Ulnar Collateral Ligament Reconstruction in Adolescents: A Systematic Review.

BACKGROUND: Ulnar collateral ligament (UCL) reconstruction is an established surgical technique to restore UCL deficiency, especially in the overhead throwing athlete. Over the past decade, the number of patients requiring UCL reconstruction has increased significantly, particularly in the adolescent patient population. Return-to-play rates after UCL reconstruction reported in the literature have ranged from 33% to 92%, and a recent systematic review noted a return-to-play rate of 89.40% in all high school athletes. PURPOSE: To evaluate the outcomes, particularly return-to-play rates and subj

Biceps-to-triceps transfer in tetraplegia. The medial route.

Eight tetraplegic patients (13 elbows) were treated by biceps-to-triceps transfer. To avoid the risk of radial nerve injury, we chose a medial routing of the biceps. The mean follow-up after surgery was 17.8 months (range, 4-47 months). No complications were encountered. Active extension of the elbow was achieved in each case. The mean postoperative active range of motion of the elbow was 6 degrees extension and 137 degrees flexion. After the biceps-to-triceps transfer mean extension torque of the elbow was 3.7 Nm and mean flexion torque was 10 Nm. In eight elbows in which it was measured, the

The role of ultrasound and magnetic resonance imaging in the evaluation of the forearm interosseous membrane. A review.

The interosseous membrane of the forearm is an important structure to consider in cases of elbow and forearm trauma; it can be injured after elbow or forearm fractures, leading to longitudinal forearm instability. Diagnosis of interosseous membrane injuries is challenging, and failure in diagnosis may result in poor clinical outcomes and complications. Magnetic resonance imaging and ultrasound have shown to be valuable methods for the evaluation of this important structure. Both techniques have advantages and limitations, and its use should be adapted to each specific clinical scenario. This a

Clinical concepts for treatment of the elbow in the adolescent overhead athlete.

Injuries to the adolescent elbow are common because of the repetitive overuse inherent in many overhead sport activities. The management of these patients is greatly facilitated through a greater understanding of the demands placed on the upper extremity kinetic chain during these overhead activities as well as a detailed examination and rehabilitation for the entire upper extremity kinetic chain. Particular emphasis on improving rotator cuff strength and muscular endurance, along with scapular stabilization, is a critical part of elbow rehabilitation in these patients. In addition, the use of

Lateral condylar fracture and ipsilateral ulnar shaft fracture: Monteggia equivalent lesions?

Displaced ulnar shaft fractures are frequently associated with radiohumeral dislocation, producing the Monteggia fracture-dislocation. Fractures not previously thought to coexist were seen in a young boy following a severe automobile-pedestrian accident. The displaced ulnar shaft fracture was associated with a displaced lateral condylar fracture with preservation of the radiocapitellar joint and capsule. This fracture was treated with rigid internal fixation in spite of the open nature of the ulnar shaft fracture. This treatment permitted early range of motion of the elbow with early use of am

Platelet-Rich Plasma for Elbow Pathologies: a Descriptive Review of Current Literature.

PURPOSE OF REVIEW: Platelet-rich plasma is used in many orthopedic pathologies such as tendinopathies and ligament injuries. The clinical results reported in the scientific literature are, however, confusing. The aim of this manuscript is to provide a narrative literature review regarding the role of PRP in the most common elbow soft tissue pathologies. RECENT FINDINGS: The response to PRP seems to be favorable when compared to steroid injection for pain management and for patient-reported outcomes in lateral epicondylitis. PRP injection does not seem to have the potential complications assoc

Using Stress Ultrasonography to Understand the Risk of UCL Injury Among Professional Baseball Pitchers Based on Ligament Morphology and Dynamic Abnormalities.

BACKGROUND: Ulnar collateral ligament (UCL) injury of the elbow is a common and debilitating problem seen frequently among elite baseball pitchers. Ultrasound is a useful diagnostic tool in evaluating UCL injuries. HYPOTHESIS: Evaluation with stress ultrasound of the elbow to measure the morphology of the UCL and the ulnohumeral joint space gapping is indicative of higher risk of UCL injury among professional baseball pitchers. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: Ultrasound imaging was used to assess the medial joint laxity of the elbow of 70 asymptomatic professional

Pediatric Isolated Anterior Elbow Dislocation: A Case Report.

CASE: Anterior dislocation of the elbow is rare in children. This case involves a 9-year-old boy with an anterior dislocation of the right elbow. During surgery, the distal humerus was found to have buttonholed through the triceps and anconeus fascia. An open reduction was performed, and the avulsed periosteal sleeve was sutured back to the humeral origin. The elbow was stable with a mild flexion deformity at the last follow-up, 1 year after surgery. CONCLUSION: This case highlights an uncommon elbow injury with an unusual mechanism, emphasising the need to consider anterior dislocation in si

A case report of children's divergent dislocation of the elbow and review of literature.

