[Glaucoma diseases in Rhineland-Palatine 2010. Epidemiology and utilization of healthcare].

BACKGROUND AND PURPOSE: Data on the epidemiology of glaucoma are sparse for Germany. Case registries from the Association of Statutory Health Insurance Physicians (Kassenärztliche Vereinigung) can be used to estimate disease prevalence rates for different types of glaucoma and to illustrate the utilization of glaucoma healthcare. METHODS: Analysis of healthcare data for suspected glaucoma cases and open angle, angle closure, secondary and other types of glaucoma was carried on the basis of case registries from the Association of Statutory Health Insurance Physicians in Rhineland-Palatine for

The high-pressure bleb.

A 45-year-old male presented with a slowly progressive, painless swelling in his right eye for the past 6 months. He had undergone an open globe injury repair 10 years back and an eventful cataract surgery 5 years back. The presenting visual acuity in the affected eye was perception of light with inaccurate projection of rays. The intraocular pressure was 44 mm Hg with advanced glaucomatous cupping. The swelling was identified to be a communicating sub-tenon cyst secondary to scleral wound dehiscence from secondary angle closure glaucoma. Wound re-suturing, cyst excision and diode laser cyclop

Glaucoma in microspherophakia: presenting features and treatment outcomes.

PURPOSE: To report the clinical features, management, and treatment outcomes of glaucoma in microspherophakia. METHODS: Medical records of 159 eyes of 80 subjects with microspherophakia were reviewed. The clinical features at presentation, presence of glaucoma, methods of treatment, and their outcomes were noted. Glaucoma was diagnosed based on intraocular pressure (IOP)≥22 mm Hg on 2 different occasions and/or glaucomatous optic disc damage. Angle closure was defined as occludable angles >270 degrees with or without presence of peripheral anterior synechiae. RESULTS: Glaucoma was diagnosed

[The congener eye in patients with unilateral classical malignant glaucoma].

PURPOSE: To prospectively study in a follow-up period, ranging from 5 to 20 years, "the fate" of the fellow eye in patients with unilateral classical malignant glaucoma. METHODS: The fellow eye of 10 patients had been thoroughly examined immediately after the beginning of an adequate therapy of the unilateral classical malignant glaucoma. RESULTS: In 6 cases the fellow eye was apparently normal and 4 cases exhibited an intermittent primary angle-closure glaucoma. Surgery of the fellow eye was accepted immediately by two patients; in one a peripheral iridectomy was carried out and in the othe

Nail-Patella Syndrome and Glaucoma: A Case Report and Review of the Literature.

Nail-patella syndrome (NPS) is a rare autosomal dominant disease characterized by nail dysplasia, aplastic or hypoplastic patellae, elbow dysplasia, and presence of iliac horns. Renal or ocular abnormalities are also associated with the disease. We report the case of a 57-year-old woman affected by NPS and having haploinsufficiency of the LMX1B gene who experienced severe bilateral chronic angle-closure glaucoma in both eyes and that was successfully managed with a flap-express procedure in the right eye. The left eye had no light perception, and medical treatment was considered. Glaucoma is t

A randomized study of trabeculectomy and subconjunctival administration of fluorouracil in primary glaucomas.

Forty-one adult patients with primary open angle glaucoma and nine adult patients with chronic angle closure glaucoma underwent trabeculectomy in one eye each. Twenty-one eyes with primary open angle glaucoma and four with chronic angle closure glaucoma were randomly assigned to receive four to six subconjunctival injections of fluorouracil for 10 days after surgery. Twenty-five control eyes did not receive fluorouracil. Intraocular pressure was 20 mm Hg or lower in 24 eyes (96%) in the treatment group after mean follow-up of 17.8 months and in 19 control eyes (76%) after mean follow-up of 17.

Glaucoma in India: estimated burden of disease.

Over the last decade the prevalence of glaucoma has been reported by the Vellore Eye Survey, Andhra Pradesh Eye Disease Study, Aravind Comprehensive Eye Survey, Chennai Glaucoma Study, and West Bengal Glaucoma Study. There have been some differences largely because of methodologic variations. We use the reported age and gender stratified prevalence estimates from these studies and the Indian population census estimates to calculate the number of persons with glaucoma or at risk of the disease in the country. On the basis of the available data, we estimate that there are approximately 11.2 mill

Familial nanophthalmos: management and complications.

Nanophthalmos is a rare form of congenital hypermetropia, in which, the individual is at a high risk of developing angle-closure glaucoma. We report a family of nanophthalmos affecting a brother and sister, as confirmed by biometry. Genetic analysis revealed an autosomal recessive mode of inheritance. The sister developed angle-closure glaucoma, which was refractory to medical and laser therapy. She underwent surgical interventions which resulted in complications. The clinical presentation, management, and complications encountered are discussed.

