Variation in the use of 12-lead electrocardiography for patients with chest pain by emergency medical services in North Carolina.

BACKGROUND: Prehospital 12-lead electrocardiography (ECG) is critical to timely STEMI care although its use remains inconsistent. Previous studies to identify reasons for failure to obtain a prehospital ECG have generally only focused on individual emergency medical service (EMS) systems in urban areas. Our study objective was to identify patient, geographic, and EMS agency-related factors associated with failure to perform a prehospital ECG across a statewide geography. METHODS AND RESULTS: We analyzed data from the Prehospital Medical Information System (PreMIS) in North Carolina from Janua

Total Excess Mortality Surveillance for Real-Time Decision-Making in Disasters and Crises.

Crises such as Hurricane Maria and the coronavirus disease 2019 (COVID-19) pandemic have revealed that untimely reporting of the death toll results in inadequate interventions, impacts communication, and fuels distrust on response agencies. Delays in establishing mortality are due to the contested definition of deaths attributable to a disaster and lack of rapid collection of vital statistics data from inadequate health system infrastructure. Readily available death counts, combined with geographic, demographic, and socioeconomic data, can serve as a baseline to build a continuous mortality su

IASM: A System for the Intelligent Active Surveillance of Malaria.

Malaria, a life-threatening infectious disease, spreads rapidly via parasites. Malaria prevention is more effective and efficient than treatment. However, the existing surveillance systems used to prevent malaria are inadequate, especially in areas with limited or no access to medical resources. In this paper, in order to monitor the spreading of malaria, we develop an intelligent surveillance system based on our existing algorithms. First, a visualization function and active surveillance were implemented in order to predict and categorize areas at high risk of infection. Next, socioeconomic a

Area-level global and local clustering of human Salmonella Enteritidis infection rates in the city of Toronto, Canada, 2007-2009.

BACKGROUND: Salmonella enterica serotype Enteritidis (S. Enteritidis) remains a major foodborne pathogen in North America yet studies examining the spatial epidemiology of salmonellosis in urban environments are lacking. Our ecological study combined a number of spatial statistical methods with a geographic information system to assess area-level heterogeneity of S. Enteritidis infection rates in the city of Toronto. METHODS: Data on S. Enteritidis infections between January 1, 2007 and December 31, 2009 were obtained from Ontario's surveillance system, and were grouped and analyzed at the fo

Human rabies in rural Bangladesh.

Rabies is a major public health problem in Bangladesh, where most of the population live in rural areas. However, there is little epidemiological information on rabies in rural Bangladesh. This study was conducted in 30 upazilas (subdistricts) covering all six divisions of the country, to determine the levels of rabies and animal bites in Bangladesh. The total population of these upazilas was 6 992 302. A pretested questionnaire was used and data were collected by interviewing the adult members of families. We estimated that in Bangladesh, 166 590 [95% confidence interval (CI) 163 350-170 550]

Geographic epidemiology of gonorrhoea and chlamydia on a large military installation: application of a GIS system.

OBJECTIVES: The geographic epidemiology of infectious diseases can help in identifying point source outbreaks, elucidating dispersion patterns, and giving direction to control strategies. We sought to establish a geographic information system (GIS) infectious disease surveillance system at a large US military post (Fort Bragg, North Carolina) using STDs as the initial outcome for the model. METHODS: Addresses of incident cases were plotted onto digitised base maps of Fort Bragg (for on-post addresses) and surrounding Cumberland County, NC (for off-post addresses) using MAPINFO Version 5. Socio-environmental determinants of the leptospirosis outbreak of 1996 in western Rio de Janeiro: a geographical approach.

The environmental and social context in which a leptospirosis outbreak took place during the summer of 1996 in the Rio de Janeiro Western Region was examined by using spatial analysis of leptospirosis cases merged with population and environmental data in a Geographical Information System (GIS). Important differences were observed between places where residences of leptospirosis cases are concentrated and other places in the region. Water supply coverage, solid waste collection, sewerage system coverage and flood risk area were the main determining variables from an initial list of ten. The in

The Geography of Acute Cardiac Care: Toward Elimination of Cardiology Deserts in New Mexico.

