Mechanical characteristics of cortical bone pins designed for fracture fixation.

Pins constructed from cortical bone may provide a reasonable alternative to other fracture-fixation devices by circumventing some of the complications associated with stainless steel and synthetic biodegradable implants. However, it is unknown whether cortical bone pins provide comparable strength compared to conventional pins. Using four-point bending, we compared the mechanical characteristics of 1.2-mm allogeneic cortical bone pins milled from specific regions of human tibiae and femora to commercially available 1.1-mm diameter stainless steel pins and 1.3-mm diameter polydioxanone pins. We

[Conservative treatment of fractures of the long bones during the growth phase].

In the treatment of fractures of the long bones in children and adolescents, surgical and conservative methods are not mutually exclusive alternatives; rather, each can complement the other in the overall treatment strategy. Many operative procedures, such as fixation of juxtaarticular, metaphyseal fractures, need to be supplemented by conservative procedures, such as immobilization by means of casts. We refer to methods that are performed without anaesthesia and do not involve the implantation of osteosynthetic foreign material as conservative. These are: immobilization with no further treatm

The effect of parathyroidectomy on bone fracture risk in patients with primary hyperparathyroidism.

BACKGROUND: Parathyroidectomy may increase bone density in primary hyperparathyroidism (PHPT), but it is unclear whether fracture risk is decreased. HYPOTHESIS: Parathyroidectomy decreases fracture risk. DESIGN: Retrospective cohort study with median follow-up of 6.5 years. SETTING: Twelve regional hospitals in California. PATIENTS: One thousand five hundred sixty-nine patients with PHPT. INTERVENTIONS: Parathyroidectomy or observation. Main Outcome Measure Fracture-free survival. RESULTS: Mean initial calcium, parathyroid hormone, and creatinine levels were 11.2 mg/dL (2.8 mmol/L), 123.

Mechanics and mechano-biology of fracture healing in normal and osteoporotic bone.

Fracture repair, which aims at regaining the functional competence of a bone, is a complex and multifactorial process. For the success of fracture repair biology and mechanics are of immense importance. The biological and mechanical environments must be compatible with the processes of cell and tissue proliferation and differentiation. The biological environment is characterized by the vascular supply and by many biochemical components, the biochemical milieu. A good vascular supply is a prerequisite for the initiation of the fracture repair process. The biochemical milieu involves complex int

Longitudinal bone insufficiency fracture of the tibia in a renal transplant recipient.

We report a case of longitudinal tibial fracture as the first manifestation of bone insufficiency in a 50-year-old patient who had received a renal transplant 12 years earlier. The epidemiological, clinical, and imaging features of these fractures are reviewed. Bone loss occurs in the long term in about half of renal transplant recipients. The main causes are preexisting renal osteodystrophy; glucocorticoid therapy; and hyperparathyroidism, whether residual or secondary to imperfect graft function. The effects of cyclosporine therapy on bone metabolism remain unclear. Identification of patient

Aneurysmal bone cyst following tibial fracture: a case report.

A 14 year-old boy developed a tibial aneurysmal bone cyst (ABC) following a closed tibia fracture. The tumor formed in a site remote from the fracture and was not radiographically apparent until one year following the traumatic event. Most ABC's present due to fracture or pain, but this lesion was discovered during routine follow up films of the tibial shaft fracture. This case lends support to the debated theory that ABC's are reactive bone lesions and provides a rare radiographic glimpse at the lesion's early rate of development.

[Healing fibular bone fracture in rats during extended suspension ].

Healing of peroneal bone fracture was studied histologically and histomorphologically in rats exposed to a 30-d tail-suspension. To fracture bones, anesthetized rats were operated on day 15 of suspension. Hind limb deprivation of the support loading for a period of 30 days minimizes healing processes in fractured peroneal bones as was evidenced by undersized callus and lowered strength of bone consolidation. Mass-metric and histomorphometric investigations showed that, as compared with the control, on day 15 of regeneration the callous mass and volume in the suspended rats were by 34% and 35%

Clinical and experimental study on facial paralysis in temporal bone fracture.

OBJECTIVE: To study the main prognostic factors and significance of facial nerve decompression for facial paralysis in temporal bone fracture. METHODS: The main relative prognostic factors of 64 patients with facial paralysis were analyzed. An experimental model of facial paralysis was made. The expansion rates of facial nerve in the facial canal opening group and the facial canal non-opening group were measured and observed under electron microscope. RESULTS: The main factors affecting the prognosis were facial nerve decompression and selection of surgery time. The expansion rate of facial

Postmenopausal osteoporosis. Strategies for preventing bone loss, avoiding fracture.

