The distal arthrogryposis (DA) syndromes are a distinct group of disorders characterized by contractures of two or more different body areas. More than a decade ago, we revised the classification of DAs and distinguished several new syndromes. This revision has facilitated the identification of five genes (i.e., TNNI2, TNNT3, MYH3, MYH8, and TPM2) that encode components of the contractile apparatus of fast-twitch myofibers and cause DA syndromes. We now report on the phenotypic features of a novel DA disorder characterized primarily by plantar flexion contractures in a large five-generation Ut…

The development of contractures is a common complication after burn injuries. Axillary burns often result in limited abduction of the arm and present a major hindrance in rehabilitation. To prevent axillary contractures after burn injury, we perform a special grafting technique. In this study we treated 17 patients with 23 axillary burns using this technique. Patients were splinted early, and an intensive physiotherapy program was started 5 days after splinting. After 12 months, the mean abduction of the successfully treated axillary burns was 152 degrees. A secondary reconstruction was needed…

Contracture of the fibrous connective capsules that form around breast implants is the most frequent complication for silicone mammary prostheses. Capsule contraction is thought to be associated with the alteration in the mechanical properties of the tissue, but to date such a correlation has not been quantified. This study aimed to develop an experimental protocol to investigate the mechanical properties of the periprosthetic tissue and their dependence on the severity of capsular ontracture. Capsule samples were harvested from donors undergoing surgical correction or re-implantation of breas…

Primary peritoneal carcinoma, like ovarian cancer, usually causes non-specific abdominal symptoms, and often presents at a late stage. We report a case of primary peritoneal carcinoma where the development of abdominal symptoms was preceded for 6 months by paraneoplastic palmar contractures. This case demonstrates the importance of recognising palmar contractures as a potential sign of neoplastic disease.

The use of the anterolateral thigh fasciocutaneous flap in the reconstruction of soft-tissue defects around the knee among burn patients is described. The anterolateral thigh fasciocutaneous flap was used for eight patients (all male; mean age, 45 years; age range, 32 to 60 years). Flexion contracture was observed for seven patients with unhealed wounds and one patient with a healed burn wound. The anterolateral thigh flap was used as a free flap for six patients and as a distally based island flap for two patients. The flaps ranged from 8 to 17 cm in width and from 12 to 30 cm in length. Seve…

Immobilization with external fixation bridging the knee joint in extension is frequently used after sustaining a war injury to the region of the knee joint with femoral and tibial bone fractures. Immobilization of the knee with plaster splints is performed in the same position. This usually prolonged treatment results in extension contractures of the knee joint. From June 1991 until March 1994, 54 patients with extension contractures of the knee caused by war wounds were treated at the Department of Orthopedics, Zagreb University Hospital Center, in Zagreb, Croatia. Results of surgical treatme…

The condition in which ankle dorsiflexion is restricted is known as equinus contracture (EC). Equinus contracture is purported to be associated with a number of clinical conditions. However, there are no data to support or refute a clinician's ability to diagnose EC by clinical exam. We prospectively evaluated the maximum ankle dorsiflexion with the knee fully extended in 68 people (34 patients with isolated fore- or midfoot pain and 34 asymptomatic subjects) both by clinical exam and by a custom-designed ankle goniometer. We compared the likelihood of agreement of the clinical impression (equ…

Congenital contractural arachnodactyly (CCA) is an autosomal dominant connective tissue disorder, comprising marfanoid habitus, flexion contractures, severe kyphoscoliosis, abnormal pinnae, and muscular hypoplasia. It is now known that mutations in the gene encoding fibrillin-2 cause CCA. Interestingly, mutations described to date cluster in the fibrillin-2 region homologous to the so-called neonatal Marfan syndrome region of fibrillin-1. Thus, it has been hypothesized that the relative infrequency of CCA compared with the Marfan syndrome is due to the limited region of the gene targeted for m…

The term epidermolysis bullosa refers to a group of disorders characterized by skin blistering following minor trauma. The hands, because of constant use during normal daily activity, are especially exposed to blistering, with secondary scarring leading to pseudosyndactyly, adduction contracture of the thumb, and flexion or extension contracture of the fingers. The standard surgical approach for the correction of these deformities is based on the incisional release of pseudosyndactyly and contractures, and split skin grafting of the secondary wounds. A simplified approach without skin grafting…

We identified all patients treated by local fasciectomy at the Department of Hand Surgery, Uppsala University Hospital between 1965 and 1996. A total of 2375 operations were performed on 1600 patients. We found a male:female ratio of 5.9:1. Women had a higher mean age at first operation (62.4 years) than men (59.8 years). One-third of the men required repeated surgery and one-quarter of the women. Early age at first operation was associated with recurrent disease.

A 34-year-old man with multiplex congenita contractures underwent replacement of 2 hips and 1 knee. Even though a good range of movement was achieved at surgery and intensive physiotherapy, his joints returned to their preoperative status within 2 years. This outcome suggests that total joint replacement has little to offer the patient with multiplex congenita contractures who has immobile joints.

This study reports the results of 98 operations for correcting knee-flexion-contractures, which were performed between 1972 and 1989 in 60 patients with myelomeningocele. 13 knees had hamstring lengthening and 85 knees had radical flexor release. In 4 patients, who had flexion-contractures of more than 50 degrees, the soft tissue release was combined with supracondylar extension osteotomy. 58 patients had additional hip-flexion-contractures and 39 patients had feet deformities. The average age at time of surgery was 8 years, 3 months. The average follow-up-period was 65 months with a minimum o…

In the recent 3 years operations were carried out in 8 patients with postburn extension contractures of the knee joints, which accounted for 12.9% of all cicatricial contractures of this joint. The anatomical essence of the contracture was ascertained. A method of surgical treatment was developed, which consisted in restoration of the cutaneoadipose layer in the region of the knee with a cutaneofascial graft taken from the contralateral leg and subsequent excision of the scars on the thigh, by freeing of the muscles extending the joint, and restoration of the skin surface with a split nonperfo…

Two hundred and seventy-eight surgically treated postburn metacarpophalangeal joint extension contractures in children were reviewed. A classification system based on the limitation of passive metacarpophalangeal flexion was devised to direct surgical intervention and assess postoperative results. Type I (47%) digits demonstrated greater than 30 degrees of metacarpophalangeal flexion with the wrist fully extended, and scarring was generally limited to the dorsal skin. Type II (34%) digits demonstrated less than 30 degrees of metacarpophalangeal flexion with the wrist maximally extended, and sc…

The author reports a forty year old man presenting with Dupuytren's contracture affecting both hands, feet and auricular conchae. The patient is epileptic having been treated for twenty eight years with phenobarbitone and phenytoin. He had histologically proven Dupuytren's disease on both hands, but unfortunately no material was available from the ears. The follow up to date suggests that the auricular lesions have progressed slowly.

The results of antiinflammatory radiation therapy were analyzed in 17 patients with trigeminal neuralgia and 7 patients with post-neuritic++ contractures of mimetic muscles. The therapy yielded positive results and was adequately tolerated by all patients. In the follow-up, no changes of the patients' general status or hematological indices were detected.

An important element in the care of the burn victim is the prevention and treatment of burn wound contractures. Since limited objective quantitative information is available on the incidence of contractures after thermal injury, or on the factors that predispose individuals to their development, a review of all patients seen from July 1980 through January 1986 for surgical correction of burn wound contractures was conducted. Among the 53 patients selected for study, the incidence of contractures was higher in the pediatric patients, 7.8%, than in the adult patients, 2.0% (P less than 0.001), a…
