Possible role for cryoballoon ablation of right atrial appendage tachycardia when conventional ablation fails.

Focal atrial tachycardia arising from the right atrial appendage usually responds well to radiofrequency ablation; however, successful ablation in this anatomic region can be challenging. Surgical excision of the right atrial appendage has sometimes been necessary to eliminate the tachycardia and prevent or reverse the resultant cardiomyopathy. We report the case of a 48-year-old man who had right atrial appendage tachycardia resistant to multiple attempts at ablation with use of conventional radiofrequency energy guided by means of a 3-dimensional mapping system. The condition led to cardiomy

Meta-analysis of the efficacy of cold coagulation as a treatment method for cervical intraepithelial neoplasia: a systematic review.

BACKGROUND: Cold coagulation is an ablative method for treatment of cervical intraepithelial neoplasia (CIN). Despite reports of efficacy against all grades of CIN (CIN1-3), cold coagulation has been infrequently used since the 1980s, and was absent from the recent Cochrane review on CIN treatment. OBJECTIVES: To provide a systematic review of cold coagulation efficacy and acceptability for CIN treatment through meta-analysis of clinical reports and a randomised control trial. SEARCH STRATEGY: A literature search in PubMed, Web of Science, EMBASE, and regional databases yielded 388 papers. T

Successful cryoablation of Mahaim tachycardia in a child with Ebstein's anomaly.

Mahaim fibers with decremental atrioventricular (AV) node-like conduction properties comprise less than 3 % of accessory pathways. Radiofrequency ablation of right atriofascicular pathways guided by a distinct Mahaim potential detected at the anterolateral to posterolateral tricuspid annulus or in the right ventricular free wall is a safe and highly effective treatment method. The case report presents a 16-year-old boy with Ebstein's anomaly and symptomatic wide complex tachyarrhythmia. The electrophysiologic study and the entire ablation procedure were performed using a three-dimensional mapp

Cryorecanalization of an obstructed bronchial stent in a 12-year-old boy.

We report a 12-year-old boy who underwent stent implantation into the left main bronchus at the age of 9.5 years for palliation of severe bronchial stenosis. He had developed complete obstruction of the stent by granulation tissue resulting in respiratory deterioration and the requirement of mechanical ventilation. The stent obstruction was treated at the age of 11.5 years by cryorecanalization. In this technique, cooling of the tip of the cryoprobe is used to induce adherence of the obstructing tissue allowing the subsequent removal of tissue particles by retraction of the probe. Under genera

Cryoballoon ablation: a novel technique for treating focal atrial tachycardias from the pulmonary veins.

AIMS: Cryothermic ablation using a cryoballoon is a novel technique which has been used to treat paroxysmal atrial fibrillation. In this study, we wanted to test this technique to treat focal atrial tachycardias (ATs) from the pulmonary veins (PV). METHODS AND RESULTS: Five patients (four women, one man, mean age 43 +/- 16 years) with severe symptoms due to focal AT originating from a PV were studied. A single transseptal puncture was done. After confirmation of the diagnosis by conventional mapping, a 23 or 28 mm cryoballoon catheter was positioned in the PV of interest. Freezing was done fo

Catheter cryoablation of cardiac arrhythmias.

PURPOSE OF REVIEW: The purpose of the review is to provide an update on the safety and efficacy of catheter cryoablation. RECENT FINDINGS: Catheter cryoablation is a safe and clinically effective method for ablation of atrioventricular nodal reentrant supraventricular tachycardia. Although the acute procedural success rate of catheter cryoablation for this arrhythmia may be slightly lower than that reported for radiofrequency ablation, it has an excellent safety profile, with no reported instances of inadvertent atrioventricular block requiring implantation of a permanent pacemaker. Using thi

Hepatic cryotherapy involving the vena cava. Experimental study in a pig liver model.

The neurogenic thoracic outlet syndrome includes the symptoms caused by the compression of the brachial plexus at some point between the interscalene triangle and the inferior border of the axilla. Five patients diagnosed as having neurogenic thoracic outlet syndrome were examined. We determinated clinical and electrophysiological features which lead to thoracic outlet syndrome diagnosis. The clinical features included paresthesia and pain along the inner part of the arm, forearm and hand. Diminished sensation was rare. Muscle weakness was usual and muscle atrophy was associated in severe case

Irving S. Cooper (1922-1985): a pioneer in functional neurosurgery.

