Necrotizing fasciitis is a fatal, rapidly progressive, often initially unrecognized condition. Mortality rates range from 30% to 76%. Prognosis depends on the delay of diagnosis, antimicrobial treatment and surgical excision of all necrotic tissue. A case of postpartum perineal necrotizing fasciitis arising from episiotomy is presented. Prompt recognition and aggressive therapy resulted in a favorable outcome despite significant morbidity.

A 28-year-old woman, 1 month post partum, who presented with headache and gradual onset of total ophthalmoplegia of the left eye, was found at transsphenoidal exploration to have a primary intrasellar germinoma. Craniospinal irradiation resulted in complete resolution of the intracranial tumor. Extensive mediastinal metastasis at 8 months after the operation resulted in a fatal outcome.

A middle aged man was hospitalised in the state of acute renal failure with erythema nodosum lepromum. He was having progressive anaemia, oliguria, azotemia and impaired renal function. The outcome was fatal. Autopsy revealed smooth congested kidney with histopathological features of rapidly progressive - (crescentric) glomerulo-nephritis, presumably a result of immune-complex deposition from recurrent erythema nodosum leprosum episodes.

The authors report a new case of induced pemphigus: a 64-year-old man has had rheumatoid arthritis for 10 years. When he developed his pemphigus, he had been taking D-penicillamine and piroxicam for about 8 months. He needed high doses of prednisolone associated with azathioprine to clear his eruption. Mucormycosis is an uncommon opportunistic fungal infection caused by Zygomycetes. It usually affects immunocompromised, diabetic and trauma patients with infected wounds. Visceral leishmaniasis is a highly morbid and incapacitating infection, which usually presents with prolonged fever, weight loss and hepato-splenomegaly. Despite the availability of effective treatment, the disease can have a high mortality even at referral centers. A review study examined the clinical course of measles diagnosed in children being treated for malignant disease in Newcastle upon Tyne during 1973-86. Of the 17 cases diagnosed, five were fatal.
