Lymphatic filariasis elimination and schistosomiasis control in combination with onchocerciasis control in Nigeria.

This paper describes a pilot initiative to incorporate lymphatic filariasis (LF) elimination and urinary schistosomiasis (SH) control into a mature onchocerciasis control program based on community-directed ivermectin treatment in central Nigeria. In the same districts having onchocerciasis we found LF (as determined by blood antigen testing in adult males) in 90% of 149 villages with a mean prevalence of 22.4% (range 0-67%). Similarly, SH, as determined by dipstick reagent testing for blood in urine from school children, was found in 91% of 176 villages with a mean orevalence in school age ch

Lymphatic filariasis in Papua New Guinea: prospects for elimination.

Lymphatic filariasis is a significant public health problem in several Pacific island countries. Papua New Guinea is one of the most populous countries in this region, and 39% of its residents are estimated to be infected with Wuchereria bancrofti. The Ministries of Health of the 22 islands and territories in the Pacific region are committed to taking action against lymphatic filariasis. Accordingly, a regional collaborative effort aimed at the control of filariasis has been organized under the auspices of a program referred to as PacELF. The main objective of PacELF is to eliminate filariasis

[The control of lymphatic filariasis].

Recent advances in the diagnosis, treatment, and pathophysiology of lymphatic filariasis have raised hopes for eradication. Development of an easy-to-use, rapid-format diagnostic tool has facilitated precise location of infectious zones thus allowing quick intervention. Two new drugs, i.e., ivermectin and albendazole, have been shown to be highly effective in the management of microfilariasis due to Wuchereria bancrofti, Brugia malayi, and Brugia timori thus greatly expanding a therapeutic arsenal once limited to diethylcarbamazine. The current eradication strategy is designed to break the cha

Mattancherry urban agglomeration, a diminishing focus of lymphatic filariasis in Kerala.

Parasitological survey conducted recently in Mattancherry, Kerala showed a low prevalence of microfilariae (mf), with a mf rate of 1.81%, compared to that of 1956 when the mf rate was 14.7%. No brugian infection was encountered in the present investigation, though both bancroftian and brugian infections were reported earlier. Culex quinquefasciatus, the vector of bancroftian filariasis was the most predominant (93.3%) mosquito species, and its infection and infectivity rates were 0.76% and 0.54% respectively. Mansonioides spp., the vectors of brugian filariasis constituted only 0.2% of the tot

Determination of the earliest appearance and peak count of microfilariae of Wuchereria bancrofti and Brugia malayi after taking a single dose of diethylcarbamazine at noon.

Six nocturnal microfilarial carriers (three with Wuchereria bancrofti and three with Brugia malayi) from mainland China were selected to determine the earliest appearance and peak count of microfilariae (mf) administration of DEC at noon. Before diethylcarbamazine (DEC) administration, blood samples of 60 microliters were obtained by finger-prick at 2 h intervals for 24 h. A blood sample was then taken just before each carrier intaking 100 mg, 150 mg or 200 mg of DEC orally at noon. After drug administration, blood samples were obtained at 45 and 60 sec and at 2, 3, 4, 5, 10, 15, 20, 25, 30, 4

Genetic aspects of lymphatic filariasis.

The genetics of human susceptibility to lymphatic filariasis, the genetic basis of filarial susceptibility in vector mosquitos, and the genetic constitution of human filarial parasites and their mosquito vectors are reviewed. It is evident that our present knowledge on the genetics of lymphatic filariasis is still very meagre. The need to study various genetic aspects of the disease is highlighted.

Lymphatic abnormalities in human filariasis as depicted by lymphangioscintigraphy.

BACKGROUND: Investigation into filarial lymphedema has been hampered by the lack of a simple, safe, and easily repeated test to image the peripheral lymphatic system. Recent refinements in radionuclide lymphangioscintigraphy have established this noninvasive technique as the initial procedure of choice for visualizing lymphatics. Accordingly, we applied lymphangioscintigraphy to patients with filariasis and, for purposes of interpretation, compared the findings with those in patients with non-filarial lymphedema. METHODS: Thirty-three patients with classic symptoms or signs consistent with ac

Hydrocele in filarial and non filarial patients. Histopathological, histochemical and ultrastructural studies.

