PURPOSE: Femoral neck fractures are common injuries with over 1 million occurring yearly. This video demonstrates a case of a displaced femoral neck fracture treated with a cemented hemiarthroplasty through a posterior approach.
METHODS: Multiple approaches to the hip for arthroplasty have been described, each with advantages and disadvantages. Careful technique is critical to success.

We report about an 81-year-old male patient who suffered a medial femoral neck fracture after a fall on the left leg. A total hip arthroplasty was planned based on the appropriate indications. The planning was complicated by the presence of an intramedullary Küntscher femoral nail inserted after a motorcycle incident approximately 60 years ago. After fenestration of the proximal lateral femur, the nail was sawn off and a cemented total hip arthroplasty and plate osteosynthesis were performed.

The authors consider specific general status of 23 patients aged from 26 through 63 years who underwent operations of total endoprosthesis of the hip joint for aseptic necrosis of the femoral bone head (18 cases) and pathological fracture of the femur neck (5 cases). Out of 23 patients 11 were given dialysis treatment and 12 underwent allotransplantation of the kidney. A technique of complex preoperative management is described that allowed avoidance of general peri- and postoperative complications.

BACKGROUND: The importance of regional anesthesia techniques is increasing as part of multimodal analgesia strategies for managing postoperative pain in pediatric hip and femur surgeries.
CASE PRESENTATION: We aim to present two cases, aged 10 years and 18 months, scheduled for surgery due to femoral neck fractures. As part of multimodal pain management, ultrasound-guided PENG + LFCN blocks were performed in both cases. Postoperative pain was assessed using VAS and FLACC scales.

Distal greater trochanteric transfer is a rarely used procedure for treatment of hip deformity. Surgical technique is described. Potential for changing morphology of proximal end of the femur is underlined and pitfalls indicated. The main advantage of the procedure is reconstruction of the femoral neck. It is biomechanically more sound than valgus osteotomy done frequently in similar cases. Advantages of trochanteric transfer are confirmed by other authors who after this procedure achieved clinical and radiological improvement in patients with degenerative disease of the hip.

The authors comment on the results of treatment of 40 children with inveterate pathologic dislocation of the hip caused by meta-epiphyseal osteomyelitis. Conservative setting of the dislocation which they carried out in children aged below 3 years by the methods described in the article was positive in a little less than half of the patients with preserved or only partly destroyed head and neck of the femur. When conservative treatment is not successful, the choice of the method of surgical intervention is determined by the degree of the morphological changes in the hip joint.

In congenital dislocation of the hip, growth-disturbance lines as shown radiographically regularly occur in the proximal end of the femur after operative procedures are performed under general anesthesia. They can be seen in children who are between the ages of six months and four years and tend to disappear over several years. I think that the final shape of the femoral neck can be predicted by closely observing the development and displacement of the growth-disturbance lines that are induced at the time of surgical treatment.