BACKGROUND: The divergent dislocation of the elbow is not common in children, and the imaging is difficult and challenging. This often leads to misdiagnosis or inappropriate treatment. The literature has reported a total of 19 cases currently. METHODS: A 10-year-old girl with divergent dislocation of the elbow was admitted in our department in November 2013. When playing basketball, her right elbow was injured on the concrete floor. After injury, her right elbow joint became severely swollen, with obvious deformity. The anteroposterior X-ray of elbow showed right olecranon and coronoid fractu

[Results of open arthrolysis for extrinsic elbow stiffness via radial access. Indications and limits of the procedure].

Open release of elbow stiffness via the lateral approach has proven to be a reliable option in the treatment of elbow stiffness over the last years. For evaluation of patient outcome after this procedure the current literature and studies were reviewed and analyzed with special reference to the increase in range of motion and patient satisfaction. It appears that particularly posttraumatic elbow stiffness with contracture of the capsule and minor alterations of the joint surface can achieve improvement in mobility and patient satisfaction. Therefore careful selection of patients is important f

Kinematics and stability of the fractured and implant-reconstructed radial head.

Controversy exists as to the optimal management of radial head fractures. Biomechanical studies have been conducted to quantify elbow stability for simulated wedge fractures, head excision, and head replacement, with and without the integrity of the collateral ligaments. Our in vitro studies have demonstrated that in the ligamentously intact elbow, kinematics and stability are slightly altered with simulated depressed wedge fractures up to 120 degrees of the radial head, markedly altered with head resection, and improved after radial head replacement. Radial head excision decreases elbow stabi

[Vascular and nervous lesion and opening of the capsule in the last stage of dislocation of the elbow. A case report].

The authors describes a case of posterior dislocation of the elbow in the extreme stage, associating total destruction of the capsule and ligaments, a total arterial lesion, elongation of the median nerve and the opening of the focus at Tscherne's stage III. This case allows considering the biomechanical aspects of dislocation of the elbow and updating the principles of its management.

Treatment of upper extremity nerve defects by direct suturing in high elbow or wrist flexion.

PURPOSE: To evaluate functional outcomes after direct suturing of upper extremity nerve defects in high elbow or wrist flexion. METHODS: A retrospective review was conducted in patients treated for median, ulnar, or radial nerve defects between 2011 and 2019. Inclusion criteria were a defect > 1 cm and a minimal follow-up period of 1 year. Nerve defects were bridged by an end-to-end suture in 90° elbow flexion or 70° wrist flexion for 6 weeks. RESULTS: Nine patients with a mean age of 30.2 years were included. The patients presented with two ulnar nerve defects, four median nerve defects, an

Irreducible posterolateral elbow dislocation.

Elbow dislocations are a high-energy traumatic event resulting in loss of congruence of a stable joint. The majority of elbow dislocations can be reduced by closed means and treated conservatively. We present a case of an irreducible elbow dislocation with reduction blocked by the radial head buttonholed through the lateral ligamentous complex. We performed open reduction with release followed by repair of the lateral ligamentous complex. Clinicians need to understand this unique variant of an elbow dislocation to appropriately treat this operative injury.

Proximal radioulnar translocation associated with elbow dislocation and radial neck fracture in child: a case report and review of literature.

Proximal radioulnar translocation with radial neck fracture and elbow dislocation is extremely rare. We report a case of a 5-year-old boy who was presented with elbow dislocation, and proximal radioulnar translocation was diagnosed a day after the injury. Mini-open technique was used to reduce the translocation and radial neck fracture. The patient finally regained full range of elbow motion and forearm rotation. This case had clinical importance in that the reverse instability of the elbow was observed compared with the previous reports.

Outcomes using the extensor digitorum communis splitting approach for the treatment of radial head fractures.

BACKGROUND: Surgery on the radial head is usually performed via the Kocher interval. Iatrogenic injury to the posterior interosseous nerve (PIN) and lateral ligamentous complex are known complications of lateral elbow approaches. The extensor digitorum communis (EDC) splitting approach for lateral elbow exposure is known to provide better access to the anterior half of the radial head while reducing the risk of injury to the lateral ligaments. The aim of this study was to provide clinical outcome data for the EDC splitting approach. METHODS: Thirteen patients with closed radial head fractures

Arthroscopic Lateral Collateral Ligament Repair for the Acute Elbow Dislocation in Primary Lateral Sclerosis Patient: A Case Report.

INTRODUCTION: Without adequate muscle function as the dynamic elbow stabilizer, in a patient with a neuromuscular disorder, such as primary lateral sclerosis (PLS), the osteoligamentous structures are the only elbow stabilizers. To provide elbow stability, these structures should be restored. CASE: A 30-year-old female with PLS suffered from pain and dysfunction of her elbow after a previous elbow injury and current elbow dislocation. MANAGEMENT AND OUTCOMES: We performed an arthroscopic lateral collateral ligament repair. At 6 months postoperative, her elbow gained stability and a painless

Roller skating--is it a dangerous sport?

A prospective survey of 111 cases of roller skating injuries within one year are reported. Males were more commonly injured than females. There was a high incidence (86%) of serious injuries, 28% of which required surgical treatment. The wrist (23%) was the commonest region involved, followed by the shoulder (20%), the elbow (15%) and the ankle (12%). Collision with other skaters and loss of control were the main factors leading to injury. Better rink discipline, instruction classes and safety publicity should be helpful in minimising accidents.