Twenty biopsied material of the testicular tissue were obtained from patients with unilateral hydrocele (eight due to lymphatic filariasis and twelve of idiopathic causes). Twenty biopsies lymphatic were also obtained from the normal unaffected testes and were used as control. The data obtained from histopathological, histochemical and ultrastructural studies revealed alternation in the testicular tissue with hydrocele than in the normal control testis. In the filarial hydrocele 37.5% of the cases showed thickening of the basement membrane and fibrosis with arrest of the spermatogenesis at the

On the uptake of Wuchereria bancrofti microfilariae in vector mosquitoes of different susceptibility to filarial infections.

The quantitative uptake of Wuchereria bancrofti microfilariae by mosquito species of different susceptibility to filarial infection was studied. There was no relationship between the degree of susceptibility and the number of ingested microfilariae. However, in all tested mosquito strains the females ingested as a mean 1.2-1.9 times more microfilariae of W. bancrofti than expected.

Brugia malayi: antibody responses to larval antigens in infected and immunized jirds.

Vaccination with irradiated third stage Brugia malayi larvae (L3) has been reported to induce partial protective immunity to L3 challenge in jirds. The purpose of this study was to identify antigens that may be targets of protective immunity in this model. Jirds were immunized by s.c. injection of irradiated L3 and challenged either s.c. or i.p. Necropsy was performed 11 wk after challenge. Partial protection was achieved in s.c. challenged animals; worm recovery was only 41% of that observed in unvaccinated controls, and worms recovered from immunized animals were stunted. Worm recoveries in

Critical analysis of an epidemetrons model for the assessment of bancroftian filariasis endemicity in some areas in Egypt.

This study attempted a trial fit of observed epidemiological data in Egypt using the Sasa-WHO-Southgate epidemetrons model to quantify the endemicity and risk of transmission of filariasis. The geometric and arithmetic means (GM, AM) were tried in addition to the recommended four epidemetrons of the model, prevalence rate (PR); median microfilaria density (MfD50); and linear regression coefficients (a and b). Data were based on microfilaria counts in 20 mm3 finger-prick night blood samples from all individuals 6 years of age and older of both sexes in a cluster of eight filaria-endemic village

A Brugia species infecting rabbits in the northeastern United States.

Brugia sp. microfilariae were observed in more than 60% of wild rabbits collected on Nantucket Island, Massachusetts. The microfilariae measured 294-344 microns in length and had the characteristic subterminal and terminal nuclei observed in other Brugia microfilariae. The microfilaria is similar to those described for Brugia leporis in rabbits in Louisiana. This may be the Brugia species responsible for 21 documented cases of human infection in the northeastern United States.

Antigenic similarity between the mosquito vectors of malaria and filariasis.

Rabbit antisera were raised to the malaria vector Anopheles tessellatus, head-thorax, abdomen, and midgut preparations. Reactivity of the antisera with An. tessellatus and the filariasis vector, Culex quinquefasciatus, were examined by enzyme-linked immunosorbent assay (ELISA) and Western blots. Although many antigens were shared between the two species of mosquitoes, tissue- and species-specific antigens also were identified.

Use of parasite antigen detection to monitor macrofilaricidal therapy in Brugia malayi-infected jirds.

Improved methods are needed to evaluate new treatments for filarial infections. We have recently developed a monoclonal antibody-based enzyme immunoassay to detect circulating parasite antigen in sera from Brugia malayi-infected jirds. In the present study, parasite antigen levels were compared to parasitological parameters after treatment of B. malayi-infected jirds with CGP 20376 that has been reported to be active against both microfilariae and adult worms of this parasite. Microfilariae were cleared promptly and permanently after CGP 20376 treatment, and no adult worm was recovered in jird

Injection of microfilariae induces resistance to Brugia malayi infection in ferrets and accelerates development of lymphostatic disease.

Ferrets injected intravenously with living microfilariae or cutaneously with microfilariae followed by intravenous injection developed partial resistance to a challenge infection as measured by recovery of adult filariae from lymphatics. Following a challenge infection, the ferrets injected with microfilariae developed lymphatic pathology characteristic of a chronic infection or that observed following multiple infections. There was disruption of lymphatic drainage of the infected limb and lymphoedema. The results suggest that immune responses to antigens of microfilariae, presumably antigens

Cuticular proteins of Brugia filarial parasites.

Surface-labelling techniques have been used to delineate a number of constituent molecules of the cuticle in adult stage Brugia malayi and Brugia pahangi. These molecules can be separated by virtue of their physical properties, and localised either by sequential solubilisation of intact cuticles or immunoelectron microscopy with relevant antisera. The major structural components of the cuticular matrix consist of a set of collagenous proteins of diverse molecular weight ranging from 36 to 160 kDa, cross-linked by disulphide bonds and confined to the basal and inner cortical layers. Each stage
