The Critical Care Society of Southern Africa Consensus Guideline on ICU Triage and Rationing (ConICTri).

BACKGROUND: In South Africa (SA), administrators and intensive care practitioners are faced with the challenge of resource scarcity as well as an increasing demand for intensive care unit (ICU) services. ICU services are expensive, and practitioners in low- to middle-income countries experience the consequences of limited resources daily. Critically limited resources necessitate that rationing and triage (prioritisation) decisions are routinely necessary in SA, particularly in the publicly funded health sector. PURPOSE: The purpose of this guideline is to utilise the relevant recommendations

Frailty in Cardiorenal Care: A Geriatric Perspective on the 2026 European Society of Cardiology Guidelines.

The 2026 European Society of Cardiology Guidelines for the Management of Cardiovascular Disease and Chronic Kidney Disease provide an integrated framework for cardiorenal risk detection, treatment modification, and health service planning. However, chronological age, kidney function, albuminuria, and conventional cardiovascular risk measures cannot fully capture the clinical heterogeneity of older adults. Frailty, sarcopenia, cognitive impairment, multimorbidity, polypharmacy, and limited life expectancy may substantially alter both the expected benefits and burdens of treatment. Conversely, a

Comprehensive Computed Tomography Evaluation of Gastric Cancer: 2026 Consensus Recommendation From the Korean Society of Abdominal Radiology.

Gastric cancer continues to be a significant global health challenge, particularly in East Asia, necessitating precise imaging for optimal management. Although multidetector computed tomography (MDCT) has become the primary tool for staging and response evaluation, clinical practice is hampered by a lack of standardized protocols and interpretation criteria. To address these needs, the Korean Society of Abdominal Radiology established a consensus-based guideline for gastric cancer CT. Using a modified Delphi process with a panel of experts, 10 key statements were developed and validated with a

Making Alliances With Patients Dependent on Benzodiazepines: A Provider's Experience.

Tens of millions of benzodiazepine (BZD) prescriptions are written annually for the outpatient management of anxiety disorders and insomnia. Many prescribers do not follow published treatment guidelines for these disorders. Psychiatric-mental health nurse practitioners (PMHNPs) regularly meet patients who have been treated with BZDs for years. The dangers posed by outpatient BZD use are recognized, especially among older adults, and their use should be minimized or eliminated. There are multiple manualized approaches to outpatient down-titration of BZDs, but little evidence about which methods

A survey of Canadian urologists' and radiologists' perspectives regarding the use of prostate MRI in biopsy-naive patients.

INTRODUCTION: In biopsy-naive men, guidelines now support magnetic resonance imaging (MRI) to improve detection of clinically significant prostate cancer (PCa), reduce overdiagnosis of insignificant PCa, and avoid biopsy in low-risk cases; however, the demand for MRI will be difficult to meet within the Canadian universal healthcare system. We evaluated Canadian urologists' and radiologists' practices, perceptions, and anticipated barriers to MRI implementation in this setting. METHODS: Separate surveys were iteratively developed for urologists and radiologists to inquire about the use/expect

Risk of Osteoporosis and Anemia in Plant-Based Diets: A Systematic Review of Nutritional Deficiencies and Clinical Implications.

The global shift toward plant-based diets is accelerating, driven by growing awareness of health, environmental, and ethical concerns. While these diets are linked to reduced risks of chronic diseases, emerging evidence highlights potential nutritional deficiencies, particularly in calcium, iron, and vitamin B12, that may compromise bone and hematologic health. This systematic review investigates the relationship between strict plant-based dietary practices and the risks of anemia and osteoporosis. Following PRISMA guidelines, we conducted a comprehensive search across four databases PubMed, S

Coordination of hospital discharge for older people with dementia: insights from politicians and local government officials.

BACKGROUND: After hospital care, older people with dementia often require further health and social care services. Hospital discharge is a complex process in which such support is planned and coordinated. This typically requires collaboration between professionals with different responsibilities representing various authorities to ensure integrated care. In Sweden, the decentralised organisation of health and social care services may lead to variation in discharge procedures and in how collaboration across care providers and authorities is organised. Politicians and civil servants play a centr

Implementing Joint External Evaluations of the International Health Regulations (2005) capacities in all countries in the WHO African region: process challenges, lessons learnt and perspectives for the future.

Following the West Africa Ebola virus disease outbreak (2013-2016), the Joint External Evaluation (JEE) is one of the three voluntary components recommended by the WHO for evaluating the International Health Regulations (2005) capacities in countries. Here, we share experience implementing JEEs in all 47 countries in the WHO African region. In February 2016, the United Republic of Tanzania (Mainland) was the first country globally to conduct a JEE. By April 2022, JEEs had been conducted in all 47 countries plus in the island of Zanzibar. A total of 360 subject matter experts (SMEs) from 88 org
