Predictors of Severe Morbidity After Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy for Patients With Colorectal Peritoneal Carcinomatosis.

BACKGROUND: Severe morbidity after cytoreductive surgery (CRS) followed by hyperthermic intraperitoneal chemotherapy (HIPEC) is, besides the obvious short-term consequences, associated with impaired long-term outcomes. The risk factors for severe morbidity in patients with peritoneal carcinomatosis (PC) of colorectal origin are poorly defined. This study aimed to identify risk factors for severe morbidity after CRS + HIPEC in patients with colorectal PC. METHODS: Patients with colorectal PC who underwent CRS + HIPEC between 2007 and 2015 were categorized and compared between those with and th

Comparison of laparoscopic versus open surgery in a three-stage operation for obstructive left-sided colorectal cancer.

BACKGROUND: Treatment for obstructive left-sided colorectal cancer (OLCC) typically consists of a three-staged procedure. During the first stage, the obstruction is managed with diversion colostomy. Traditionally in the second stage, we perform open resection for the primary tumor. In this study, we evaluated the feasibility of laparoscopic resection of OLCC with diversion colostomy in terms of operative results and short-term outcomes. METHODS: A total of 20 patients underwent laparoscopic resection for OLCC (study group), 48 patients underwent open resection for OLCC (control group 1), and

Transjugular intrahepatic portosystemic shunt placement increases feasibility of colorectal surgery in cirrhotic patients with severe portal hypertension.

BACKGROUND: Colorectal resection in cirrhotic patients is associated with high mortality and morbidity related to portal hypertension and liver insufficiency. METHODS: This retrospective study evaluated the clinical outcomes of cirrhotic patients who underwent transjugular intrahepatic porto-systemic shunt (TIPS) placement before colorectal resection for cancer. Main outcomes measures were postoperative morbidity and mortality rates. RESULTS: TIPS placement was successful in all eight patients and significantly decreased the mean hepatic venous pressure gradient from 15.5 ± 2.9 to 7.5 ± 1.9

Successful voiding after trial without catheter is not synonymous with recovery of bladder function after colorectal surgery.

PURPOSE: The need for monitoring postoperative urine output and the possibility of lower urinary tract dysfunction following colorectal surgery necessitates temporary urinary drainage. Current practice assumes recovery of lower urinary tract function to coincide with successful micturition after removal of urethral catheter. The aim of this study was to analyze the recovery of bladder function following colorectal surgery. METHODS: Patients undergoing colorectal operations underwent preoperative and postoperative uroflowmetry and residual urine estimation. All patients were catheterized supra…

Microsatellite instability as a marker in predicting metachronous multiple colorectal carcinomas after surgery: a cohort-like study.

PURPOSE: In case-control studies, it was reported that microsatellite instability might be helpful in predicting the development of metachronous multiple colorectal cancers. The purpose of this cohort-like study was to determine whether microsatellite instability is a novel independent marker in predicting metachronous colorectal carcinomas after colorectal cancer surgery. METHODS: Three hundred twenty-eight colorectal carcinoma patients were surveyed by periodic colonoscopy for at least three years after surgery. Among these, DNA from paraffin-embedded sections was available for 272 cases. D

Perioperative and oncological outcomes of extended surgery for colorectal cancer in younger patients: a propensity score-matched study.

BACKGROUND: Extended surgery, including multivisceral resection, synchronous metastasectomy, and concomitant procedures for coexisting diseases, is occasionally required to achieve oncological clearance in patients with colorectal cancer. However, the perioperative and oncological impact of extended surgery in younger patients remains unclear. This study aimed to evaluate short- and long-term outcomes of extended surgery in younger patients with colorectal cancer. METHODS: We conducted a single-center retrospective study of patients aged ≤ 65 years who underwent colorectal cancer surgery betw

Extraintestinal enterobiasis presenting as a cervical mass in an immunocompromised adult after colorectal cancer surgery: A case report.

Enterobius vermicularis is a common intestinal helminth, whereas extraintestinal disease is rare and may present as a tumor-like lesion. We report a 50-year-old man with common variable immunodeficiency (CVID) and prior right hemicolectomy with adjuvant chemotherapy for cecal adenocarcinoma (pT3N1bM0) who developed a slowly enlarging left-sided cervical mass during oncologic follow-up. The lesion was painless and showed no overlying skin changes. Neck ultrasonography demonstrated a hypoechoic, round mass over the sternothyroid muscle with an elongated hyperechoic compartment and no Doppler flo

Effect of Monofilament Sutures on Incisional Hernia After Laparoscopic Colorectal Surgery: A Focus on Patients With Umbilical Hernia.

BACKGROUND/AIM: Incisional hernia (IH) is a common complication of laparoscopic colorectal surgery and is associated with risk factors such as obesity, diabetes, and smoking. Umbilical hernia is recognized as a strong predictor of IH; however, the effect of suture type on the occurrence of IH remains unclear. We aimed to evaluate whether the use of monofilament sutures reduced the incidence of IH, particularly in patients with umbilical hernias. PATIENTS AND METHODS: This retrospective cohort study included 223 patients who underwent laparoscopic colorectal surgery between January 2013 and De…

Advances and Challenges in the Application of Novel Oral Anticoagulants for Venous Thromboembolism Prevention Following Colorectal Cancer Surgery.

BACKGROUND: Venous thromboembolism (VTE) is a common but severe complication following colorectal cancer (CRC) surgery. Traditional anticoagulants such as low molecular weight heparin (LMWH) and vitamin K antagonists face limitations in clinical due to requirements for frequent monitoring, subcutaneous administration, and poor patient adherence. Novel Oral Anticoagulants (NOACs), with advantages including oral administration, stable pharmacokinetics, and no requirement for routine monitoring, have emerged as potential alternatives for postoperative VTE prophylaxis. METHODS: This narrative rev

Cancer-Related Malnutrition and Oxidative Stress in Colorectal Cancer Surgery: A Narrative Review of Pathophysiology and Postoperative Outcomes.

Colorectal cancer (CRC) is a major global health concern with high morbidity and mortality, particularly complicated by postoperative complications. This narrative review explores the interplay between cancer-related malnutrition (CRM) and oxidative stress (OS) as key modifiable risk factors influencing surgical outcomes in CRC patients. Drawing on the recent literature, the article analyzes the multifactorial pathophysiology of CRM, including systemic inflammation, micronutrient deficiency, and metabolic derangements, and its role in weakening antioxidant defenses. Concurrently, oxidative str