Cardiovascular disease remains the leading cause of mortality in New Mexico, where access to timely acute cardiac care is substantially influenced by geography. Large portions of the state are characterized by vast distances, limited specialty infrastructure, and a predominantly rural and frontier population, exposing the limitations of population-density-based models of regionalized cardiovascular care. Drawing on clinical experience across both tertiary referral centers and rural hospitals and informed by publicly available state-level mortality and health infrastructure data, this Perspecti

Review on the molecular tools for the understanding of the epidemiology of animal trypanosomosis in West Africa.

The epidemiology of animal trypanosomosis around Bobo-Dioulasso (Burkina Faso, West Africa) benefited a lot in the last years from the progress of molecular tools. The two most used molecular techniques were the polymerase chain reaction for the diagnosis of the disease in cattle and the characterization of the trypanosomes in the host and the vector on one hand, and the microsatellite DNA polymorphism in tsetse flies to study the intraspecific genetic variability of the vector on the other hand. The results obtained in the Sideradougou area during a recent two year survey with these technique

Passively sensing SARS-CoV-2 RNA in public transit buses.

Affordably tracking the transmission of respiratory infectious diseases in urban transport infrastructures can inform individuals about potential exposure to diseases and guide public policymakers to prepare timely responses based on geographical transmission in different areas in the city. Towards that end, we designed and tested a method to detect SARS-CoV-2 RNA in the air filters of public buses, revealing that air filters could be used as passive fabric sensors for the detection of viral presence. We placed and retrieved filters in the existing HVAC systems of public buses to test for the

The influence of travel time on emergency obstetric care seeking behavior in the urban poor of Bangladesh: a GIS study.

BACKGROUND: Availability of Emergency Obstetric Care (EmOC) is crucial to avert maternal death due to life-threatening complications potentially arising during delivery. Research on the determinants of utilization of EmOC has neglected urban settings, where traffic congestion can pose a significant barrier to the access of EmOC facilities, particularly for the urban poor due to costly and limited transportation options. This study investigates the impact of travel time to EmOC facilities on the utilization of facility-based delivery services among mothers living in urban poor settlements in Sy

Spatio-temporal clustering of American Cutaneous Leishmaniasis in a rural municipality of Venezuela.

BACKGROUND: American Cutaneous Leishmaniasis (ACL) is an endemic disease in most Latin-American countries and a public health problem. The number of new cases in the world is thought to be about 1.5 million each year. A new epidemiologic pattern has been observed in the last years, in this sense, Geographic Information Systems (GIS) combined with methods of spatial analysis provide powerful new tools for understanding it. STUDY OBJECTIVE: To investigate the spatial and temporal features of American Cutaneous Leishmaniasis, in an area known to be endemic. METHODS: We retrospectively conducted…

Satellite imaging and vector-borne diseases: the approach of the French National Space Agency (CNES).

Tele-epidemiology consists in studying human and animal epidemic, the spread of which is closely tied to environmental factors, using data from earth-orbiting satellites. By combining various data originated from satellites such as SPOT (vegetation indexes), Meteosat (winds and cloud masses) and other Earth observation data from Topex/Poseidon and Envisat (wave height, ocean temperature and colour) with hydrology data (number and distribution of lakes, water levels in rivers and reservoirs) and clinical data from humans and animals (clinical cases and serum use), predictive mathematical models

Licensing telemedicine: the need for a national system.

The expansion of information technology has shattered geographic boundaries, allowing for extraordinarily increased access to health information and expanded opportunities for telemedicine practice across state boundaries. But despite its recent growth, telemedicine technology remains embedded in a state-based licensure system that places severe limits on its expansion. The current system of medical licensure is based primarily on statutes written at the turn of the 20th century. This system is inadequate to address the emerging medical practices and future uses of medical technology in the te
