Malignant cerebral edema is a complication of large volume ischemic stroke secondary to middle cerebral artery occlusion. Left untreated, it can lead to death in up to 80% of cases. While medical options for management are limited, surgical decompression can play a key role in reducing mortality and, in some cases, improving functional outcomes.

Aggressive resection in Metastatic spinal tumors (MST) includes three main en bloc techniques: vertebrectomy, hemivertebrectomy, and corpectomy. En bloc excision aims to remove the tumor in one piece without violation, ensuring appropriate pathological margins. Vertebrectomy (marginal or wide en bloc excision) is indicated when the tumor is centrally located with at least one uninvolved pedicle. Thoracic lesions are often managed in a single stage, whereas lumbar lesions usually require a two-stage approach to reduce neurological risk. Posterior mobilization precedes anterior tumor delivery, w

Central cord syndrome (CCS) is the most common incomplete traumatic spinal cord injury; however, it remains poorly defined, with no consensus diagnostic criteria, an ambiguous and variable clinical presentation, and significant overlap with other incomplete spinal cord injuries. The available evidence indicates that CCS is best understood, not as a discrete anatomic lesion, but as a clinical phenotype arising from distinct pathophysiologic mechanisms. Management decisions should be driven by structural stability, injury severity, and underlying cord substrate. Patients presenting with structur

Ulnar nerve neuropathy is the second most common neuropathy of the upper extremity. The condition can easily be recognized clinically. The diagnosis can further be supported by either ultrasound or neurophysiological examination. Most cases can be treated conservatively. Surgery is recommended if conservative treatment fails or in severe clinical cases. Decompression is the primary procedure performed with a success rate of up to 87%. In some cases, a transposition of the ulnar nerve is necessary. We recommend early referral and consideration of surgery when conservative treatment fails, as ar

Trepanation is widely recognized as the earliest known cranial operation, yet its functional purpose remains debated. Traditional interpretations have emphasized ritual practice, treatment of cranial trauma, or empirical responses to neurological symptoms. We propose a focused historical-neurosurgical interpretation: that a meaningful subset of prehistoric trepanations may represent a plausible early empirical form of cranial decompression. The strongest quantitative evidence comes from the Cusco-region Peruvian series, in which 66 trepanned individuals accounted for 109 perforations and 26 in

INTRODUCTION: Tension pneumoperitoneum (TPP) is a rare but life-threatening condition characterized by free intraperitoneal air causing intra-abdominal hypertension and hemodynamic compromise. It shares physiologic features with abdominal compartment syndrome and requires timely recognition and intervention. This systematic clinical review evaluates the presentation, diagnostic strategies, management approaches, and outcomes associated with TPP.
METHODS: A comprehensive literature search was conducted across PubMed, EMBASE, and Google Scholar for studies published between 2004 and 2024. Searc

Spinal Cord Injury (SCI) is a condition leading to inflammation, edema, and dysfunction of the spinal cord, most commonly due to trauma, tumor, infection, or vascular disturbance. Symptoms include sensory and motor loss starting at the level of injury; the extent of damage depends on injury severity as detailed in the ASIA score. In the acute setting, maintaining mean arterial pressure (MAP) higher than 85 mmHg for up to 7 days following injury is preferred; although caution must be exercised when using vasopressors such as phenylephrine due to serious side effects such as pulmonary edema and

PURPOSE: Neonatal limb compartment syndrome (NLCS) is a rare and potentially limb-threatening condition defined as an increased pressure within a confined limb compartment. Clinicians may fail to distinguish NLCS from other mimicking conditions. Misdiagnosis is possible due to a low index of suspicion for this condition, which may delay appropriate and urgent treatment. A comprehensive review of the available literature was performed to explore common themes in NLCS and identify gaps in the evidence to guide future studies.
METHODS: A literature search was conducted in PubMed, Scopus, and Goo

OBJECTIVE: The idea to treat migraine patients with a surgical procedure is relatively recent. The aim of this paper was to describe the surgical techniques and our 11-year experience in migraine surgery.
PATIENTS AND METHODS: From June 2011 till December 2021, we have performed MH decompression surgery over 527 patients with either frontal (90 pts), occipital (232 pts) or temporal (205 pts) migraine trigger sites.
RESULTS: The surgical procedure elicited a positive response in: (a) occipital trigger site, 95% of patients (86% complete recovery); (b) frontal trigger site, 87% of patients (32

OBJECTIVE: To review the definition and possible etiologies for C 5 palsy.
METHODS: The literature on C 5 palsy at home and abroad in recent years was extensively reviewed, and the possible etiologies were analyzed based on clinical practice experience.
RESULTS: There are two main theories (nerve root tether and spinal cord injury) accounting for the occurrence of C 5 palsy, but both have certain limitations. The former can not explain the occurrence of C 5 palsy after anterior cervical spine surgery, and the latter can not explain that the clinical symptoms of C 5 palsy is often the motor d

INTRODUCTION: Migraine headache is a widespread neurovascular disorder with an enormous social and economic impact. A subgroup of patients cannot be managed with pharmacological therapy. Although surgical decompression of extracranial sensory nerves has been proposed as a valid alternative treatment option, the medical community remains reluctant to accept it.
MATERIALS AND METHODS: This systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. An electronic search was performed in September 2020 on PubMed, ScienceDirect, CENT

Nerve root compression by foraminal pathology is challenging for a surgeon to decompress without violating the facet joint, which may necessitate a fusion procedure. One nonfusion approach to foraminal pathology is a combination intracanal approach for a laminotomy/foraminotomy followed by a paraspinal Wiltse approach for far lateral decompression. Unfortunately, even with the combination approach, it continues to be difficult to achieve adequate decompression without violating much of the facet joint overlying the nerve root. Spine endoscopy offers the ability to decompress the foraminal port

Lumbar spinal stenosis is typically a degenerative condition that leads to compression of the spinal canal and lateral recess, resulting in leg pain and walking disability. Surgical management is indicated after failure of non-surgical management or rapidly worsening neurological impairment. The traditional approach is a laminectomy with foraminotomy and partial facetectomy but a newer minimally invasive option, unilateral laminectomy for bilateral decompression (ULBD), seems to demonstrate the better postoperative outcomes due to its unilateral exposure. ULBD involves a midline incision, open

The purpose of this note is to describe an obtuse-angled laminotomy of C7 during cervical decompression that aims to preserve cervicothoracic junction stability and potentially reduce pain. Cervical spondylotic myelopathy can result from degenerative cervical spinal disease including, herniated disk material, osteophytes, redundant ligamentum flavum, or ossification of the posterior longitudinal ligament. Surgical intervention for multilevel myelopathy aims to decompress the spinal cord and maintain stability of the cervical spine. Multilevel laminectomy is traditionally used when degenerative
