Evaluation of bleeding on probing and gingival crevicular fluid enzyme activity for detection of periodontally active sites during supportive periodontal therapy.

This study aimed to analyze the enzyme activity in gingival crevicular fluid (GCF) and its association with clinical parameters, especially bleeding on probing (BOP), and thus reconsider the significance and accuracy of recording BOP. A total of 184 patients who had entered supportive periodontal therapy were selected and GCF was collected from 401 sites before recording the clinical parameters, probing pocket depth (PPD), BOP, clinical attachment level, gingival index and plaque index. The enzyme activity of neutrophil elastase and aspartate aminotransferase and amount of protein in GCF were

Improved management of lysosomal glucosylceramide levels in a mouse model of type 1 Gaucher disease using enzyme and substrate reduction therapy.

Gaucher disease is caused by a deficiency of the lysosomal enzyme glucocerebrosidase (acid beta-glucosidase), with consequent cellular accum … r of Gaucher cells in the liver. This was likely because the additional deployment of SRT following enzyme therapy slowed the rate of reaccumulation of GL-1 in the affected organs. Hence, in patients whose disease has been stabilized by intravenously administered recombinant glucocerebrosidase, orally administered SRT with Genz-112638 could potentially be used a …

Use of angiotensin-converting enzyme inhibitor/angiotensin II receptor blocker therapy to reduce cardiovascular events in high-risk patients: Part 1.

Cardiovascular disease is understood as a continuum; risk factors induce a pathophysiologic cascade that culminates in end-organ failure. The renin-angiotensin system (RAS) influences multiple aspects of the pathophysiology via hemodynamic and nonhemodynamic effects. Many long-term clinical trials provide overwhelming evidence of benefits of angiotensin-converting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs) across the cardiovascular continuum, including benefits regarding hypertension, myocardial infarction, stroke, renal disease, and heart failure. Trials also indicate

Abnormal expression and processing of uromodulin in Fabry disease reflects tubular cell storage alteration and is reversible by enzyme replacement therapy.

Uromodulin (UMOD) malfunction has been found in a range of autosomal dominant tubulointerstitial nephropathies associated with hyperuricaemia, gouty arthritis, medullary cysts and renal failure-labelled as familial juvenile hyperuricaemic nephropathy, medullary cystic disease type 2 and glomerulocystic kidney disease. To gain knowledge of the spectrum of UMOD changes in various genetic diseases with renal involvement we examined urinary UMOD excretion and found significant quantitative and qualitative changes in 15 male patients at various clinical stages of Fabry disease. In untreated patient

Aspirin, beta-blocker, and angiotensin-converting enzyme inhibitor therapy in patients with end-stage renal disease and an acute myocardial infarction.

OBJECTIVES: We sought to examine the use and impact of standard medical therapies in patients with end-stage renal disease (ESRD) faced with an acute myocardial infarction (AMI). BACKGROUND: The poor prognosis of patients in this high-risk population has become increasingly well recognized. METHODS: Using the ESRD database and the Cooperative Cardiovascular Project (CCP) database, we identified AMI patients who were receiving either peritoneal dialysis or hemodialysis before admission. The early administration of aspirin and beta-blockers was compared between ESRD and non-ESRD patients and t

[Enzyme activity and content of glycerophosphate shuttle metabolites in the cortex and medulla oblongata during hyperbaric oxygenation therapy of acute hemorrhage].

The experiments on cats have demonstrated that hyperbaric oxygenation (303.9 kPa, 60 min) in acute blood loss (24.0 +/- 0.8 ml/kg) has an essential effect on glycerophosphate shuttle mechanism: it stimulates mitochondrial glycerol-3-phosphate dehydrogenase in the sensorimotor cortex and medulla oblongata, providing glycerol-3-phosphate dehydrogenation, activates cytoplasm hydrogen delivery to mitochondrial respiratory chain, prevents glycerol-3-phosphate and NADH level increase and cytoplasm glycerol-3-phosphate dehydrogenase inhibition, limiting lactate production.

First multicenter real-world analysis of switching to next-generation enzyme replacement therapies in late-onset Pompe disease.

BACKGROUND: Next-generation enzyme replacement therapies (ERTs) for late-onset Pompe disease (LOPD), including avalglucosidase alfa and cipaglucosidase alfa with miglustat, have been developed to improve muscle targeting and enzyme stability. Real-world evidence on therapy switching between ERT preparations remains limited. METHODS: A prospective, observational, multicenter cohort study was conducted across German neuromuscular centers. Adults with genetically confirmed LOPD who transitioned to avalglucosidase alfa (Aval) or cipaglucosidase alfa with miglustat (Cipa/Mig) between August/2022 a

Pharmacogenetic Profiling of Cytochrome P450 Enzymes in Plasmodium vivax Patients Treated with Chloroquine and Primaquine: Implications for Personalized Malaria Therapy.

