A novel technique for proximal interphalangeal joint fracture-dislocations: the second toe middle phalanx osteochondral graft.

Many different surgical techniques have been used to treat unstable dorsal proximal interphalangeal joint fracture-dislocations. The authors have used the base of the middle phalanx of the second toe base as an alternative autograft to treat this type of injury. This retrospective study assessed the clinical outcomes of this procedure in 11 patients. Range of motion, grip strength, Disability of the Arm, Shoulder and Hand score and donor site morbidity were assessed at regular intervals postoperatively. Nine patients had acute injuries and two had chronic injuries. The mean range of motion in

Portable mini-fluoroscopy improves operative efficiency in hand surgery.

A paired case cohort study was performed using retrospective review of operative times for defined hand surgical procedures in an attempt to quantify efficiency with and without the use of portable fluoroscopy. Patients included in the study underwent 1 of 4 defined surgical procedures controlled to ensure similar operative technique (total wrist fusion, in situ 4-corner fusion, closed reduction/internal fixation using K-wires of phalangeal shaft fractures, and metacarpophalangeal or interphalangeal joint fusions using K-wires). One group used intraoperative standard film radiographs and the o

Flexible-implant arthroplasty and finger replantation.

We performed flexible-implant arthroplasty in three metacarpophalangeal joints in two patients at the time of finger replantation. We believe flexible-implant arthroplasty is preferable to permanent arthrodesis for most patients.

Comparison of Ultrasonographic Features Among Healthy Individuals and Patients With Trigger Finger.

PURPOSE: The trigger finger is one of the leading causes of finger pain and triggering. The flexor tendons are stabilized by a fibrous band called the annular pulley. The first annular pulley (A1 pulley) is located at the level of the metacarpophalangeal joint. Trigger finger is characterized by thickening of the A1 pulley. There is a lack of clinical and ultrasonographic risk factors that may help clinicians. The main purpose of the study was to measure the thickness of the flexor tendon and A1 pulley in patients with trigger finger compared to healthy volunteers and to reveal risk factors of

Assessment of movement time in patients with essential tremor.

BACKGROUND: We have observed mild bradykinesia in essential tremor (ET) patients, which do not satisfy the criteria of Parkinson's disease (PD). OBJECTIVE: To compare the mean movement time for repetitive movements around distal and proximal joints in ET patients with normal controls by using a simple test paradigm. PATIENTS AND METHODS: Seventeen patients with ET and 14 control subjects were instructed to tap with the index finger sequentially on push-button microswitches. Movement times around metacarpophalangeal, wrist, elbow, and shoulder joints of the right side were tested. The data co

Immediate free flap reconstruction of traumatic defects.

In a series of 180 transfers of free flaps were in 11 cases on upper extremities applied emergency flaps, both for immediate reconstruction of function and for the replacement and covering of tissue defects. In one case a scapular flap was used simultaneously with the replantation of the forearm. In three cases was applied a forearm flap for the reconstruction of the thumb and of the scalped index finger, and in three cases a central venous flap for traumatic defects of tissues on fingers. These flaps yielded surprisingly good functional results after an injury associated with a loss of dorsal

"Trigger finger" secondary to partial rupture of the superficial flexor tendon.

A case of "trigger finger" due to flexor digitorum superficialis tendon "tag" following a hyperextension injury to the metacarpophalangeal joint is reported. The tag formed as a result of avulsion of the flexor digitorum superficialis tendon slip from its insertion in the middle phalanx.

Associations between MRI-defined synovitis, bone marrow lesions and structural features and measures of pain and physical function in hand osteoarthritis.

OBJECTIVES: To explore associations between MRI features and measures of pain and physical function in hand osteoarthritis (OA). METHODS: Eighty-five patients (77 women) with mean (SD) age of 68.8 (5.6) years underwent contrast-enhanced MRI of the interphalangeal joints (dominant hand) and clinical joint assessment. One investigator read the MRIs for presence/severity of osteophytes, joint space narrowing, erosions, bone attrition, cysts, malalignment, synovitis, flexor tenosynovitis, bone marrow lesions (BMLs) and ligament discontinuity according to the proposed Oslo hand OA MRI score. Managing Swan Neck and Boutonniere Deformities.

