BACKGROUND: Globally, the most common cause of cranio-facial fractures is road traffic accident (RTA) with computerized tomography (CT) scan as the gold standard in the diagnosis of patients with cranial and facial fractures. The purpose of this study is to assess the pattern of cranial and facial fractures on CT in head injured patients following RTA.
MATERIALS AND METHODS: Using CT, detailed analyses of 236 patients with head injury following RTA were performed between 2006 and 2008, Data recorded included cause of injury, age and gender distribution, cranial and mid-facial fractures sustai

Facial trauma is a common presentation to the emergency department, encompassing a spectrum from soft tissue injuries to fractures of the facial skeleton. Beyond the evident physical consequences, patients frequently face significant psychosocial issues, which are often overlooked in the recovery phase of treatment. The purpose of this review was to establish the experiences of patients following facial trauma using qualitative patient-reported data. A scoping review, searching four electronic databases (MEDLINE, Web of Science, PsychINFO, and Cochrane Library) using PRISMA methodology was con

The facial nerve is often injured in head trauma. Computed tomography (CT) is the diagnostic study most frequently used to evaluate temporal bone fractures, but it does not demonstrate all soft tissues well. Recently, magnetic resonance imaging (MRI) has been used to evaluate certain soft tissues, including cranial nerves. Experience with MRI evaluation of temporal bone trauma is limited and consists primarily of anecdotal reports. This study assesses the accuracy of MRI in evaluating experimentally induced acute intratemporal facial nerve lesions. The tympanic segments of the right facial ner

Midface fractures are a common occurrence and may be associated with other injuries. It is important to perform diagnosis and treatment planning on an interdisciplinary basis. CT is the gold standard diagnostic modality. Classification according to the pattern of fracture is via the LeFort and other systems. Treatment objectives are anatomic repositioning to preserve the function of the eyes, the infraorbital nerve, and facial symmetry, as well as to preserve masticatory function. The number of surgical access routes should be minimized and incisions of the facial skin avoided wherever possibl

The utilization of the bicoronal scalp flap in craniofacial trauma has proved indispensable in the management of severe craniofacial injuries. It provides vast exposure of such critical structures as the cranium, frontal sinus, orbit and upper midface, compared with that for previous techniques of facial fracture reduction. Although the flap has great utility, severe complications, such as facial nerve injury, diplopia, telecanthus, and scalp necrosis, can occur. This article reviews the surgical anatomy, technique, and indications for the safe utilization of the bicoronal scalp flap approach …

Massive skull base injuries require detailed preoperative neurological and neurovascular assessment prior to undertaking surgical repair of isolated cranial nerve deficits. We present the management of a patient with traumatic facial paralysis, cerebrospinal fluid leak, and carotid artery cavernous sinus fistula as the result of a gunshot wound to the skull base. The carotid artery cavernous sinus fistula was ultimately controlled with super-selective embolization via the vertebral artery. The facial nerve injury was then safely treated with mobilization of the labyrinthine and vertical segmen

First branchial cleft anomalies are rare. Its estimated incidence is one in 100,000. Clinically, patients present with recurrent otorrhea, periauricular swelling, and/or flowing fistula in the neck. Surgical removal of the tract is considered the best treatment option for the first branchial cleft fistula. Due to the close relationship between the fistula tract and the facial nerve branches, facial nerve injury is one of the not uncommon complications of this surgery. Different variations in the relationship between the fistula tract and the facial nerve trunk and its branches have been mentio

Second or more surgical procedures on parotid are usually difficult and may induce injury on the facial nerve. The authors report their experience about 42 patients. The choice between a total parotidectomy or a surgical excision of a tumor depends of the number of surgical procedure, the type of initial procedure and the histological type of the tumor. Second surgical procedures for a wrong initial histological diagnosis (11 cases) were always a total parotidectomy with facial nerve preservation. The risk of a partial or total facial palsy is higher after several recurrences (3 of 8 cases). T

