODOLOGY: There were 235 CRC patients diagnosed in our centre in 1984-2001. These were mailed an invitation letter for a preventive examination for their FDR older than 40 years and a questionnaire about occurrence of malignancies in their family. Colonoscopy was performed in 52 FDR and sex/age matched controls. RESULTS: T

Microsatellite instability-a useful diagnostic tool to select patients at high risk for hereditary non-polyposis colorectal cancer: a study in different groups of patients with colorectal cancer.

BACKGROUND: Clinical diagnosis of hereditary non-polyposis colorectal cancer (HNPCC) is based on a typical family history. As molecular genetic testing is predominantly restricted to these families, gene carriers not meeting the clinical criteria may be missed. AIMS: To examine the value of microsatellite instability (MSI) as a tool to increase the likelihood for uncovering a mismatch repair germline mutation in patients with colorectal cancer and to identify a genotype-phenotype relation in families with verified mutations. METHODS: Systematic search for germline mutations (hMSH2 and hMLH1

Contrast-enhanced ultrasound improves accurate identification of appendiceal mucinous adenocarcinoma in an old patient: A case report.

BACKGROUND: Adenocarcinoma of appendiceal origin is far rarer than other colorectal carcinomas and its preoperative diagnosis is challenging. To our knowledge, utility of contrast-enhanced ultrasound (CEUS) to diagnose it is much less. METHOD: A 61-year-old man presented with abdominal pain in the right lower quadrant for 20 days. In order to fulfill an accurately preoperative diagnosis, he received laboratory and imaging tests such as carcinoembryonic antigen (CEA), computer tomography (CT), CEUS and endoscope. DIAGNOSIS AND INTERVENTION: He was initially suspected of suffering appendicitis

Off-Hour Admission and Mortality Risk for 28 Specific Diseases: A Systematic Review and Meta-Analysis of 251 Cohorts.

BACKGROUND: A considerable amount of studies have examined the relationship between off-hours (weekends and nights) admission and mortality risk for various diseases, but the results remain equivocal. METHODS AND RESULTS: Through a search of EMBASE, PUBMED, Web of Science, and Cochrane Database of Systematic Reviews, we identified cohort studies that evaluated the association between off-hour admission and mortality risk for disease. In a random effects meta-analysis of 140 identified articles (251 cohorts), off-hour admission was strongly associated with increased mortality for aortic aneury
