BACKGROUND: This study aims to evaluate the outcomes of two-stage bone grafting versus amputation for the treatment of distal phalangeal osteomyelitis.
METHODS: We conducted a retrospective multicenter study of 102 patients with distal phalangeal osteomyelitis, of whom 53 underwent amputation and 49 underwent two-stage bone grafting. Preoperative characteristics were analyzed, including sex, age, BMI, prevalence of diabetes and osteoporosis, infection etiology, and fingers involved. The primary endpoint was infection recurrence. Meanwhile, patient-reported outcomes such as hand function and a

OBJECTIVES: To evaluate the impact of calcium suppression (CaSupp) imaging of spectral computed tomography (SpCT) on fracture detection in clinical routine in the acute trauma setting of the hand and wrist.
MATERIALS AND METHODS: Retrospective inclusion of 125 patients who underwent both X-ray and SpCT examinations of the hand and wrist in an acute trauma setting. Two independent readers evaluated fracture presence on X-ray, conventional CT, and conventional CT plus CaSupp images (time interval 4 weeks, each). Bone bruise (BBr) presence was evaluated on CaSupp images. Sensitivity and specific

BACKGROUND: Fractures represent a significant global public health concern, particularly affecting younger and older populations; however, there is a notable lack of comprehensive epidemiological research focusing on fracture among adolescents and young adults (AYAs).
METHODS: We assessed the fracture burden in AYAs using GBD 2021 data. Concentration curves and indices were used to evaluate socioeconomic inequalities. Trends and projections were estimated using Bayesian age-period-cohort (BAPC) models.
RESULTS: In 2021, the global incidence of fractures among AYAs reached 40.77 million cases

BACKGROUND: The wrist/forearm complex is one of the most commonly fractured body regions, yet the impact tolerance of the wrist is poorly understood. This study sought to quantify the injury tolerance of the adult male forearm-wrist complex under loading simulating axial impact to an outstretched hand.
METHODS: Fifteen isolated cadaveric forearm/wrist specimens were tested. Loading was applied via an instrumented drop tower device designed to impact the palmar surface of the hand with the wrist extended to approximately 90 degrees. Impact severity was modulated by adjusting the boundary condi

Enchondromas are the most common benign cartilaginous bone tumors arising in the medullary cavity of the small bones of the hand. In contrast, chondrosarcomas, commonly occurring in the pelvis and proximal femur and humerus, are very uncommon at this site. We report an unusual case of chondrosarcoma arising in the ring finger proximal phalanx with its radiological and histological features and reviewed the literature. The patient was an 80-year-old man whose chief complaint was swelling and pain for seven years. The findings of cortical irregular thickening by plain radiography and computed to

Creating musculoskeletal models in a paediatric population currently involves either creating an image-based model from medical imaging data or a generic model using linear scaling. Image-based models provide a high level of accuracy but are time-consuming and costly to implement, on the other hand, linear scaling of an adult template musculoskeletal model is faster and common practice, but the output errors are significantly higher. An articulated shape model incorporates pose and shape to predict geometry for use in musculoskeletal models based on existing information from a population to pr

BACKGROUND: Schizophrenia was clinically documented to co-occur with fractures and aberrant bone mineral density (BMD), but the potential causal relationship remained unclear. This study aimed to test the causal effects between schizophrenia and fractures as well as aberrant BMD by conducting Mendelian randomization (MR) analyses.
METHODS: Two-sample MR was utilized, based on instrumental variables from large genome-wide association studies (GWAS) of schizophrenia as exposure, to identify the causal association of schizophrenia with mixed fractures, fractures at different body sites (includin

OBJECTIVE: To explore a new approach to treat old scaphoid fracture by grafting bones from distal radius and fixing absorbable screws.
METHODS: From January 2002 to May 2007, 18 patients with old scaphoid fractures were treated by grafting bones from distal radius and fixing absorbable screws. All of the 18 patients (13 males and 5 females, aged from 17 to 41 years old) had the injury history of the palm of hand touching the ground. They felt tenderness on the anatomical snuff-box and tubercle of scaphoid bone. The grasping force decreased. The range of motion of the wrist was (36 +/- 2) degr

From 1979 to 1995 in our clinic, vascularized bone grafting was performed in 29 patients with large bone defects, established nonunion, congenital pseudoarthrosis, or avascular necrosis in the upper extremity. Four patients had traumatic bone defects, six had posttraumatic nonunions, two had congenital pseudoarthroses, five had amputations, nine had defects following tumor resection, and three had other lesions. Reconstructed sites were the humerus in 7 patients, the radius in 12, the ulna in 2, both radius and ulna in 1, and the metacarpal and phalangeal bones in 7. Donor bones were fibula in

Hypermobility of the trapeziometacarpal joint is commonly considered to be a potential risk factor for osteoarthritis. Nevertheless, the results remain controversial due to a lack of quantitative validation. The objective of this study was to evaluate the effect of joint laxity on the mechanical loadings of cartilage. A patient-specific finite element model of trapeziometacarpal joint passive stiffness was developed. The joint passive stiffness was modeled by creating linear springs all around the joint. The linear spring stiffness was determined by using an optimization process to fit force-d

An ulnar cleft hand is a rare congenital deformity presenting with a cleft between the ring and little finger. It may be associated with the absence of a finger. The goals of treatment are cleft closure, increasing the length of the hypoplastic little finger and preservation of function. We report two patients with ulnar cleft hand without missing fingers. Both underwent cleft closure. One was treated by lengthening of the fifth metacarpal with an external fixator, and the other by finger translocation. Both patients achieved a near normal length of the little finger and a reasonable functiona

Preiser's disease, also known as idiopathic avascular necrosis of the scaphoid, and five-fingered hand are rare hand conditions. In this report, we present a case of a 25 year old female patient who had avascular necrosis of the scaphoid and five-fingered hand.

SEVERE AND COMPLEX TRAUMATIC HAND . Serious and complex traumatic injuries of the hand are multiple tissue injuries (skin, vascular, bone and nerves), requiring urgent surgical management. They are responsible for significant functional and aesthetic sequelae. Their incidence is clearly decreasing thanks to information, prevention and security measures. The absolute emergencies are injections under pressure, amputations-devascularizations, and serious mutilations. The objective of the management is to make a functional and aesthetic hand: restoration of the opposition of the thumb with a key-p

INTRODUCTION: Bone gigantic cell tumours (BGCTs) are locally aggressive benign tumours, which can lead to significant functional impairment due to extensive bone destruction and soft tissue invasion and typically occur in the meta-epiphyseal region of long bones with the distal radius being the third most common site. Advanced BGCTs of the wrist present a significant challenge for the medical staff due to their occurrence in subarticular bone. Bone reconstruction of the hand with 3D assisted engineering has been described with promising results. In this article, we present a novel approach for

Pre- and post-operative radiological appearances of various isolated congenital malformations of the hand are described, as observed in a personal series of 52 cases. Six groups of malformations can be distinguished, and are, in decreasing order of frequency: agenesias, syndactylies, duplications, amniotic bands, arthrogryposis, and macrodactyly.