Irving S. Cooper (1922-1985), the son of a salesman, worked his way through high school, college, and medical school to become one of the pioneers in functional neurosurgery. He developed several novel techniques for the surgical management of Parkinson's disease and other crippling movement disorders. A keen interest in the physiology of movement disorders was kindled by his doctoral research and continued during his neurosurgical training. He began to apply this knowledge to surgical practice in 1952 when he began his faculty career as Assistant Professor of Surgery at New York University. A

Cervical dysplasia treatment: key issues for developing countries.

Many developing countries face serious obstacles that have hindered establishment of successful cervical cancer control programs. Various countries are now seeking to strengthen cytology services and identify simple low-cost screening strategies; but any real gains in reducing cervical cancer incidence and mortality will also require effective treatment of women with preinvasive disease. Despite a trend toward conservative outpatient approaches for treating cervical dysplasia in industrialized countries, clinicians in many developing countries still rely primarily on invasive inpatient methods

Retinopathy of prematurity--current diagnosis and management.

Despite advances in ophthalmological care of premature infants, retinopathy of prematurity (ROP) remains a still unsolved problem for paediatricians as well as ophthalmologists. A survey of the current literature concerning drug therapy and surgical management as related to the different stages of ROP is given. The classification system for ROP according to the International Committee is presented as well as our screening policy in relation to the literature. The effectiveness in preventing severe cases of ROP and the toxicity of vitamin E supplementation in high-risk premature infants is stil

Cyclocryotherapy as an alternative treatment for primary glaucoma.

Cyclocryotherapy was used as an alternative treatment for primary glaucoma in a series of 56 patients, both with narrow and open angle. In this prospective study treatment was successful in 87.7% of the cases (Peak I.O.P. < or = 20 mm Hg) with a mean follow up period of 10.1 months. Mean intraocular tension decreased from 29.7 mm Hg preoperatively down to 17.25 mm Hg postoperatively. About 60% of the eyes, however, needed an additional local treatment. Postoperative complications were rare and mild. A conjunctival congestion, and a mild self limiting iridocyclitis, were the most frequent ones.

Retinopathy of prematurity: approaches to surgical therapy.

Retinopathy of prematurity (ROP) is a vasoproliferative disease with infiltration of the vitreous gel. Contraction of the infiltrate results in traction detachment with typical peripheral trough formation. Cryotherapy inhibits proliferation in the active stages of the disease (Stage 3, 'plus'). With development of traction detachment involving the macula (Stage 4B) encircling buckling procedures should first be tried. Closed vitrectomy is considered when high retinal detachment and opacification by vitreous strands occur. The surgical technique is described. Anatomical (64%) and functional (43

Retinal detachment. A study of a population-based patient material in Sweden 1971-1981. III. Surgical results.

Surgical results and complications were studied retrospectively in 590 cases of retinal detachment in a population-based material of 538 patients. Of the 590 cases 96.3% were operated upon, and 22.9% of these underwent one or more re-operations. The technique used was cryo- or photocoagulation (100%) + scleral buckling or an encircling band (96%) + drainage of subretinal fluid (80%). The surgical complications were intraocular bleeding (13%), choroidal detachment (7%), and proliferative vitreo-retinopathy (5%). All complications were significantly more frequent after re-operations. Complete re

Suprachoroidal implantation for the treatment of retinal detachment.

A temporary apposition of retina and choroid has been created during retinal detachment surgery by injection of sodium hyaluronate into the suprachoroidal space in 14 patients. The iatrogenic choroidal detachment lasts at least seven to ten days, allowing adequate time for retinopexy to permanently seal the retinal hole. Suprachoroidal implantation is compatible with and additive to conventional retinal detachment techniques, or it can be used alone as a primary treatment modality. Further clinical evaluation and development are necessary to establish indications and limitations for this techn

[Prevention, in the contralateral eye, of a giant tear ablation].

In this paper the authors report on 11 patients with giant tear detachment, whose fellow eyes were prophylactically treated with cryopexy or laser coagulation. The patients were followed up for between 4 and 12 years. The prophylactic lesions were located radially or circumferentially (circumscribed or extending through 180 degrees). In 4 out of 11 patients new tears developed during the follow-up period. The tears occurred between 1 month and 7 1/2 years after prophylactic treatment. In one of these cases a giant tear (90 degrees) developed in spite of prophylaxis: this was a case in which th
