Slipped capital femoral epiphysis (SCFE) is a common adolescent hip disorder. This article reviews the major controversies in SCFE as of the year 2005. These are (1) treatment of the unstable SCFE, (2) the role of osteotomy in the treatment of SCFE, (3) prophylactic fixation of the contralateral hip in children presenting with unilateral SCFE, and (4) methods of fixation in the very young child with SCFE.

Various methods of performing proximal femoral realignment in adolescents with severe slipped capital femoral epiphysis exist. We report the technique and early results of a percutaneous, opening wedge subtrochanteric femoral osteotomy using an external fixator for correcting multiplanar deformities in such patients. Nine adolescents with severe slipped capital femoral epiphysis underwent a percutaneous osteotomy at an average age of 14.5 years. Mean operative blood loss was 61 ml, with 2 days of inpatient stay and 129 days of external fixation time. At an average follow-up of 23 months, hip f

This particular ailment has many designations in the literature but none is quite adequate. Here we use the terminology slipped capital femoral epiphysis (SCFE). The anatomy of the proximal femur in all mammals reflects their growth and function. The main forces acting on the head are perpendicular to the growth plate. The thick tear-proof perichondrium-periosteum on the femoral neck is like a stocking attached to the epiphysis and the trochanter region. Growth in length causes a strong tensile stress in the periosteum, pressing the epiphysis against the metaphysis and thus stabilizing the vul

AIM: The frequency of bilateral slipped capital femoral epiphysis (SCFE) is 5-80%. A set of boy twins developed left-sided SCFE within 2 years. We tried to find a relationship between SCFE and HLA typing.
METHOD: We obtained the HLA typing and compared it to previously reported cases.
RESULTS: By comparing 14 gene loci we made sure that they are identical twins. In our patients as well as in may of previously reported cases of sets of boy twins HLA phenotype A2 were found, while in the sets of girl twins HLA phenotype A11 and B12 were found.
CONCLUSIONS: HLA phenotyping is needed in a large

Slipped capital femoral epiphysis (SCFE) leads to an unphysiologic function in adolescent hips. Evaluation of the slippage as well planning of corrective osteotomies is a major three-dimensional (3D) problem. Therefore, the current clinical evaluation, which is based on biplanar plain radiographs, cannot be satisfying. More is needed than simply measuring the femoral geometry to evaluate the impact of a slippage onto the physiologic hip function. We have developed a computer-based system for planning and evaluation of reorienting osteotomies in severe cases of SCFE. In our system, CT-based 3D

PURPOSE OF THE STUDY: Allogenic B.M.T. has been investigated as a curative treatment of anemia in S.C.D. Furthermore, correction of functional asplenia has been observed after B.M.T.; it remains to be determined if such a treatment may reserve other organ damage and particularly bone abnormalities and osteonecrosis frequently associated with S.C. hemoglobinopathies.
MATERIAL AND METHODS: The aim of this retrospective review was to evaluate the radiologic and MRI results of 2 patients with avascular necrosis before the B.M.T. Roentgenographic diagnosis of osteonecrosis before B.M.T. included,

Premature closure of the capital femoral physis after pin fixation of slipped capital femoral epiphysis (SCFE) in the juvenile population (at least 1 year less than the established mean) can lead to growth disturbances of the proximal femur. A retrospective review of 21 patients (33 hips) who had a pinning of a juvenile SCFE was performed. Growth disturbances including greater trochanteric overgrowth, coxa vara, and coxa breva were noted in 64% of the hips. An 80% incidence of bilateral involvement was noted in the juvenile group. Consideration should be given to prophylactic pinning of the co

Transtrochanteric rotational osteotomies were successful in 342 hips (274 patients) for the treatment of idiopathic and steroid-induced osteonecrosis of the femoral head, Perthes' disease, slipped capital femoral epiphysis, osteoarthritis, and other disorders. One hundred fifty-eight hips (113 patients) with idiopathic and steroid-induced necrosis were followed up for two to 11 years. In 121 of 158 hips, excellent results were obtained clinically and roentgenographically. In 42 Grade I or II hips, excellent results were obtained in every respect (success rate, 86%). Eighty-eight of 93 hips in

During an eight-year period, twenty-four Southwick osteotomies were performed in twenty-one patients with severe chronic slipped capital femoral epiphysis. After follow-ups ranging from two to ten years (average, four and one-half years), correction of the deformity had been maintained in all but one patient, who has loss of fixation of the osteotomy. One patient had joint-space narrowing, avascular necrosis of the femoral head, and significant restriction of motion which was still present three years after operation. We concluded that this osteotomy gives good correction of the deformity but