With the increase in the elderly population in the United States, the number of people seeking care for trauma injuries is expected to increase. However, nationwide studies on epidemiological profile of elderly facial fractures remain sparse. The authors present the epidemiological measures of elderly facial fractures on national scale. Characteristics of facial fractures among younger adults (18-64 years old) and the elderly population (65-74 years old, 75-84 years old and 85 years and above) have been examined using the 2016 American College of Surgeons-Trauma Quality Improvement Program dat

To investigate the effect of FK506 on apoptosis of facial motor neurons in rats and its possible mechanism. A total of 48 Wistar rats were randomly divided into experimental group and control group. Facial nerve injury model was established by transection of facial nerve at stylomastoid foramen. Rats in experimental group and control group were provided with FK506 and normal saline by intraperitoneal injection, respectively. The morphology of facial neurons was observed under light microscope at different time points after injury. Apoptotic facial motor neurons were detected by TdT-mediated dU

OBJECTIVE: Delayed facial nerve palsy (dFNP) secondary to head injury is definitely uncommon. Although the mechanism of immediate facial nerve paralysis is well-studied, its delayed presentation remains debated. Given the dearth of available information, we reported herein our experience with 2 cases of posttraumatic dFNP. This systematic review aimed to evaluate all available information on dFNP and to assess treatment outcome also comparing conservatively and surgically approaches.
DATA SOURCES: Pubmed, Scopus, and Web of Science databases were systematically screened.

Nerve substitution is an important tool in facial reanimation. The goal is to reinnervate the distal facial nerve and musculature using an alternative cranial nerve in order to achieve facial movement, symmetry, and tone. Multiple donor nerves have been used for nerve transfer procedures, the most common being hypoglossal, masseteric, and cross-facial nerve graft. Each donor nerve has its advantages and disadvantages. Multiinnervation uses the use of multiple donor nerves in order to leverage the benefits while balancing the pitfalls of each nerve. The nerve transfer depends on the type of ner

Acute facial nerve injuries involving the total facial nerve (n = 202) and its segmental branches (n = 63) were repaired with a variety of neural (n = 225) and myofascial transfer (n = 40). A system for evaluating results based on facial symmetry and tone at rest, recovery of voluntary mimetic activity, synkinesis, and recovery of selective function in discrete facial nerve divisions is presented. The best results were achieved with immediate direct end-to-end neural-epineural anastomotic repairs. The least favorable results were seen with myofascial transpositions and long extratemporal rerou

Tracheal trauma is uncommon but carries major morbidity and mortality. A 26-year-old man sustained a near-transection of the cervical trachea due to penetrating trauma. Venovenous extracorporeal membrane oxygenation support allowed a controlled primary repair with muscular buttress and facilitated airway management. Facial injuries prevented oral intubation, and retrograde intubation through the transection established an airway. On the 10th postoperative day a percutaneous tracheostomy was performed through the surgical site. This case discusses the management, technical details, and adjuncts…

Penetrating craniofacial trauma, although uncommon, has a high potential for death or serious morbidity from injury to vital neurovascular structures. An in situ facial foreign body, particularly if large, presents significant challenges beyond safe and timely removal. Airway management, stabilization of the object, management of increased intracranial pressure, and identification of injuries to local structures are all issues that may require addressing. We present a case of penetrating facial trauma from a vice clamp, with an in situ foreign body, that illustrates several of these challenges

The paper reviews 27 cases of traumatic facial paralysis undergone surgical treatment. Most of the cases showed injuries to the horizontal and perigeniculate segments of the facial nerve. All cases have been followed up for 6 months to 2 years. Complete recovery occurred in 20 of the cases and partial recovery in 7 cases (II in 2, III in 5). Trans-mastoid/attic approach was used which yielded exposure of the geniculate ganglion and distal part of the labyrinthine segment of the nerve. Indications and surgical approach were discussed.

Facial nerve injuries produce lagophthalmos and consequent ocular disease caused by corneal exposure. The management of the affected eye in patients with facial palsy has been improved. Previously ointment, eye drops, taping, partial or complete tarsorrhaphy was the primary treatment of the inability to close the eyelid. Other mechanical techniques for reanimating lid closure, including palpebral springs, encircling the upper and lower eyelids with silicone or fascia lata, and temporalis muscle transfer. The most popular and widely used static procedure in facial nerve palsy is the upper eyeli
