[Intraspinal enterogenous cyst. Case report].

The authors report the case of a 7 years old boy with cervical pain and tetraparesis progressing by two months. He presented an intraspinal cystic lesion from C4 to C6. Exeresis of the lesion was undergone by anterior way through a corpectomy of C5-C6 followed by reconstruction with bone graft of the iliac crest and metallic plaque. Anatomic diagnosis of the lesion was enterogenous cyst. Aspects of clinical presentation, embryology and treatment of these cysts are discussed.

Ectopic vas deferens, imperforate anus and hypospadias: a new triad.

An ectopic vas deferens is a rare congenital anomaly frequently associated with rectal abnormalities. We describe 2 patients with ectopic vas deferens associated with an imperforate anus and hypospadias. There are 19 previous reports of patients with ectopic vas deferens, 5 of which were bilateral. One of our patients, a child with bilateral ectopic vas deferens, had the highest reported insertion of an ectopic vas deferens. The embryology and management of this rare condition are reviewed.

Ocular and facial maldevelopment: the role of neural crest.

Three children are described who show a spectrum of clinical abnormalities affecting their eyes and non ocular tissues. These entities are best explained as disorders of neural crest migration and once recognised should lead to a search for other systemic developmental disorders. The mechanism of combined facial skeleton and eye malformation is discussed in the light of our current understanding of ocular embryology, and the justification for considering these and other entities as examples of Neurocristopathies is explored.

First branchial cleft anomalies and the facial nerve.

First branchial cleft anomaly is an uncommon clinical problem that can be difficult to diagnose and treacherous to treat. It is generally believed that branchial anomalies arise from incomplete resolution of branchial cleft remnants. They may be a fistulous tract or cystic lesions, and they may be found in all age groups. This article presents three cases of first branchial cleft anomaly and offers an overview of the regional embryology and guidelines for surgical management and facial nerve preservation.

Intraventricular fetus-in-fetu. Case report.

The authors present a case of an intraventricular 14-cm fetus in a 6-week-old infant who presented with enlarging head size. The entire intracranial fetus, with recognizable limbs, trunk, head, and exomphalos, was removed totally from the lateral and third ventricles, with excellent recovery in the patient, who is now 18 months old. The cytogenetical and tissue histological studies are reported. The embryology, operative procedure, and pathology of this condition are discussed and the literature briefly reviewed.

First branchial cleft anomalies.

Our experience with anomalies of the first branchial cleft is reviewed. This includes 38 cases, the largest series collected to date. A new classification is proposed based on the anatomic findings--whether cyst, sinus, or fistula is present. The embryology and pathology of first cleft anomalies are discussed and an approach to the diagnosis and management of these lesions is given. In addition, a patient with a branchial cleft defect involving the middle ear space is reported.

Anatomy of the pituitary gland.

The pituitary gland, historically misconceived as a structure devoted to mucus excretion, has undergone centuries of anatomical and functional reinterpretation. From the ancient humoral theories of Galen to the molecular frameworks of modern embryology, this review offers a comprehensive historical and anatomical overview of the gland. Emphasis is placed on the evolution of terminology, the gland's dual embryologic origin, intricate vascularization, and topographic organization within the sella turcica. Clarifying historical controversies and integrating developmental, surgical, and neuroendoc…

Dorsal bony septum: a split cord malformation variant.

Split cord malformations (SCMs) are rare spinal anomalies and their classification is still a matter of debate. There is no widespread consensus on the embryological basis of this entity. The unified theory, proposed by Pang et al. [Neurosurgery 1992;31:451-480], was an attempt to explain the embryogenetic mechanism as a basic error occurring around the time when the primitive neuroenteric canal closes. We report two unusual cases of SCMs with a dorsally situated bony spur. We analyzed the radiological, clinical and surgical features of the lesions and were not able to classify these cases acc

Inadequacy of APUD concept in explaining production of peptide hormones by tumours.