The stages in adolescent slipping of the upper femoral epiphysis are classified in relation to treatment. The operation of open replacement of the displaced femoral head is described, and the results of a personal series of seventy-three such operations are presented. Open replacement is excellent treatment for severe chronic slipping so long as the growth plate is still open. The greater incidence of avascular necrosis in acute-on-chronic cases is probably due to damage to the blood supply of the head at the time of the acute slip or kinking of the vessels before replacement. Prolonged tracti

BACKGROUND: Although femoral retroversion has been linked to the onset of slipped capital femoral epiphysis (SCFE), and may result from a rotation of the femoral epiphysis around the epiphyseal tubercle leading to femoral retroversion, femoral version has rarely been described in patients with SCFE. Furthermore, the prevalence of actual femoral retroversion and the effect of different measurement methods on femoral version angles has yet to be studied in SCFE.
QUESTIONS/PURPOSES: (1) Do femoral version and the prevalence of femoral retroversion differ between hips with SCFE and the asymptomat

BACKGROUND: Avascular necrosis (AVN) of the capital femoral epiphysis (CFE) after an unstable slipped capital femoral epiphysis (SCFE), femoral neck fracture or traumatic hip dislocation can result in severe morbidity. Treatment options for immature patients with AVN are limited, including a closed bone graft epiphysiodesis (CBGE). However, it is unclear whether this procedure prevents AVN progression.
QUESTIONS/PURPOSES: We investigated whether early MRI screening and CBGE prevented the development of advanced AVN changes in the CFE and the rates of complications with this approach.

Nearly one-third of children in the United States are overweight or obese by their preteens. Tall stature and accelerated bone elongation are characteristic features of childhood obesity, which cooccur with conditions such as limb bowing, slipped epiphyses, and fractures. Children with obesity paradoxically have normal circulating IGF-I, the major growth-stimulating hormone. Here, we describe and validate a mouse model of excess dietary fat to examine mechanisms of growth acceleration in obesity. We used in vivo multiphoton imaging and immunostaining to test the hypothesis that high-fat diet i

BACKGROUND: Slipped Capital Femoral Epiphysis (SCFE) is a prevalent pediatric orthopedic condition. Treatment options range from in situ pinning to various osteotomies, with the Modified Dunn procedure gaining significant attention over the past two decades. However, the suitability of this procedure for different SCFE subtypes and the risk of avascular necrosis (AVN), particularly in moderate and severe cases, remains controversial. This study aims to report the midterm clinical and radiographic outcomes of the Modified Dunn procedure in treating SCFE, emphasizing the factors contributing to

BACKGROUND: Severe slipped capital femoral epiphysis (SCFE) leads to femoroacetabular impingement and restricted hip motion. We investigated the improvement of impingement-free flexion and internal rotation (IR) in 90 degrees of flexion following a simulated osteochondroplasty, a derotation osteotomy, and a combined flexion-derotation osteotomy in severe SCFE patients using 3D-CT-based collision detection software.
METHODS: Preoperative pelvic CT of 18 untreated patients (21 hips) with severe SCFE (slip-angle>60 degrees) was used to generate patient-specific 3D models. The contralateral hips

OBJECTIVE: This study aimed to investigate the usefulness of bone single-positron emission tomography/computed tomography (SPECT/CT) of the hip in predicting the later occurrence of avascular necrosis (AVN) after slipped capital femoral epiphysis (SCFE) or femoral neck fracture in pediatric patients. The quantitative parameters of SPECT/CT useful in predicting AVN were identified.
MATERIALS AND METHODS: Twenty-one (male:female, 10:11) consecutive patients aged < 18 years (mean age ± standard deviation [SD], 11.0 ± 2.7 years) who underwent surgery for SCFE or femoral neck fracture and postoper

The standard treatment of stable slipped capital femoral epiphysis (SCFE) is generally accepted to be in situ pinning. Controversy exists regarding the treatment of unstable SCFE, including the role of a purposeful closed reduction or open reduction. The objective of this study was to investigate the rate of avascular necrosis (AVN) with purposeful closed reduction and in situ pinning of unstable SCFE. The authors retrospectively reviewed 221 patients with 302 SCFE hips treated with in situ pinning between 2000 and 2014. Forty-eight patients (50 hips) presented with an unstable SCFE. All unsta

BACKGROUND: The proximal femoral head separation (FHS) or epiphysiolysis is a prevalent disorder affecting the chicken femur epiphysis, being considered a risk factor to infection which can cause bacterial chondronecrosis with osteomyelitis in broilers. To identify the genetic mechanisms involved in epiphysiolysis, differentially expressed (DE) genes in the femur of normal and FHS-affected broilers were identified using RNA-Seq technology. Femoral growth plate (GP) samples from 35-day-old commercial male broilers were collected from 4 healthy and 4 FHS-affected broilers. Sequencing was perform
