Recent guidelines for dyspepsia, defined as pain or discomfort centred in the upper abdomen, emphasize that in younger patients with no alarm features and not taking nonsteroidal anti-inflammatory drugs, testing for Helicobacter pylori and treatment of the infection if present is a standard of care. If H. pylori is not present, empirical management (e.g. acid suppression) is often prescribed. A 42-year-old woman presented with epigastric pain and vomiting. Upper gastrointestinal endoscopy revealed three gastric ulcers. Histologic examination of biopsies from the ulcers showed cytomegalovirus inclusion bodies. The appearance of IgM antibodies to cytomegalovirus indicated a recent and primary infection. Antral biopsy specimens from patients with acid peptic disease were cultured in parallel on Skirrow medium (SM) and sheep blood agar (SBA) for Campylobacter pylori. In institution 1, 23 of 88 (26.1%) endoscopies were positive on SBA compared with 37 of 88 (42.0%) on SM (P = 0.0001). In institution 2, 29 of 49 (59.2%) endoscopies were positive for C. pylori on SBA and 27 of 49 (55.1%) were positive on SM (P = 0.50). The addition of SM optimizes the recovery of C. pylori from antral biopsy specimens.

Despite the ubiquity and varied manifestations of Ascaris lumbricoides infestation in man, gastric ascariasis is rare. Herein, three patients are described all of whom presented with features of chronic intermittent gastric outlet obstruction caused by roundworms inhabiting the stomach. The ascarides were demonstrated in the stomach by radiology in two patients and by endoscopic examination in one. The possible explanations for the rarity of "gastric ascariasis" include the "housekeeping" peristaltic activity and the acid milieu of the stomach.

A 63-year-old female presented with upper epigastric pain. On endoscopy a giant gastric ulcer was observed on the lesser curvature of the stomach. Biopsies taken from the ulcer showed epitheloid cell granulomas with caseation, suggesting a diagnosis of granulomatous gastritis probably tubercular in origin. No AFB was seen in the biopsied material, the culture also failed to grow AFB. Radiograph of the chest was normal. On anti-tubercular treatment the patient showed remarkable recovery and the ulcer healed completely following 3 1/2 months of treatment.

The authors present the results of 35 patients with alcaline gastritis submitted to truncal vagotomy and jejunostomy by Roux in technic. Preoperative diagnosis was founded in clinical symptoms and endoscopy with histopathology. In a follow-up of 84,8% of the patients, clinical results were excellent or good in 92,8%, showing relief of pain and vomits, gastric endoscopy almost normal and on histopathology.
