A Novel Pancreaticojejunostomy Training Model Utilizing Deceased Donor Pancreas.

INTRODUCTION: Pancreaticoduodenectomy (PD) is a complex operation with high postoperative morbidity, largely driven by pancreaticojejunostomy (PJ) leak. In 2021, US general surgery residency graduates performed only 9.5 ± 7 pancreatic operations, suggesting a need for additional experience with complex pancreatic anastomosis. We hypothesized that human deceased donor pancreas and jejunum could be utilized to create a high-fidelity PJ training experience. METHODS: Faculty pancreas surgeons were surveyed to determine factors influencing resident participation in PJ and expected impact of a proc

Internal Medicine Residency Point-of-Care Ultrasound (POCUS) Consensus Recommendations for Core Indications and Applications.

BACKGROUND: Bedside point-of-care ultrasound (POCUS) is quickly becoming a routine part of internal medicine practice and training. The optimal POCUS training venue for internists is at the graduate medical education or residency stage of their career. Despite increased training, clinical use, and broad internal medicine society endorsement in the United States, no internal medicine POCUS consensus curricula exist. The goal of this consensus process was to guide the core elements for inclusion within internal medicine residency POCUS curricula in the United States. METHODS: A 4-step modified

A Survey of Program Directors on Procedural Competence and Volume in the US Pulmonary and Critical Care Fellowships.

BACKGROUND: In the United States, Pulmonary and Critical Care Medicine (PCCM) fellowship training traditionally requires performing a minimum number of bronchoscopy and pleural procedures to be deemed competent. However, expert panel recommendations favor assessments based on skill and knowledge. PCCM trainees have a variable exposure to the advanced procedures in the presence of interventional pulmonary (IP) fellowships, so we surveyed the PCCM program directors (PD) across the United States to assess the procedural volume and competency of their fellows. METHODS: Survey invitations were ema

Pediatric Palliative Care Subcompetencies for Pediatric Critical Care Medicine Fellowship Trainees.

OBJECTIVES: Integration of pediatric palliative care (PPC) and pediatric critical care medicine (PCCM) is essential in providing high-quality patient care. To date, no standardized or recommended framework for educating PCCM fellowship trainees in palliative care exists. We aimed to develop PPC subcompetencies for PCCM fellows within the constructs of the established Accreditation Council for Graduate Medical Education (ACGME) six core competencies. DESIGN AND SETTING: An eight-member multicenter panel consisting of joint PPC and PCCM clinicians with expertise in fellow education curricula de

RISE: Resident Initiative in Surgical Ethics. Creating A Comprehensive Curriculum for General Surgery Residents.

INTRODUCTION: Modern surgical practice presents increasingly complex ethical challenges, shaped by technological advancements and a shift toward shared decision-making and patient autonomy. While technical proficiency is essential, a strong foundation in ethics is critical for surgical residents. However, ethics education in many residency programs remains unstandardized and insufficient. To address this gap, we developed the RISE (Resident Immersion in Surgical Ethics) curriculum using Kern's 6-Step Approach to curriculum development. METHODS: A targeted local needs assessment was conducted

An Academic Family Physician's Point-of-Care Ultrasound (POCUS) Experience.

In family medicine (FM), point-of-care ultrasound (POCUS) usage is rising. Clinically, acute outpatient concerns often lends itself to more opportunities to use POCUS. Common scans utilized in the acute, outpatient setting include soft tissue masses, foreign body assessments, preprocedural guidance for incision and drainage and soft tissue mass excisions, deep vein thrombosis (DVT) ultrasound (US), joint effusions, right upper quadrant (RUQ) US assessments to check for gallstones, lung US for respiratory infections (especially in children), shoulder rotator cuff assessments, bladder US, and US

Assessing the competitiveness of applicants in the ophthalmology match based on interest in pediatric ophthalmology.

BACKGROUND: The number of pediatric ophthalmology and strabismus (PO&S) fellowship positions filled each year remains consistently lower compared with other subspecialities. It is unclear where along the recruitment pathway trainees interested in pediatrics decide to select other subspecialities. This study assesses for differences in the competitiveness of ophthalmology residency applicants based on interest in PO&S, which may impact their ability to matriculate into residency. METHODS: We reviewed the 2022-2023 San Francisco (SF) Match applications submitted to a single ophthalmology reside

Delivering Serious News in Pediatric Cardiology-A Pilot Program.

Pediatric cardiology fellows receive limited training on delivering serious news. This is a teachable skill through simulation-based communication. While studies have shown the use of communication courses in pediatrics, there have been none in pediatric cardiology. Pediatric cardiologists recognize the importance of good communication and desire further development of these skills. Based on an internal needs assessment, three cases were developed; fetal hypoplastic left heart syndrome, teenager with new hypertrophic cardiomyopathy, and young-adult with Fontan failure. A 4-h simulation course

A Reproducible Peritonsillar Abscess Incision and Drainage Model for Junior Trainees.

OBJECTIVES: To describe the design and construction of a reproducible, low-cost, peritonsillar abscess (PTA) incision and drainage simulator and assess its impact on trainees' confidence. METHODS: The 2-part simulator we developed consisted of a manikin head with a fixed, partially open mouth and a modular PTA mold. The mold is created by injecting a lotion and water mixture into plastic bubbles, followed by silicone solidification. Neodymium magnets secure the silicone-abscess packet to the manikin's palate. The simulator was utilized during an academic otolaryngology residency training prog

Nationwide Effect of COVID-19 on Cases Performed During Pediatric Orthopaedic Surgery Fellowship Training in the United States.

INTRODUCTION: The COVID-19 pandemic negatively affected surgical training in the United States. We hypothesized that reported case volume during pediatric orthopaedic surgery fellowship training would decrease markedly during the 2019 to 2020 academic year, which corresponded with the COVID-19 outbreak. METHODS: The Accreditation Council for Graduate Medical Education provided nationwide case logs for accredited pediatric orthopaedic surgery fellows (2017 to 2021). Annual reported case volumes were extracted and summarized as means ± SD. Parametric tests were used to compare annual case volum
