BACKGROUND: Syndesmotic injuries of the ankle without fractures can result from external rotation, abduction and dorsiflexion injuries. Kinematic studies of these trauma mechanisms have not been performed. We attempted to describe the kinematics of the tibiofibular joint in cadaveric specimens using radiostereometry after sequential ligament sectioning, and resulting from different trauma mechanisms and axial loading, in order to put forward treatment guidelines for the different types of syndesmotic injuries.
METHODS: We assessed the kinematics of the distal tibiofibular joint in fresh-froze

PURPOSE: The purpose of this study was to examine whether the results of a reconstruction using frozen autograft in combination with vascularized fibula are comparable to other reconstructive methods in limb-salvage surgery for tibial sarcoma with regard to the functional outcome and complications.
METHODS: Between 2008 and 2012, nine patients with bone sarcoma of the tibia underwent excision of the affected segment that was then frozen and reimplanted with an ipsilateral vascularized fibular graft within it. Patients were examined clinically and radiographically.
RESULTS: The mean follow-up

PURPOSE: To determine the effectiveness of fixed, fiber-reinforced resin bridges on 4 ultrashort implants with a sufficient implant survival success rate of at least 90% in highly atrophic jaws.
MATERIALS AND METHODS: A prospective temporal cohort study was designed, with the inclusion of 10 patients. Of the 10 patients, 8 had atrophic jaws (7 women, 1 man; mean age at implant placement 58.5 years, range 40.7 to 73.9) and 2 had fibula-mandibular transplants (1 woman, aged 76.5 years; and 1 man, aged 69.3 years). All 10 patients had their dentition restored with 4 ultrashort implants. The impl

PURPOSE: During open-wedge high tibial osteotomy, fracture occurring in the insufficient osteotomy before distraction of the osteotomy gap is an important complication. The objective of this study was to evaluate bone mineral density (BMD) around the proximal tibiofibular joint (PTFJ) and the osteotomy hinge. The hypotheses of this study were (1) BMD would be higher in the level of PTFJ, compared with that of above--or below--the level of PTFJ, (2) BMD of the posterolateral side of the hinge would be higher than that of the anterior or lateral side.
METHODS: Computed tomography was used to de

Pilon fractures are complex injuries involving the weight-bearing articular surface of the distal tibia and are frequently associated with relevant soft tissue damage. The treatment requires a structured strategy in which the condition of the soft tissues plays a central role. These fractures result either from low-energy rotational injuries or high-energy axial compression trauma. While the former typically occur in older patients and are associated with limited soft tissue injury, the latter often lead to complex fracture patterns with severe articular impaction and metaphyseal comminution.

Reconstructive surgery of the clavicle using free vascularised fibula grafting (FVFG) is sometimes required for the management of severe bone loss or non-union. As the procedure is relatively rare, there is no universal agreement on the management and outcome. This systematic review aimed to first, identify the conditions for which FVFG has been applied; second, to gain an understanding of the surgical techniques used; and third, to report outcomes related to bone union, infection eradication, function and complications. A PRISMA strategy was used. Medline, Cochrane Central Register of Control

The tibiofibular syndesmosis is a ligamentous structure connecting the distal ends of the tibia and fibula. This article presents and compares the available methods of treatment of syndesmosis injuries. Current publications from the online database PubMed and relevant available books are analysed and compared with AO Surgery guidelines. Metal and bioabsorbable syndesmotic screws and the suture button technique, including all possible modifications of these approaches, are compared. The focus is on the technical aspects of the procedures, functional outcomes and complications of these procedure

Masquelet technique is one of the modalities for the treatment of long bone defect. Using cancellous bone graft to fill the bone defect is always a concern in children due to the small size of their iliac crest and open growth plate. We reported a case of 13-year-old male who presented with gap non-union of middle third of tibia. We applied a modified Masquelet technique by using only the cortical fibular graft instead of cancellous bone to fill the space surrounded by induced membrane. Fibula was used as a nonvascularized strut graft and matched stick graft to achieve complete union. We concl

Fibulo-scapho-lunate fusion is a technique that allows residual movement in the wrist in case of wide bone resection replacing the distal radius by a vascularised fibular transfer. Some authors have used this technique with favourable results but the distal synthesis seems to not be standardised at all, many different osteosynthesis methods have been proposed. This paper reports a complete review of the present day literature about this subject and, evaluating the different proposed osteosynthesis techniques referred in literature, suggests a standardization of the synthesis methods with dorsa

BACKGROUND: Although a remarkable success in the treatment of congenital pseudarthrosis tibia (CPT) had been achieved, failure rate is still high and the likelihood of amputation is still considerable. The current study evaluates the outcome of induced membrane technique in the treatment of failed previously operated patients of congenital pseudarthrosis of the tibia. We hypothesized that induced membrane technique will improve union rates in CPT with failed previous multiple operations.
PATIENTS AND METHODS: Nineteen consecutive patients of failed previously operated CPT were prospectively i

BACKGROUND: Pathogenesis of ankle disorders with a large bone defect is varied and treatment for those problems is challenging for orthopaedic surgeons. The aims of this study were to present an operative technique of arthrodesis for the reconstruction of the ankle and hindfoot with a large bone defect using a soft tissue-preserved fibular strut graft and to report outcomes of consecutively treated patients.
METHODS: Twelve feet from 11 patients, including feet with aseptic necrosis of the talus and total ankle implant loosening, were treated. Bone defects were filled using iliac bone and/or

Internal fixation is widely used in the operative treatment of fractures, but various problems, such as local osteoporosis, may occur when living bone is fixed with a rigid metal plate. We propose a new method of immobilisation, which we have termed "cushioned plate fixation", in an attempt to solve some of these problems. Cushioned plate fixation involves a silicone rubber cushion placed between the steel plate and the bone. On the basis of data obtained from animal experiments in vivo and mechanical tests in vitro, we conclude that the cushion is effective in preventing osteoporosis without

BACKGROUND: Pediatric acute compartment syndrome (PACS) is a challenging condition to diagnose and can result in significant morbidity for children. Our aims were to: (1) determine if overnight observation is warranted in isolated diaphyseal tibial fractures; (2) identify risk factors of PACS.
METHODS: Acutely managed children with isolated tibial diaphyseal fractures +/- fibula fractures from 2012 to 2023 were reviewed. This cohort was cross-referenced with a list of children undergoing fasciotomies. Demographic information and the mechanism of injury were documented. Fractures were classifi…

INTRODUCTION: Open fractures in the elderly are distinct compared to younger populations. The purpose of this study is to follow a series of open fractures of the lower extremity in the geriatric population to better prognosticate outcomes.
METHODS: We performed a retrospective chart review of patients over the age of 65 years old who were treated for an open, lower extremity fracture across two level I trauma medical systems. Patients were included if they had documented wound healing problems in the postoperative period, or 6 months of follow-up, or if they had a definitive radiographic out

INTRODUCTION: Escherichia hermannii is an infrequent monomicrobial pathogen in human infections. Notably, it has never been documented as the sole isolate in cases of infected radial shaft fractures.
CASE REPORT: This report presents a case of a non-union in a distal third radius shaft fracture associated with an E. hermannii-infected implant that remained in situ. The patient, who suffered a crush injury 3 months prior, initially underwent open reduction and internal fixation using a fibula strut graft secured with a titanium plate, along with primary wound closure. However, after 12 weeks,
