Intraventricular conduction disturbances and paroxysmal atrioventricular block in a young patient with MELAS.

We present the case of a 36-year-old male patient with MELAS (mitochondrial encephalomyopathy, lactic acidosis and stroke-like episodes) who developed intraventricular conduction disturbances and syncopal episodes due to a paroxysmal atrioventricular block. This case suggests that in MELAS, as well as in other mithochondriopathies, intraventricular conduction disturbances and atrioventricular block can be features of the disease. In our case, progression toward atrioventricular block was rapid, suggesting that in MELAS patients presenting with worsening conduction system anomalies, pacemaker i

Onset of reentrant ventricular tachycardia modulated by variable impulse propagation through the zone of slow conduction.

We report an 18-year-old woman, who had undergone surgical repair of tetralogy of Fallot, and then underwent successful intracardiac recordings within the zone of slow conduction (ZSC) of the reentry circuit on the initiation of premature ventricular contractions (PVC) and reentrant ventricular tachycardia (VT), obtained with multielectrode basket catheter. PVC a) were caused by wavefronts of sinus cycles entering and exiting the ZSC, and b) were not repetitive because the wavefront of the PVC could not reenter the ZSC. Similarly, VT developed after delayed potentials of sinus cycles, and was

Prediction of the atrial flutter circuit location from the surface electrocardiogram.

Identification of atypical atrial flutter (AFL) (non-cavo-tricuspid isthmus-dependent) prior to the electrophysiology laboratory is potentially useful because it allows appropriate procedural planning and enables discussion of the likely success rates and risks of the procedure with the patient. Typical counterclockwise AFL has a stereotypic appearance, the electrocardiogram (ECG) is predictive of the diagnosis in the majority of cases, and ablation procedures are associated with a high degree of safety and success. Atypical right atrial and left AFLs have a highly variable flutter wave morpho

Dual conduction in a Mahaim fiber.

The case of an 8-year-old girl with incessant nonsustained left bundle branch block-like tachycardia refractory to antiarrhythmic drug therapy is reported. Electrophysiologic study revealed the presence of a right-sided accessory atriofascicular pathway. Episodes of nonsustained tachycardia were found to be based upon a dual response in AV conduction over the Mahaim fiber to one P wave. No reentrant tachycardia could be induced. The arrhythmia was cured by catheter ablation targeting a Mahaim potential at the right lateral tricuspid annulus. The findings can be explained by longitudinal dissoc

Shortening of conduction time over arborized atrioventricular accessory pathway with Mahaim fibers physiology just before interruption during radiofrequency ablation.

A 21-year-old woman had paroxysmal wide QRS tachycardia with a left bundle branch block configuration and a retrograde conducted P wave just behind the QRS complex. An electrophysiological study revealed antidromic atrioventricular tachycardia involving an atrioventricular connection with decremental conduction as the anterograde limb and normal atrioventricular node as the retrograde limb. During constant pacing from the high right atrium (HRA) at the cycle length (CL) of 600 ms, the QRS configurations were not identical to those during the wide QRS tachycardia or constant pacing at the CL of

Electrophysiology of the right anterior approach to the atrioventricular node: studies in vivo and in the isolated perfused dog heart.

INTRODUCTION: Previous reports have described electrophysiologic properties and rate-dependent responses in the transitional cell zone of the posterior AV nodal input (slow pathway). The purpose of this study was to investigate the electrophysiology of the anterior transitional cell zone (fast pathway) in vivo and in a Langendorff preparation perfused with a nonblood solution containing butanedionemonoxime to inhibit contraction. METHODS AND RESULTS: In five anesthetized dogs, the His-bundle electrogram recorded from the aortic root included atrial activity in close proximity to the anterior

Mechanisms and clinical manifestations of supraventricular tachycardias.

Supraventricular tachycardia (SVT) is used as a general label for a variety of fast heart rhythms originating in any part of the conduction system above the ventricles. Supraventricular tachycardia encompasses subclasses of tachycardias such as: atrioventricular nodal reentrant tachycardia (AVNRT), atrioventricular reciprocating tachycardia (AVRT), atrial tachycardia, and atrial flutter. Nurses may encounter patients with these common types of dysrhythmias in inpatient or outpatient settings. Supraventricular tachycardia causes numerous symptoms which may make life difficult for these patients

[Multiple pathway tachycardias: electrophysiological properties and fulguration of accessory pathways in 2 cases].

We report two patients (male, 35 and female, 28 years old) with recurrent and refractory tachycardias referred for an electrophysiological study. EKG showed left lateral accessory pathway in both, two tachycardias with identical morphology but with different frequency and activation sequence were detected. One was orthodromic with anterograde conduction by the His-Purkinje budle and retrograde by the accessory pathway and the other with slow nodal anterograde conduction. The accessory pathway was first fulgurated with radiofrequency at the third attempt. Posteriorly, a nodal reentry tachycardi

[Heart arrhythmias in advanced age].

Holter monitoring has revealed that bradycardiac and tachycardic arrhythmias are frequent in apparently healthy subjects aged over 75. Even in elderly patients with known cardiovascular disease, however, such arrhythmias usually do not correlate with the clinical symptoms giddiness, palpitations or syncope. For this reason, prior to possible therapeutic measures, an accurate investigation of the underlying cause and the demonstration of a causal relationship between documented arrhythmias and symptomatology is necessary. Account must be taken of the particular pharmacotherapeutic situation in

Electrophysiologic effects of disopyramide in Chagas' disease with bundle branch block.

We report the acute electrophysiologic effects of disopyramide in 16 asymptomatic chagasic individuals with right bundle branch block. The drug increased the heart rate, prolonged the H-V interval and enhanced the conductivity through the A-V node.

Reentry within the atrioventricular node in man: a reassessment.

6 patients with reentrant supraventricular tachycardia were studied using the atrial extra-stimulus (S1-S2) technique. 3 patients had smooth curves of H1-H2 intervals plotted against A1-A2 intervals, and echoes occurred only in the relative refractory period of the AV node. These patients met traditional criteria for reentry within the AV node, but all 3 were found to have accessory pathways which functioned only in the retrograde direction. 3 other patients had discontinuous curves of H1-H2 intervals plotted against A1-A2 intervals, and the echo zone was confined to coupling intervals produci
