Traumatic brachial plexus injuries are life changing, often leaving affected individuals with severe functional deficits. Recent advances in nerve transfers have allowed surgeons to improve elbow flexion, shoulder abduction, and prehension of the hand in some patients. We hypothesize that in a patient who lacks both biceps and triceps function, a double fascicular transfer may be the key to restore elbow flexion and extension. In three cadaver upper limbs, we transferred the expendable motor fascicle of the ulnar nerve to the biceps branch of the musculocutaneous nerve, and the expendable moto…

BACKGROUND: Open elbow dislocations are rare injuries. Most of the evidence related to these dislocations is found in case reports or in series with closed injuries. We reviewed the experiences of three centers in the treatment of open elbow dislocations.
QUESTION/PURPOSES: We compared the results after operative treatment of simple and complex open dislocations in terms of (1) ROM, (2) functional score, and (3) complications.
METHODS: Eighteen patients were retrospectively included in this study: 11 with simple open elbow dislocations and seven with complex open elbow dislocations. Mean age…

BACKGROUND: Elbow arthrodesis is typically reserved as a salvage procedure. Current literature suggests that satisfactory outcomes can be expected if fusion occurs. There is a paucity of literature on outcomes of elbow arthrodesis after failed elbow arthroplasty with deep infection.
METHODS: Five elbow arthrodeses were performed after a failed elbow arthroplasty due to deep infection. Clinical data were retrospectively reviewed for all surviving patients. Fusion angle, complications, and time to fusion or resection arthroplasty were recorded. The procedure was considered a failure if resectio…

BACKGROUND: Bradydiadochokinesia is one main clinical symptom in idiopathic Parkinson's disease (IPD). The pathogenesis of bradydiadochokinesia is not completely clear.
METHODS: Fifteen patients with IPD and 15 age-matched healthy volunteers had to perform rhythmic alternating flexion and extension movements in the elbow joint. The rhythm was provided auditorily by a click tone stimulator. Six maneuvers (spatial extents of 48 and 83° at frequencies of 0.45, 0.75 and 1.25 Hz) had to be absolved. The potentiometer converted the horizontal forearm movements into a variable voltage.
RESULTS: The…

Many options exist for reconstruction of the posterior elbow/olecranon area following wound formation. Careful early wound management is crucial to ensure successful outcomes following reconstruction. Local and regional options are preferred methods for soft tissue coverage in this region. Common flap options include the reversed lateral arm flap, the radial forearm flap, posterior interosseous artery flap, brachioradialis muscle flap, flexor carpi ulnaris flap, and the latissimus flap. The advantages and disadvantages of these flap options are discussed in this review.

PURPOSE: To describe and evaluate the lateral para-olecranon approach for total elbow arthroplasty and to compare it with the paratricipital and triceps splitting approaches.
METHODS: A total of 34 patients who underwent total elbow arthroplasty were evaluated: 25 with rheumatoid arthritis (28 elbows) and 9 with fractures. The average duration of follow-up was 54 months (range, 12-105 mo). Of the 28 elbows with rheumatoid arthritis, 17 underwent a triceps splitting approach, 6 a lateral para-olecranon, and 5 a paratricipital approach. Of the 9 fracture cases, 5 patients underwent a lateral pa…

INTRODUCTION: Sideswipe injuries constitute a subgroup of complex elbow trauma. Almost all of these are high energy open injuries. There is no fixed protocol that is followed in the earlier studies. The injury pattern is grotesque and ill managed with poor functional outcome. We report the functional outcome in our series of patients who sustained sideswipe injuries.
MATERIALS AND METHODS: Thirty four patients presenting with sideswipe injuries around the elbow were managed and functional results evaluated. The patients were followed for 15-94 months (mean 74 months). 32 of these were males a…

Tennis elbow is a common and difficult-to-treat condition largely because of lack of evidence. The natural history is unknown, but the condition is described as self-limiting. The objective of this case report is to describe the natural course of two control participants (pain free), who later developed tennis elbow, patient history, clinical findings, and ultrasound and colour Doppler examination before, during and after a period of tennis elbow.

BACKGROUND: Juvenile osteochondritis dissecans (OCD) of the elbow typically affects the capitellum and may be "contained" (surrounded by intact cartilage) or "uncontained" (extending beyond the lateral cartilaginous margin). The purpose of this investigation was to compare the clinical presentation, radiographic findings, and surgical results of patients with contained versus uncontained lesions.
METHODS: Forty-three elbows in 42 patients who underwent surgery for OCD were followed for an average of 19.5 months. Average age at surgery was 14.2 years (range, 11.2 to 18.2 y); there were 16 fema…

Subcutaneous emphysema of the upper extremity is rare. Crepitation on physical examination and visible gas on radiographs raise the concern of gas gangrene due to gas-producing bacteria. Rapid establishment of a differential diagnosis is necessary to initiate proper treatment. We present a case of subcutaneous emphysema after elbow arthroscopy caused by a noninfectious genesis. A 59-year-old woman with loose bodies in her left elbow due to mild degenerative joint disease and restricted range of motion was offered an elbow arthroscopy with removal of loose bodies and arthrolysis. Postoperativel…

Posttraumatic elbow stiffness in children can be a severe obstacle to accomplishing basic activities of daily living, including dressing, and participating in recreational activities. The etiology of this stiffness is typically extrinsic or instrinsic pathology caused by an earlier supracondylar humerus fracture, elbow dislocation, or elbow fracture-dislocation. Children with functional limitations, typically associated with flexion contractures greater than 30 degrees or less than a functional arc (30 to 130 degrees) can benefit from surgical contracture release, which consistently improves r…

The elbow is a rare location for septic arthritis, and this fact may produce diagnostic errors. The purpose of this study is to determine the most typical clinical presentation and to evaluate the management and functional outcome of these patients. We present our experience with this entity. This is a retrospective study of seven cases between February 2004 and November 2007, five girls and two boys with a mean age of 2.8 years, ranging from 1.2 to 4.9 years. We explain our diagnosis criteria and management algorithm. Functional results were evaluated using the Mayo Elbow Performance Score. I…

BACKGROUND: Pulled elbow (nursemaid's elbow) is a common injury in young children. It results from a sudden pull on the arm, usually by an adult or taller person, which pulls the radius through the annular ligament, resulting in subluxation (partial dislocation) of the radial head. The child experiences sudden acute pain and loss of function in the affected arm. Pulled elbow is usually treated by manual reduction of the subluxed radial head. Various manoeuvres can be applied. Most textbooks recommend supination of the forearm, as opposed to pronation and other approaches. It is unclear which m…

This paper reports on luxation of the elbow joint without concomitant fracture in a 1-month-old foal. Conservative treatment, with closed reduction and full-limb bandaging, including caudal and lateral splints, seemed successful initially, however, failed to provide enough stability and luxation recurred, and open reduction and surgical placement of prosthetic collateral ligaments was required. Luxation of the elbow joint should be considered when acute non-weight bearing forelimb lameness occurs associated with pain and swelling in the area of the elbow in young foals. Closed reduction failed…

Fifty-six patients who suffered from chronic persistent tennis elbow of more than six months duration were randomly assigned to two active treatment groups. Group 1 (n = 29) received high-energy extracorporeal shock wave treatment (ESWT; 1,500 shocks) at 18 kV (0.22 mJ/mm(2)) without local anaesthesia; group 2 (n = 27) underwent percutaneous tenotomy of the common extensor origin. Both groups achieved improvement from the base line at three weeks, six weeks, 12 weeks and 12 months post-intervention. The success rate (Roles and Maudsley score: excellent and good) at three months in the ESWT gro…
