Necrotizing fasciitis of the breast in a breastfeeding mother: A case report.

INTRODUCTION: Necrotizing fasciitis (NF) is a rapidly progressive life-threatening infection involving necrosis of subcutaneous tissues and fascia. Though it mainly affects the extremities, perineum and abdominal wall, it rarely affects the breast. PRESENTATION OF CASE: A 30-year-old woman on postpartum day seven presented with one-day history of fever with chills and rigors and painful, swollen and tender left breast. Although the initial diagnosis was mastitis, the disease progressively worsened with the breast developing ischaemic necrotic areas in the lower quadrants and nipple areolar re…

Synchronous Multifocal Necrotizing Fasciitis in an Infant: A Case Report.

We report an exceptional form of a synchronous multifocal necrotizing fasciitis (NF) affecting a 16-month-old girl. A high clinical suspicion of NF was confirmed by her laboratory risk indicators for necrotizing fasciitis score and magnetic resonance imaging. Blood cultures were positive for Streptococcus pyogenes . Successful outcomes were achieved through antibiotic treatment and an isolated surgical exploration using negative-pressure wound therapy.

Imaging features of nodular fasciitis: a review of 89 cases.

OBJECTIVE: To review the spectrum of clinical and imaging features of nodular fasciitis, emphasizing those most characteristic of this diagnosis. MATERIAL AND METHODS: A retrospective review of our institutional pathology and imaging databases from 2006 to 2024 identified 89 patients with confirmed nodular fasciitis pathology and imaging evaluation. For the purpose of analysis, lesions were subdivided into 2 basic groups: superficial and deep. Superficial lesions were defined as those involving the skin, superficial fascia, subcutaneous fat, and/or extending to the deep fascia. Deep lesions w…

A Case of Nodular Fasciitis Protruding From the Anterior Nostril.

Primary nodular fasciitis of the nasal cavity is quite rare, and only a few cases have been reported. The patient was a 40-year-old man whose chief complaint was a nasal tumor. We suspected fibrosarcoma and operated. The final diagnosis was nodular fasciitis. There was no recurrence at six months postoperatively. In this report, we describe a case of nodular fasciitis protruding from the anterior nostril and reviewed the literature.

Unmasking the Veiled Intruder: A Complicated Case of Eosinophilic Fasciitis.

Eosinophilic fasciitis (EF) is a rare inflammatory disease characterized by skin and fascial thickening. Unlike systemic sclerosis, EF lacks internal organ involvement and specific autoantibodies, with peripheral eosinophilia as a hallmark feature. Patients may exhibit joint pain and contractures due to fibrosis. We present a case of a patient who presented with skin thickening involving her upper and lower extremities and was ultimately diagnosed with EF based on a skin biopsy. This case underscores the importance of recognizing the unique clinical and histological features of EF.

Do the fasciae of the soleus have a role in plantar fasciitis?

Plantar fasciitis is a chronic, self-limiting, and painful disabling condition affecting the inferomedial aspect of the heel, usually extending toward the metatarsophalangeal joints. There is compelling evidence for a strong correlation between Achilles tendon (AT) loading and plantar aponeurosis (PA) tension. In line with this, tightness of the AT is found in almost 80% of patients affected by plantar fasciitis. A positive correlation has also been reported between gastrocnemius-soleus tightness and heel pain severity in this condition. Despite its high prevalence, the exact etiology and path…

A Case of Necrotizing Fasciitis Mimicking a Burn in an Elderly Patient.

Necrotizing fasciitis travels along the fascial plane and surrounding soft tissue, leading to ischemia and necrosis. Fournier's gangrene is a type of necrotizing fasciitis invading the deep and superficial planes of the perineal/genital region. It is rapidly progressive in nature and may have life-threatening consequences. Fournier's often exhibits a misleading clinical presentation and can be mistaken for other conditions, such as hematoma, phlebitis, cellulitis, or septic arthritis. Since the ramifications of delayed diagnosis can be clinically significant, recognition of potential mimics is…

The Impact of Cefuroxime Susceptibility on Aeromonas Necrotizing Fasciitis Outcomes.

Despite aggressive antibiotic therapy and surgical debridement, Aeromonas necrotizing fasciitis (NF) can lead to high amputation and mortality rates. Our study compares the different antibiotic minimum inhibitory concentrations (MICs) via Epsilometer tests (E-tests) between non-survivors and survivors of Aeromonas NF of limbs. A prospective review of 16 patients with Aeromonas NF was conducted for 3.5 years in a tertiary coastal hospital. E-tests were conducted for 15 antimicrobial agents to determine the MIC value for Aeromonas species. These patients were divided into non-survival and surviv…

Necrotizing fasciitis: an update on epidemiology, diagnostic methods, and treatment.