This study aimed to investigate the impact of common polymorphisms in key cytochrome P450 (CYP) genes (CYP2C8, CYP2C9, CYP2C19, CYP2D6, and CYP3A5) on drug levels and treatment responses in Plasmodium vivax-infected patients treated with chloroquine (CQ) and primaquine (PQ). CYP2C8 and CYP3A5 are the key enzymes that metabolize CQ, whereas CYP2D6, CYP2C9, and CYP2C19 are those that metabolize PQ. A total of 99 DNA samples from Burmese patients with microscopically confirmed P. vivax monoinfection, who received standard doses of CQ (25 mg/kg) and PQ (15 mg/kg/day for 14 days), were included for

Chirality-Driven Metal-Phenolic Artificial Enzymes Enable Renal Targeting and Antioxidative Therapy Enhancement for Acute Kidney Injury.

Despite advancements in artificial enzymes (AEs) for acute kidney injury (AKI) therapy through renal redox homeostasis modulation, the trade-off between organ-specific targeting and high multienzyme-mimicking catalytic efficiency is still a big challenge. Herein, we address this challenge through chirality-engineered Cu2+-phenolic AEs (l-phen@Cu-TA) via incorporation of l-phenylalanine (l-phen) to synergize stereoselective recognition and catalytic activity. Chiral l-phen induces electron density redistribution from phenolic ligands to Cu2+ in the cocatalytic center, significantly enhancing H2

Mechanisms of Neutralizing Anti-drug Antibody Formation and Clinical Relevance on Therapeutic Efficacy of Enzyme Replacement Therapies in Fabry Disease.

Fabry disease (FD) is a rare X-linked lysosomal storage disorder caused by mutations in the α-galactosidase A (AGAL/GLA) gene. The lysosomal accumulation of the substrates globotriaosylceramide (Gb3) and globotriaosylsphingosine (lyso-Gb3) results in progressive renal failure, cardiomyopathy associated with cardiac arrhythmia, and recurrent strokes, significantly limiting life expectancy in affected patients. Current treatment options for FD include recombinant enzyme-replacement therapies (ERTs) with intravenous agalsidase-α (0.2 mg/kg body weight) or agalsidase-β (1 mg/kg body weight) every

Prognostic Impact of Angiotensin-Converting Enzyme Inhibitors and Receptor Blockers on Recurrent Ventricular Tachyarrhythmias and Implantable Cardioverter-Defibrillator Therapies.

This study sought to assess the prognostic impact of treatment with angiotensin-converting enzyme inhibitors (ACEi) or angiotensin receptor blockers (ARB) on recurrences of ventricular tachyarrhythmias in recipients of implantable cardioverter-defibrillators (ICD). Using a large retrospective registry including consecutive ICD recipients with documented episodes of ventricular tachycardia (VT) or fibrillation (VF) from 2002 to 2016, those patients treated with ACEi/ARB were compared with patients without. The primary prognostic endpoint was the first recurrence of ventricular tachyarrhythmias

Enzyme replacement therapy in a patient of heterozygous Fabry disease: clinical and pathological evaluations by repeat kidney biopsy and a successful pregnancy.

Fabry disease is a rare X-linked lysosomal storage disorder of glycosphingolipid catabolism caused by deficient activity of the lysosomal hydrolase alpha-galactosidase A (ɑ-Gal A). A 20-year-old woman was referred to our hospital because of proteinuria and persistent macroscopic hematuria. Based on the typical renal pathological findings, deficient activity of the ɑ-Gal A, and heterozygous mutation in the ɑ-Gal A gene, she was diagnosed with Fabry disease. After 1 year of enzyme replacement therapy with agalsidase alfa at 0.2 mg/kg every other week, the patient's proteinuria and hematuria were

Autopsy case of Gaucher disease type I in a patient on enzyme replacement therapy. Comments on the dynamics of persistent storage process.

We report a female patient with Gaucher disease (GD) type I on ERT (imiglucerase) for 5 years, which led to a significant general improvement. Aged 59 years she underwent an episode of altitude sickness followed by sepsis, disseminated intravascular coagulation, and multiorgan failure. She succumbed to a cerebral haemorrhage. Autopsy revealed liver cholestatic cirrhosis and multifocal liver carcinoma with immunophenotype compatible with cholangiocarcinoma. Analysis of the storage process revealed its absence or very low levels in the majority of liver and spleen macrophages. Gaucher cells (GCs