Acute and chronic injuries to the finger extensor mechanism can result in swan neck and boutonniere deformities. Loss of coordination between the multiple, specialized components of the extensor mechanism results in tendon imbalances leading to altered interphalangeal joint flexion and extension forces. Treatments include corrective splinting and operative interventions. Swan neck deformities are functionally limiting. Surgical correction generally results in functional benefit. Boutonniere deformities are functional but aesthetically displeasing; proximal interphalangeal (PIP) joint flexion a

Volar Tilt as a Predictor in Dorsal Proximal Interphalangeal Joint Fracture-Dislocations.

Background: Restoring the volar lip of the middle phalanx is key to regaining concentric joint gliding after dorsal proximal interphalangeal (PIP) joint fracture-dislocations, but no reliable radiographic assessment currently exists. This study introduces volar tilt as a new radiographic measure. Methods: We evaluated 27 dorsal PIP fracture-dislocations in 25 patients, analysing clinical outcomes, osteoarthritis (OA) and volar tilt on lateral radiographs at injury, after extension block pinning and at 16-year follow-up. Results: Initial volar tilt averaged 22°, improving to 12° after extension

Radiological and histological analysis of two replaced interphalangeal joints with active subchondral bone resorption in erosive hand osteoarthritis: a novel mechanism?

The aim of this study was to describe the histological features of erosive hand osteoarthritis (EHOA), which is considered an aggressive subset of hand osteoarthritis (OA) characterized by severe local inflammation and degeneration of the distal and proximal interphalangeal joints. Two patients with EHOA underwent replacement with a cement-free press fit ceramic prosthesis of a proximal interphalangeal joint (PIPJ). Clinical and radiological data were collected and histological examination was performed. Radiological examination with histological correlation showed complete erosion of the arti

Modified Sublimis Tenodesis: Surgical Technique for Treating Chronic Traumatic Proximal Interphalangeal Joint Hyperextension Instability.

Many surgical techniques for treating chronic posttraumatic hyperextension instability of the proximal interphalangeal (PIP) joint involve exploration of the PIP joint, often leading to stiffness in an already traumatized joint. This article outlines the indications, contraindications, surgical technique, and postoperative management for a modified flexor digitorum sublimis tenodesis that utilizes 2 small incisions, a slip of flexor digitorum sublimis, a suture anchor, and temporary pinning of the PIP joint, while avoiding violation of the PIP capsule.

Malunion of fractures of the proximal phalangeal neck in children.

We report three cases of malunion after fractures of the proximal phalangeal neck in children. A 12-year-old boy and a 7-year-old girl had malunited fractures with dorsal and ulnar deviation of their fingers. Several years later remodeling had been achieved well for dorsal deviation but little for the ulnar deviation in each case. A 10-year-old boy had a similar malunited fracture with dorsal and ulnar deviation of his finger. The proximal bony spike, which protruded to the volar side, collided with the middle phalanx during flexion of the proximal interphalangeal (PIP) joint, so we shaved it.

[Clinical anatomy of the digital connective tissue cord of the ulnar side of the little finger].

A fibrous cord with a length of nearly 60 mm and a width of 3 mm consistency starts near the insertion of the abductor digiti minimi muscle at the ulnar border of the little finger. The fibers pass parallel to the longitudinal direction of the little finger and at the level of the osseous ridges on the palmar side of the flexion-extension axis of the finger joints. Proximally the cord receives fibers from the periosteum at the base of the proximal phalanx, from the ulnar end of the natatory ligament, and from the pretendinous fibers of the A1 pulley. Distally the digital cord has connections t…

A Cadaver Study of the Feasibility of Multidigit Allotransplantation for Reconstruction of the Metacarpal Hand.

BACKGROUND: Amputation of all fingers (metacarpal hand) can be functionally equivalent to hand amputation. Multidigit allotransplantation can benefit patients who require a full complement of fingers to return to their preinjury activities. The authors investigated the feasibility of the transfer of four fingers and thumb as a single allograft. METHODS: Four fingers and the thumb were harvested from 16 hands as an allograft based on the radial and ulnar arteries. Dorsal digital veins were dissected until confluent in the major veins. The common digital nerves were divided at the origin. The f

Dorsal transosseous reduction and locking plate fixation for articular depressed middle phalangeal base fracture.

PURPOSE: An articular depressed fragment at the base of the middle phalanx can be an obstacle to congruent reduction and stable fixation. This study assessed the outcomes of a transosseous reduction technique combined with locking plate fixation for the treatment of articular depressed middle phalangeal base fracture. METHODS: Between 2015 and 2017, seven patients (eight fingers) with intraarticular comminuted middle phalangeal base fracture were included in this study. Mean follow-up was 19 months (range 12-30 months). All patients showed depression of the articular fragment on sagittal comp