The amine precursor uptake and decarboxylation (APUD) system of cells has been claimed to derive from the embryological neural crest. This assertion has been uncritically accepted. There is much contradictory evidence, especially about the origin of the gastrointestinal and respiratory APUD cells. There is further evidence that the embryological derivation of a particular cell does not relate to the possibility of ectopic peptide hormone synthesis by malignant tumours arising from that cell type. There are many reports of APUD activity by endodermally and mesodermally derived tumours, and of "

Familial congenital duodenal obstruction.

Duodenal webs are an uncommon cause of duodenal stenosis arising from incomplete bowel recanalisation during embryological development. Despite troubling symptoms such as bilious vomiting in affected infants, prognosis is generally favourable when promptly recognised and treated surgically. Though the condition is recognised as embryological in origin, evidence for familial predisposition is lacking in the medical literature. The authors present a case of duodenal web and concurrent annular pancreas with a positive family history of congenital duodenal obstruction in the infant's father. We di

An Unusual Ectopic Parathyroid Adenoma Location: Pyriform Sinus.

Primary hyperparathyroidism (pHPT) is caused by a single or multiple parathyroid adenomas in 85% of cases, by parathyroid hyperplasia in 15%, and by parathyroid carcinomas in less than 1%. Due to the embryological development characteristics of parathyroid tissues, ectopic parathyroid adenomas can be found in various locations. Although the incidence of pHPT varies, it is not considered a rare disease. Some unusual locations of ectopic parathyroid adenomas can make it difficult for clinicians and surgeons to detect them. For this reason, it is important to understand embryology thoroughly and …

Meningocele manqué: a comprehensive review of this enigmatic finding in occult spinal dysraphism.

The term meningocele manqué (MM) was coined in 1972 to describe a broad range of surgical findings characterized by intradural bands tethering neural structures to the dorsal dura. Over the following decades, reports continued to lump intradural tethering bands associated with a variety of comorbidities under the umbrella term MM. In more recent years, disorders previously called MM have been identified as embryologically distinct and were reclassified. While this sectioning continues, there remains a set of intradural tethering disorders for which no better term than MM exists. Herein, we com

A proposed classification system for familial intestinal atresia and its relevance to the understanding of the etiology of jejunoileal atresia.

Familial cases of the various types of intestinal atresia are well described, and we now report an additional family. Based on a review of the literature, a classification system for the different types of familial atresia is presented. Current teaching attributes most jejunoileal atresias to in utero vascular accidents occurring relatively late in gestation (after the 11th or 12th week). Although some cases clearly occur this way, as a result of processes such as volvulus and intussusception, knowledge of the familial form of the disease indicates that most cases of jejunoileal atresia actual

Lingual thyroid carcinoma: a case report and review of the literature.

Ectopic thyroid tissue may reside anywhere along its embryologic path of descent. Most ectopias manifest as simple thyroglossal duct cysts in conjunction with a normally developed thyroid gland in its usual thyroid cervical bed. Lingual thyroid is a rare developmental abnormality characterized by the failure of the thyroid gland, or remnants, to descend from its embryologic site of origin at the foramen cecum to its usual pretracheal position. Carcinoma arising in a lingual thyroid is even more unusual with fewer than 30 cases reported in the literature. We report the second case of lingual pa

Correction of distal hypospadias with reconstruction of the preputium.

The method described deals mainly with a method of redistributing the skin after reconstruction of the urethra. The rationale for the operation is based on the pathologic embryology of the deformity. The technique is used in distal penile and glanular hypospadias in the absence of chordee. The meatus is relocated with a distally based turnover flap or with a meatoplasty and glanuloplasty. The lateral fusion raphes, which bifurcate in hypospadias from the ventral raphe toward the dogears of the skin hood, are then opened and sutured longitudinally, as fusion should have occurred. The postoperat…

Diplophthalmia versus cyclopia and synophthalmia. Mechanisms of doubling of the eye.

In historical and many scientific papers of the 20th century double formation of eyes has been described as cyclopia, which develops as a variably marked fusion of both eye anlagen into a single eye in median position. In contrast with mythology, no cases were viable, as cyclopia is always accompanied by other severe deformities, such as cerebral defects and proboscis. For the first time a double eye on one side - a unilateral diplophthalmia - in a viable child born in Austria is presented after a reviewal of fundamentals of general and experimental embryology, cyclopia and unilateral synophth…