Implementing osteoporosis prevention strategies through lifestyle modification and pharmacologic treatment reduces patient morbidity and mortality, as well as the cost to the healthcare system. All women should be advised about their risk factors and educated about options to reduce the risk. The primary care physician plays a crucial role in identifying women at risk who are most likely to benefit from intervention. Treatment decisions should be based on each woman's medical history and personal preferences. The optimal duration of treatment, especially with the newer agents, such as the SERM

The effect of alendronate on bone mass after distal forearm fracture.

Fracture and immobilization of an extremity lead to bone loss at the fracture and at adjacent sites. We conducted a 1-year, single-center, prospective, randomized, double-blind study to determine whether bone loss would occur in the distal radius after a Colles' fracture and whether this loss could be prevented using an antiresorptive drug (alendronate). Thirty-seven women with a recent fracture of the distal forearm and low bone mineral density (BMD) of the lumbar spine were randomized to receive either 10 mg alendronate daily or placebo. BMD of both forearms was measured at baseline and afte

Spinal trabecular bone loss and fracture in American and Japanese women.

This study examined trabecular bone mineral density (BMD) in Japanese women with and without spinal fracture, and compared the results to American women with and without fracture. The quantitative computed tomography (QCT) systems used at the University of California, San Francisco (UCSF) and at Nagasaki University were cross-calibrated. Normative BMD was assessed with the K2HPO4 liquid phantom in 538 Americans aged 20-85 years, and with the B-MAS200 phantom in 577 Japanese aged 20-83 years. These BMD were adjusted for use with the Image Analysis solid phantom using the result of cross-calibra

Cavernous sinus thrombosis following manipulation of fractured nasal bones.

Septic cavernous sinus thrombosis is a serious infective condition with a high mortality and morbidity. We report the first case to our knowledge of septic cavernous sinus thrombosis following manipulation of fractured nasal bone under general anaesthesia. The patient later developed complications of pneumonia with pleural effusion, a vegetative lesion of the mitral valve, and blindness.

Negative radioisotope bone scan in a patient with a fractured neck of femur.

Radioisotope bone scanning is a sensitive and specific method of detecting occult fractures of the femoral neck but may occasionally give false negative results. It would appear that patients with renal failure are more likely to have a false negative test and this is probably related to avascularity of the femoral head.

[Treatment of fractures of long bones associated with extensive skin avulsion].

The work analyses the experience in the treatment of 46 patients with fractures of long tubular bones and extensive wounds with avulsion of the skin at the department of emergency traumatology from 1983 to 1988. The method of choice in the treatment of fractures of the long tubular bones in this category of patients is functionally-stable osteosynthesis mainly with external fixation apparatuses on a nail or rod basis. A rational succession of the manipulations for surgical debridement of the extensive wound was determined, which made the surgeon's work much easier and reduced the time needed f

Isolated hyoid bone fracture: a case report and review of the literature.

Isolated hyoid bone fractures secondary to trauma other than strangulation are rare and have few, if any, complications. We report a case of an isolated hyoid bone fracture that resulted in sudden and severe upper airway compromise. Diagnosis of these fractures is difficult and usually can be made only with a strong degree of suspicion. Patients who suffer neck injuries in whom a hyoid fracture is diagnosed or strongly suspected should be observed for 48 to 72 hours, considered for endoscopy, and treated as warranted.

[Choice of the method of treatment of unilateral fractures of the femur and bones of the shin].

The authors present their experience in the treatment of 74 patients with unilateral fractures of the femur and the bones of the crus. The peculiarities of these injuries which unfavourably affect the results of the treatment are discussed. Four versions of treatment are distinguished, with surgical treatment of fractures of the femur and the bones of the crus being the preferable one. The best results, concerning both the duration of the period of in-patient treatment and the final outcome, were obtained in cases of combined intraosseous osteosynthesis of the femur and perosseous osteosynthes

Histopathology of the facial nerve after longitudinal temporal bone fracture.

The temporal bone histopathology in a patient who suffered repeated head trauma and a longitudinal temporal bone fracture shows unusual features associated with the facial nerve. In the distal meatal segment, a type of traumatic neuroma was found with disorganized nerve bundles and distinct areas of Schwann's cell proliferation but lacking fibrosis. Periosteal new bone formation in the labyrinthine segment narrows the fallopian canal and protrudes into the nerve, which completely fills the canal. The tympanic and mastoid segments of the nerve show severe degeneration of nerve fibers and an inc

[Fracture of the scaphoid bone--surgical treatment, indications, method and results].

In fractures of the scaphoid bone, conservative treatment leads to an osseous consolidation in 95% of all cases. A primary surgical treatment seems justified in case of instability by torsion, tilting, or shifting of the fragments, associated fractures of the same arm, as well as de Quervain dislocation fracture. The different surgical possibilities are described and the complications are compiled in a special study. The question if a recent fracture of the scaphoid bone should be managed by conservative or surgical treatment ist not justified considering the rate of 14.5% of complications, i.
