Emergency surgery for diverticular disease complicated by generalized and faecal peritonitis: a review.

There is little uniformity in either the indications for operation, the classification of the pathology or the operative management of generalized or faecal peritonitis secondary to perforated diverticular disease. Nevertheless, this review has shown a clear advantage both in terms of immediate mortality and morbidity for primary resection over conservative operations in which the colon is retained in the abdomen. We propose that, when a clinical diagnosis of localized sepsis secondary to diverticular disease is made, the management should be nonoperative with systemic antibiotics and supporti

Resection of the perforated segment. A significant advance in treatment of diverticulitis with free perforation or abscess.

As a result of improved medical management of chronic diverticular disease, perforation has become the most common indication for surgical intervention. During the past five years sixty-three patients underwent operation for colonic diverticular disease, of which forty-six were for perforation (generalized peritonitis in 8, abscess in 30, and fistula in 8). The eight patients with generalized peritonitis underwent emergency exploration for spreading peritoneal signs and were managed by resection of the perforated segment, end colostomy, and mucous fistula or Hartmann's pouch. Treatment of thir

Nontraumatic large bowel perforation: spectrum of etiologies and CT findings.

Large bowel perforation is an abdominal emergency that results from a wide range of etiologies. Computed tomography is the most reliable modality in detecting the site of large bowel perforation. The diagnosis is made by identifying direct CT findings such as extraluminal gas or contrast and discontinuity along the bowel wall. Indirect CT findings can help support the diagnosis, and include bowel wall thickening, pericolic fat stranding, abnormal bowel wall enhancement, abscess, and a feculent collection adjacent to the bowel. Common etiologies that cause large bowel perforation are colon canc

Acute Epiploic Appendagitis: An Overlooked Cause of Acute Abdominal Pain.

Acute epiploic appendagitis is a benign condition caused by inflammation of the epiploic appendages that are serosal lined outpouchings of the colon lying adjacent to the tenia coli. This rare condition has non-specific clinical findings and is frequently misdiagnosed as either acute diverticulitis or acute appendicitis. However, unlike other surgical causes of acute abdomen, epiploic appendagitis is a self-limited condition and resolves with conservative management. CT of the abdomen plays a vital role in diagnosing this condition and excluding other causes of acute abdomen. This case report

Moskowitz's Arterial and Venous Variants (Mo Type 2) in Sigmoid Resection for Diverticular Disease: A Case Report and Literature Review.

Diverticular disease of the sigmoid colon is common in Western countries, and acute diverticulitis may lead to complications requiring surgical intervention. Vascular anatomical variants, such as Moskowitz's artery and inferior mesenteric vein variations, can significantly influence surgical planning and outcomes. We report the case of a 57-year-old male with acute sigmoid diverticulitis complicated by a pericolic abscess. CT imaging revealed the presence of Moskowitz's artery and a venous variant of the inferior mesenteric vein draining directly into the portal vein. Due to clinical deteriora

Acute diverticulitis: safety and value of contrast studies in predicting need for operation.

Results of treatment of 70 patients hospitalized with acute diverticulitis were analysed to determine the value of early contrast enema in management. A water-soluble enema done during the first week of hospitalization in 48 patients showed diverticulosis with spasm (30), a leak or peridiverticulitis (16) and a normal study (two). No complications were caused by the study. Forty patients improved on medical therapy, but four relapsed after discharge. Thirty-four (49%) patients had urgent operation during hospitalization for the acute episode of diverticulitis. Findings on contrast enema correl

Transvaginal ultrasound diagnosis of colouterine fistula secondary to sigmoid colon cancer: A case report.

Colouterine fistula is an exceptionally rare pelvic fistula, typically arising from diverticulitis, malignancy, or trauma.‌ The thick uterine myometrium serves as a natural barrier, rendering direct intestinal invasion of the uterine wall unlikely and thus complicating preoperative diagnosis. ‌Dynamic real-time imaging plays a pivotal role in evaluating such lesions. This case report describes a 74-year-old woman patient who presented with a 1-month history of vaginal flatus and fecaloid discharge. The symptoms were caused by a colouterine fistula secondary to a sigmoid colon malignancy. Gynec

Infection of the sigmoid colon during TNFα antagonist therapy for chronic inflammatory joint disease.