PURPOSE OF REVIEW: The aim of this review is to discuss the latest evidence of epidemiology, diagnostic methods, and treatment of necrotizing soft tissue infections (NSTIs) with a particular focus on necrotizing fasciitis (NF). RECENT FINDINGS: NSTIs have been historically referred to as NF but encompass a broader range of infections, with variable rates ranging from 0.86 to 32.64 per 100 000 person-years, influenced by factors such as climate and seasonal variations. They have diverse microbiological profiles categorized into different types based on the involved pathogens, including polymic…

Pediatric Cervicofacial Necrotizing Fasciitis-A Challenge for a Medical Team.

Cervical necrotizing fasciitis is a very rare complication of a bacterial infection that can have a dental cause. This type of infection typically affects fascial plane, which has a poor blood supply and can affect soft tissue and cervical fascia and can spread quickly causing infection of mediastinum. Initially, in the first stage, the overlying tissues are unaffected, and this can delay diagnosis and surgical intervention. Incidence in children is extremely rare and can be frequently associated with various other general pathologies that decrease the immune system response. We present a case…

A Rare Complication of Perforated Appendicitis: A Case of Necrotizing Fasciitis.

Acute appendicitis is considered one of the most common surgical emergencies with low morbidity and mortality. However, delay in the diagnosis may lead to perforation of the appendix. Hence, complications may arise, including necrotizing fasciitis, a rare complication of a perforated appendix. We present a case of perforated appendicitis complicated by necrotizing fasciitis leading to rapid deterioration. A 75-year-old male patient presented to our emergency room with a three-day history of right lower quadrant abdominal pain and abdominal distention. On admission, computed tomography (CT) sca…

Necrotizing fasciitis of the thigh due to a secondary aortoduodenal fistula.

Background: Secondary aortoenteric fistula is an iatrogenic complication after aortic reconstructive surgery presenting with gastrointestinal bleeding and/or infectious symptoms. Infrequently, it may manifest with nonspecific and atypical clinical signs. We present a case of necrotizing fasciitis of the thigh complicating secondary aortoduodenal fistula, diagnosed with CT-scan. Case presentation: A 67-year-old man with a history of an open aortic-bifemoral bypass 6 years ago was admitted for a progressively swollen and painful right thigh for the last month. Through laboratory and morphologica…

Ultrasound for the diagnosis of necrotizing fasciitis: A systematic review of the literature.

BACKGROUND: Necrotizing fasciitis (NF) is a deadly disorder that can be challenging to diagnose on history and examination alone. Point-of-care ultrasound (POCUS) is widely available and has been increasingly used for diagnosing skin and soft tissue infections. We performed a systematic review to determine the accuracy of POCUS for diagnosing NF with subgroup analyses of the accuracy of specific POCUS examination components. METHODS: PubMed, Scopus, CINAHL, LILACS, the Cochrane databases, Google Scholar, and bibliographies of selected articles were assessed for all retrospective, prospective,…

An unusual pseudolymphoma in the context of necrotizing fasciitis: A case report.

RATIONALE: The diagnosis of lymphoma in routine diagnostics can be challenging due to clinical, morphological and immunphenotypical overlap with unusual reactive processes termed "pseudolymphomas." PATIENT CONCERNS: 45-year-old male that underwent surgical debridement for a necrotizing fasciitis of the thigh with concomitant excision of a regional lymph node. DIAGNOSES: The lymph node demonstrated an architecture-effacing activation and proliferation of lymphoblasts and was initially misdiagnosed as an aggressive lymphoma. Only in consideration of the clinical context and with the help of ad…

Lactate-Based Scoring System in the Diagnosis of Necrotizing Fasciitis.

Necrotizing fasciitis (NF) is a rare and quickly progressing infection and leads to 100% mortality if untreated. Quick diagnosis and an early and radical surgical treatment are essential for stopping bacterial progression. Unfortunately, the absence of clear clinical signs makes the diagnosis often challenging. Therefore, we searched for easy determinable predictive laboratory markers for NF. This is the first study which includes lactate values in a new score. A retrospective analysis of patients with NF (n = 44) and patients with erysipelas (n = 150) was performed. Lactate values, patients' …