Glaucoma: Diagnosis and Management.

Glaucoma is a group of eye disorders characterized by progressive deterioration of the optic nerve that can lead to vision loss. Primary open-angle glaucoma (POAG) is the most common form in the United States. The risk of POAG increases with age, family history of glaucoma, type 2 diabetes mellitus, hypotension, hypothyroidism, obstructive sleep apnea, cardiovascular disease, and myopia. Up to one-half of patients are undiagnosed because a diagnosis often requires monitoring over years to document changes suggesting POAG. These include a cup-to-disc ratio of 0.3 or greater, intraocular pressur

Quantitative Assessment of Anterior Segment OCT Parameters and Their Association with Intraocular Pressure in Primary Angle-Closure Disease.

Background/Objectives: Glaucoma represents a progressive optic neuropathy and is the leading cause of irreversible blindness worldwide. The disease is classified as primary angle-closure glaucoma (PACG) or as primary open-angle glaucoma (POAG) depending on the iridocorneal angle. Although gonioscopy remains the gold standard investigation, anterior segment optical coherence tomography (AS-OCT) may provide important data about these structures, offering a quantitative assessment of the angle. The purpose of this study is to evaluate AS-OCT parameters in patients with primary angle-closure disea

Outcomes of newly referred patients with suspected angle closure: do we need to redefine the clinical pathways?

BACKGROUND/OBJECTIVES: To investigate outcomes of referrals for suspected angle closure and explore whether anterior segment optical coherence tomography (AS-OCT) can be used to tighten triaging criteria in a glaucoma virtual clinic. SUBJECTS/METHODS: Retrospectively collected data. The first audit (04/2018-03/2019) identified referrals for suspected angle closure without other glaucoma-related findings (primary angle closure suspect (PACS) referrals). All patients underwent gonioscopy. The second audit (04-08/2019) identified patients with suspected angle closure in a virtual clinic. Managem

Case report: Cataract extraction/lensectomy, excisional goniotomy and transscleral cyclophotocoagulation: Affordable combination MIGS for plateau iris glaucoma.

We report the successful treatment of angle closure glaucoma and plateau iris with combined early cataract surgery/lensectomy, goniotomy, and transscleral cyclophotocoagulation. The underlying mechanism of angle closure and plateau iris was addressed with cataract extraction and goniosynechiolysis. Goniotomy was used to enhance trabecular meshwork outflow and lower intraocular pressure, and the "slow burn" transscleral cyclophotocoagulation was safely performed to reduce the size of the ciliary processes. The unique combinations of these procedures restored the iridocorneal angle anatomy to no

Elastosis of the lamina cribrosa in glaucomatous optic neuropathy.

The purpose of this study was to determine whether elastotic degeneration of the elastin component of the lamina cribrosa occurs in optic neuropathy associated with different types of glaucoma. Human optic nerve heads with primary open-angle, neovascular, chronic angle closure and pseudoexfoliation glaucoma, and with varying duration of disease were compared with age-matched normal eyes, using electron microscopy and immunogold labeling of elastin. The percent area occupied by immunogold-labeled elastin material was determined using a digital image analysis system. In all eyes with a history o

Emergencies in glaucoma: a review.

Most severe eye diseases and injuries ultimately require intervention by an ophthalmologist. The urgency of referral depends on various factors, including level of vision loss, duration of symptoms, and presence of comorbid diseases. Of special importance are five acute eye problems in which emergency management by primary care physicians can be critical to visual outcome: high-velocity injuries, chemical injuries, acute angle-closure glaucoma, arteritic ischemic optic neuropathy, and central retinal artery occlusion. This paper will focus on emergencies in glaucoma, especially primary and sec

[Mode of progression of visual field defects and risk factors in glaucoma patients].

A study of glaucoma was conducted at 17 institutions to clarify the mode of progression of visual field defects in specific types of glaucoma, including primary open angle glaucoma (POAG), primary angle closure glaucoma (PACG), and normal tension glaucoma (NTG). Staging of glaucoma was done by the Kosaki Classification or the Aulhorn Classification and the mode of progression was assessed by life-stable method. The progressive and non-progressive groups of patients in each glaucoma stage were compared with respect to age, intraocular pressure, refraction, and optic disc cupping. A total of 656

Late glaucoma after interstitial keratitis.

In a systematic study of 45 patients who had syphilitic interstitial keratitis early in life and, many years later, were discovered to have glaucoma, there was evidence of two different but equally common mechanisms. In one group of patients, a deep-chamber type of glaucoma was superimposed on old inflammatory changes, was characteristically refractory to medical treatment, but did well with filtering surgery. The other group of patients had reversible angle-closure glaucoma associated with anatomically small anterior segments, and typically responded well to iridectomy. The late, refractory,