We report 7 cases of sigmoid colon infection in patients taking TNFα antagonist therapy to treat chronic inflammatory joint disease. There were 5 women and 2 men with a mean age of 57.5 years (range, 21-77 years). The presenting symptoms were abdominal pain, bowel habit changes, and a fever. These symptoms developed within 6 months after starting TNFα antagonist therapy in 5 of the 7 patients. Empirical antibiotic therapy was used in all 7 patients. Surgical colectomy was performed in 4 patients, including 1 who required a temporary Hartmann's procedure. The risk of infection associated with T

Diverticular disease of right colon. Clinical variants and personal experience.

Diverticular disease of the right colon is not common, especially in western countries. It occurs in two different clinical forms, known as the "usual" and "hidden" variants. The diagnosis is not always easy, especially in the latter variant, because sometimes barium enema and CT scan are unable to distinguish this form from cancer. The final diagnosis is only intraoperative. In our experience, from 1994 to 2004, we observed 4 cases of complicated right-sided diverticulitis. Three of these patients had symptoms mimicking acute appendicitis, such as fever and abdominal pain. Only 2 of them unde

Iatrogenic spleen injury risk during robotic left colonic and rectal resections by routine left flexure mobilization technique: a retrospective study.

BACKGROUND: The routine mobilization of the left colonic flexure as a standard procedure during left colonic/rectal resection is a controversial topic in open and minimally invasive surgery. According to some authors, this maneuver may increase the risk of iatrogenic spleen damage; for others this does not change the odds. Ligaments over-traction is the most frequent injury mechanism. Some documented risk factors are reported: laparotomic approach, male gender, vascular disease, cancer, diverticulitis, surgery performed in emergency-setting. The type of procedure influences the associated risk

[Intestinal infarct caused by giant cell arteritis].

Arteritis of giant cells compromising extracranial and particularly intestinal tissues is not frequent. Therefore, it is common practice to make the diagnosis retrospectively after analyzing the surgical sample. A case is presented of an 83 year old woman admitted to the Clinical Department with a clinical course of 3 days of evolution characterized by fever and pain in the left hemiabdomen. Her personal medical history included multiple diverticulosis of colon, collecistectomy and appendicectomy. Laboratory tests showed that uremia was 0.75 g/L (N.L to 0.45 g/L), V.E.S. 90 mm at the first hou

A case report of coexistence of acute appendicitis and Meckel's diverticulitis causing generalized acute peritonitis.

INTRODUCTION: The coexistence of appendicitis and Meckel's diverticulitis is a rare condition that can lead to acute peritonitis. It is difficult to diagnose preoperatively and requires emergency surgical intervention. We report a case presenting with both lesions. CASE PRESENTATION: A 43-year-old female patient was admitted with generalized peritoneal irritation syndrome, developing in a febrile context and associated with a biological inflammatory syndrome. Computed tomography revealed signs of peritonitis and an inflamed appendix. Laparoscopy, converted to laparotomy, revealed coexisting a

Defect in colonic smooth muscle contraction in patients with ulcerative colitis.

Patients with ulcerative colitis have decreased postprandial colonic contractions. The purpose of this study was to determine whether the smooth muscle from patients with ulcerative colitis responds abnormally in vitro to different stimuli. Circular colonic smooth muscle strips from patients with ulcerative colitis, acute diverticular disease, or adenocarcinoma were stretched to the optimal length and stimulated with electrical field stimulation (EFS), bethanechol, or increased concentrations of extracellular K+. The EFS-stimulated on-contraction was similar in each group, but the off-contract

Changing concepts in diverticular disease.

Conventionally, acquired diverticular disease of the colon has been regarded as a single entity, so far as complications go. Experience at St. Vincent's Hospital, Melbourne, suggests that there are two kinds of diverticular disease, one with the classic muscle abnormality, chiefly confined to the left colon and characterized by inflammatory and perforative complications and the other without muscle abnormality, but with diverticula throughout the colon, in which bleeding is common, perhaps due to a connective-tissue abnormality which, on the one hand, allows development of diverticula in the a
