Characteristics of tinnitus and etiology of associated hearing loss: a study of 123 patients.

The aim of this study was to highlight the clinical characteristics of tinnitus and to attempt a quantitative assessment in relation to any … ority of tinnitus patients had an endocochlear deafness and, among these patients, acoustic trauma, endolymphatic hydrops, and presbyacusis were the commonest diagnoses (32%, 32%, and 23%, respectively). Of these patients, 93.7% with noise trauma an 86.9% with presbyacusis described their tinnitus as a stable, high-pitched whistling. Those patients with active Ménière' …

[Significance of the glycerol test for the diagnosis of sensorineural hearing disorders].

140 patients with various forms of sensorineural hearing loss were examined via the glycerol test (Klockhoff). In 66% of the cases with Meni … ive. These data support the specificity of the glycerol test: The results depend on the presence of endolymphatic hydrops at the time of examination. Positive results of the glycerol test were also seen in the cases of reduced sensorineural hearing associated with syphilis, as well as in cases of cerebrospinal pressure changing synchronously with fluctuating hearing lo …

Dizziness after head trauma: clinical and morphologic findings.

Twelve patients with balance problems resulting from head injury were treated by a translabyrinthine or middle fossa vestibular neurectomy. The clinical, otoneurologic, and surgical data combined with morphologic findings on the removed operative eighth nerve specimens were evaluated with the aim of deciding the site of primary lesion in each case. In six patients a peripheral lesion was interpreted. Four of them had features of delayed endolymphatic hydrops syndrome, the fifth patient might have suffered a fracture of the stapedial footplate with associated perilymphatic fistula. The sixth pa…

Oxidative stress, redox homeostasis and cellular stress response in Ménière's disease: role of vitagenes.

Ménière's disease (MD) is characterized by the triad of fluctuating hearing loss, episodic vertigo and tinnitus, and by endolymphatic hydrops found on post-mortem examination. Increasing evidence suggests that oxidative stress is involved in the development of endolymphatic hydrops and that cellular damage and apoptotic cell death might contribute to the sensorineural hearing loss found in later stages of MD. While excess reactive oxygen species (ROS) are toxic, regulated ROS, however, play an important role in cellular signaling. The ability of a cell to counteract stressful conditions, known…

Gene Delivery into the Inner Ear and Its Clinical Implications for Hearing and Balance.

The inner ear contains many types of cell, including sensory hair cells and neurons. If these cells are damaged, they do not regenerate. Inner ear disorders have various etiologies. Some are related to aging or are idiopathic, as in sudden deafness. Others occur due to acoustic trauma, exposure to ototoxic drugs, viral infections, immune responses, or endolymphatic hydrops (Meniere's disease). For these disorders, inner ear regeneration therapy is expected to be a feasible alternative to cochlear implants for hearing recovery. Recently, the mechanisms underlying inner ear regeneration have bee…

Can we use electrocochleography as a clinical tool in the diagnosis of Meniere's disease during the early symptomatic period?

CONCLUSION: Our results indicate that electrocochleography (ECoG) has limited value in diagnosing Meniere's disease during the early symptomatic period. OBJECTIVE: We hypothesized that if endolymphatic hydrops is a cause of Meniere's disease, ECoG results obtained in normal subjects would differ from those obtained during the early symptomatic period of Meniere's disease. We aimed to investigate the usefulness of ECoG in the diagnosis of Meniere's disease during the early symptomatic period. METHODS: Extratympanic ECoG was used to evaluate 60 patients in a Meniere's group (17 men, 43 women; …

Contrast enhancement of the inner ear in magnetic resonance images taken at 10 minutes or 4 hours after intravenous gadolinium injection.

CONCLUSION: Contrast enhancement of the inner ear by three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging (3D-FLAIR MRI) taken 4 h after intravenous gadolinium (Gd) injection was better than when taken at 10 min. Using heavily T2-weighted 3D-FLAIR MRI, visualization of endolymphatic hydrops (EH) was possible in the vestibule and the cochlea after a standard intravenous dose of Gd. OBJECTIVES: To define a suitable time point for imaging Gd uptake in the inner ear acquired with heavily T2-weighted 3D-FLAIR MRI after standard intravenous Gd administration. METHODS: U…

Electrocochleography in Retrosigmoid Vestibular Nerve Section for Intractable Vertigo Caused by Meniere's Disease.

Interest in electrocochleography has increased in recent years because of the discovery of an elevated summating potential to action potential amplitude ratio (SP/AP ratio) in patients with endolymphatic hydrops caused by Meniere's disease or perilymph fistula. It was the purpose of this investigation to determine whether the intraoperative SP/AP ratio will decrease after vestibular nerve section in patients with intractable Meniere's disease. Fourteen patients with medically intractable classic Meniere's disease underwent retrosigmoid vestibular nerve section. Intraoperative transtympanic ele…

[Multifrequency tympanometry in Meniere's disease: preliminary results].

Tympanometry with 220 Hz probe tone mainly measures compliance related components of the middle ear. Only higher frequency probe tones provide information on mass related components. In order to study the behavior of the mass related components in endolymphatic hydrops, the Authors tested 15 Ménière's disease patients and 10 healthy subjects using multifrequency tympanometry. In both groups tympanometry with 220 and 678 Hz probe tone was employed to evaluate resonance frequency. The presence of a "camel hump" ([symbol: see text]) pattern obtained with the 678 Hz probe tone test indicated an el…

Audiovestibular manifestations of intracranial hypotension: a descriptive clinical series.

BACKGROUND: Intracranial hypotension (IH) results from decreased cerebrospinal fluid (CSF) volume or pressure and typically presents with orthostatic headache. IH may also manifest with audiovestibular symptoms with variable association to endolymphatic hydrops (EH). However, the nature of this relationship remains unclear. OBJECTIVES: To describe the clinical, audiometric, vestibular, and imaging characteristics of patients with IH presenting with cochleovestibular dysfunction. METHODS: We conducted a retrospective observational study including 12 patients diagnosed with IH based on MRI findi…

Pressure Therapy in Bilateral Ménière's Disease: A Prospective Study.

BACKGROUND: Ménière's disease (MD) is an inner ear disorder characterized by unpredictable vertigo attacks, tinnitus, and aural fullness, ultimately leading to a decline in audiovestibular function. Linked to endolymphatic hydrops (EH), MD often progresses to bilateral involvement (BMD). When conservative therapies fail, MD requires destructive treatments. However, this is a high-risk choice in bilateral cases for hearing preservation. Pressure therapy (PT), a non-invasive treatment using low-pressure pulses to the middle ear, has shown potential in reducing EH and alleviating MD symptoms. Thi…

Surgical treatment of uncompensated vestibular disease.

Uncompensated vestibular disease is a serious, disabling disorder that substantially alters quality of life. Fortunately, most dizziness and vestibular disorders are managed conservatively or with medications and are self-limited. When medical management is unsuccessful, however, there is a wide array of surgical interventions that can significantly improve the symptoms. This article reviews the broad array of surgical techniques available for managing dizziness.
